RATION CARD MODIFICATIONS

Ration card data Modifications service is used if any corrections required in the

ration card. In this service we can Address and Member details Modifications. Address Change Allows to Change Mandal, Village/Ward and House no Pincode and FP Shop No Etc. Limitations:- In Member Details Full Name change not Allowed.

Note: The asterisk (*) denotes mandatory requirement of documents. Apply for Ration Card Modification: This section contains instructions for the operators of the Meeseva web application to apply for Ration Card Modifications. 1) In Meeseva home screen, under List of Services, select Civil Supplies Department as depicted in Figure 1.

2) Select Ration Card Modifications Service in Civil Supplies Department as depicted in Figure 2.

3) Now Ration Card Modifications Request Screen will be displayed as depicted in Figure 3.

4) Enter Ration Card no and click get Details button as depicted in Figure 4.

5) It Fetch data from Department Server and display details related to that ration card no. as depicted in Figure 5.

6) Select Address and Member Details Modifications based on Citizen request. Based on Selection Address and Member details panels are generated as depicted in Figure 6.

Note:- Here we choose either Address change/Member Modifications/ Both based on requirement.

7) Enter Modified Address details Such as House No to be Changed, Locality/Land Mark to be Changed, Mandal to be changed, Village/Ward to be changed, Pincode, FP Shop no and Proof document for Address Change as depicted in figure 7.

Note: - Here FP Shop Selection Generated based on Selection of Wish to Change FP Shop.

8) Click Edit link for updating Member details if and modifications required as depicted in figure 8.

9) Enter Modified Member details and Click update link as depicted in figure 9.

10) Select Proof Documents for Member Details modifications as depicted in figure 10.

11) Enter all the Informant details such as informant Name, Relation, Mobile No, Delevery Type and Reason for Ration card Modifications as depicted in Figure 11.

12) Collect the necessary supporting documents as indicated in the Ration Card Modifications request page. scan the documents and upload into the system, as depicted in Figure 12. Note:- Here Proof documents should be uploaded based on Selection.

Note: All the mandatory fields have to be filled in; otherwise request will not be accepted by the system. 13) Click Show Payment. Another window appears requesting to Confirm Payment as depicted in figure 13.

15) Collect the amount from the Citizen and click Confirm Payment to submit the request as depicted in figure 15.

On confirmation, a receipt will be generated as depicted in the following figure. The receipt will contain the Certificate delivery date. Note: Ensure that you have sufficient balance with your service provider (SCA), else the system will not accept the request.

Note: - On submission of the request, Citizen Receives Message. Like your Request for Ration

Card Modifications has been entered vide Application Number, Transaction Id and sent to Dept.

16) On submission, the request will be sent to the work flow of the respective Department for

further processing.

17) Once the request is approved ,then the citizen need to collect the Ration Card from franchisee where he/she applied. Note:- Print Ration Card Service is Used For Printing The Ration Card
