1.0 Introduction

1.1 PurposeThisproceduredescribestheprogramforcontinuousperformanceimprovementintheDZRadiologicalServicesDivision.TheobjectiveoftheprogramistomaintainhighperformanceofDZpersonnelatthestationsbyeffectivetraining,observations,andcoaching.Thegoaloftheprogramistoprovideaccurateperformancefeedbacktoemployees,andprovideastructuredmethodforperformanceimprovement,ifrequired.

1.2 ScopeandApplicabilityThisprocedureappliestobothRadiationProtectionandDecontaminationTechnicians.Itprovidesguidancefortheinitialqualificationofpersonnel,andastructuredapproachtopersonneldevelopment.ItisdesignedtogivetheSiteManageraconsistentmethodtoobserveperformanceintheradiologicalservicesarea,andtoassisttheemployeeinimprovingtheirperformancebyidentifyingperformanceissuesearly,anddevelopingcorrectiveactionplansforimprovement.ThisproceduredescribeshowperformanceissuesaretobecommunicatedacrosstheDZRPfleetquicklytopreventperformanceissuesatothersites.

2.0 GeneralRequirements

2.1 TheSeniorDirectorofRadiologicalServiceswillestablisharoutineconferencecalltodiscussDZsafetyandperformanceatthesites.AllRegionalandSiteManagersareexpectedtoattendthecall.Thepurposeofthecallistodetectperformanceissuesearlythroughobservationsandmakeimmediateimprovements.ProvidingsafetyandperformancedatatootherSiteManagerscanpreventtheissuefromoccurringatotherlocations.

2.2 TheSiteManagershouldcompleteAttachment4,SafetyandPerformanceObservationspriortoeachconferencecall.ThiswillincludeareviewofupcominghighriskworkactivitiesandthespecificactionbeingtakenbytheSiteManagertoensurethoseworkactivitiesarecompletedsuccessfully.Usesection3.4tohelpassesstheriskforupcomingactivities.

2.3 TheSiteManagerisrequiredtoreportallsafetyissuesinaccordancewiththeDZEHSprogram.TheSiteManagershouldcommunicatetotheRegionalManagers,andtheSeniorDirectorofRadiologicalServices,within12hours,ofanyofthefollowingsafetyevents.

2.3.1 Personnelinjurylistedasfirstaidorgreater2.3.2 Nearmisssafetyeventwherepersonnelcouldhavebeenseriouslyinjured2.3.3 Asafetyissueintheplant,whichifnotresolvedcouldseriouslyinjureDZpersonnel,

whichhasbeenbroughttotheattentionofthestationbutnotresolved.2.3.4 Seriouspersonnelmedicalissuesthatoccuronsite

2.4 Whencommunicatingsafetyissuesthefollowinginformationshouldbesupplied.

2.4.1 Currentstatusoftheemployeeandanyassistanceneeded2.4.2 Descriptionoftheevent2.4.3 WastheemployeeusingallrequiredPPEandfollowingsitesafetyprocedures2.4.4 Actionsthathavebeentakenatthesitetopreventadditionalinjuriesorresolvethe

2.5 TheSiteManagershouldcommunicatetotheRegionalManagersandtheSeniorDirectorofRadiologicalServices,within12hours,ofanyRPperformanceissuelistedbelow.

2.5.1 Failuretocompleteanadequateradiologicalsurveythatresultedinadoseordoseratealarms.

2.5.2 Failuretocompleteanadequateradiologicalsurveysothattheareawasnotproperlyposted,andcontrolledinaccordancewithstationprocedures.

2.5.3 Failuretocontrolcontaminationatthesourcecausingadditionalplantequipmentorpersonneltobecomecontaminated.

2.5.4 FailuretoprovidejobcoverageinaccordancewiththeRWPrequirements,orstationprocedures,thatresultedorcouldhaveresultedinadoseordoseratealarmorsignificantspreadofcontamination.

2.5.5 FailuretomaintaincontrolofaccesstoaHRA\LHRAincludingthecontrolofkeys2.5.6 Failuretoobtainairsamplesasrequiredbyproceduresthatresultedininternal

contaminationevents2.5.7 Failuretoproperlysurveymaterialforfreereleasethatresultedinradioactivematerial

beingreleasedoutsidetheRCA.2.5.8 Failuretoproperlybriefpersonnelonradiologicalconditionsthatresultedindoseor

doseratealarms2.5.9 FailuretoimplementALARAcontrolsthatresultedinadditionalpersonnelexposure.2.5.10 AnyperformanceissuethatcouldresultinaNRCviolation.

2.6 WhencommunicatingperformanceissuesthefollowinginformationshouldbeprovidedbytheSiteManager.UseAttachment1,PerformanceInvestigationGuide,tocollectinformationanddetermineimmediateactions.

2.6.1 Descriptionofeventincludingwhatspecificperformancewasnotcompleted(e.g.failedtosurveyroompriortoallowingaccessresultinginEADdoseratealarm)

2.6.2 Currentstatusoftheworkareaandtheimmediateactionstakentoclosetheperformancegap.

2.6.3 CriticalinformationfromAttachment1,PerformanceInvestigationGuide.2.6.4 Observationsoftheconditionsatthejobsitewhichcouldhaveimpactedthe

performance.

2.7 PerformanceImprovementPlansshouldbeimplementedwheninvestigationsintoevents,orobservedperformance,indicatethatthetechnicianisnotperformingworkthatmeetstherequiredstandardandfailingtocorrecttheperformancecouldresultinsignificanteventssuchasunpostedradiologicalhazards.

2.8 PerformanceImprovementPlansshouldbedocumentedonAttachment2,TechnicianDevelopmentPlan,Section4.AllimprovementplansmustbeapprovedbyRegionalManagersandSeniorDirectorofRadiologicalServices.

2.9 Basedontheconsequences(orpotentialconsequences)ofaneventitmaybeappropriateforthequalificationstoperformworkindependentlyinthatareaberemoveduntiltheperformanceissueshasbeencorrectedandvalidatedthroughobservationsthatitmeetstherequiredstandard.

2.10 ThedecisiontoremovequalificationsbasedonperformanceissueswillbemadebytheSeniorDirectorofRadiologicalServices.ThequalificationsthatareremovedwillbedocumentedonAttachment2,TechnicianDevelopmentPlan,Section4andremovedinPADS.

3.0 PerformanceImprovementProgram

3.1 InitialTrainingProgram3.1.1 RadiationProtectionTechnicianswillbetrainedandqualifiedbasedontherequirementsinNISP-

RP-12,TrainingandQualificationofSupplementalRadiationProtectionPersonnel.3.1.2 NISP-RP-12,TrainingandQualificationofSupplementalRadiationProtectionPersonnel,defines

therequirementsforaqualifiedJRRadiationProtectionTechnician.Thoserequirementsincludethefollowing;a. CompletionofFundamentalsforJRTaskQualificationb. CompletionofJRRPTaskRP2.01-RP2.10

3.1.3 WhileitispreferablethataJRcompleteall10JRTaskduringinitialtrainingthismaynotalwaysbepracticable.AtaminimumtheindividualshouldcompleteJRTaskRP2.01-RP2.04andRP2.06-RP2.07.Thisprovidestheindividualtheskills,andknowledgetosupportthesitesinawiderangeofjobtasks.TheremainderoftheJRqualificationswillbecompletedduringannualRPtraining.

3.1.4 Initialtrainingprovidestheknowledgetocorrectlyperformthetask.EachSTEincludesbothaknowledgetestandaperformanceevaluation.However,experienceisacriticalelementindevelopmentofhighlyskilledsupplementalpersonnel.Attachment3,JRRadiationProtectionExperienceGuidewillbeusedtoensurethetechniciangetsexperienceoverabroadcrosssectionoftheradiationprotectionfield.

3.1.5 EachTechnicianwillreceiveanAttachment3,JRRadiationProtectionTechnicianExperienceGuide,whichwillprovidetheoversightforensuringthetechniciangetsawiderangeofexperience.

3.1.6 TheSiteManagerwillreviewAttachment3foreachindividualassignedatthestationandworkwiththeSiteManagementtoensurethetechnicianreceivesasbroadexperienceaspossible.

3.1.7 CompletionofAttachment3,JRRadiationProtectionTechnicianExperienceGuideistheresponsibilityoftheindividualwithsupportfromtheSiteManager.

3.2 TrainingtoImprovePerformance3.2.1 Trainingisusedtoimproveandmaintainperformancebyestablishingthestandardsforthe

performanceofworkandvalidatingthosestandardsthroughtestingorpracticalexercises.3.2.2 Duetothehazardsassociatedwithnuclearpower,individualsarerequiredtoperformatahigh

levelofperformancetoensureregulationsandstandardsareproperlyimplemented.Doingsoensurestheworkersandthepublicareprotectedfromthehazardsofradiationexposure.

3.2.3 Performanceisevaluatedduringtrainingthroughtestingorobservationsofskillsduringpracticalexercises.

3.2.4 Itisessentialthatsomeperformancestandardsareimplementedcorrectlyeachtimetheyareperformedduetothecriticalnatureoftheactivity.Eachtrainingclass,wheretheyareobservedortested,criticalperformancestepswillbedefined.Someofthetypicalcriticalperformancestepsare:a. Postinghighradiationareawhenidentifiedb. Stoppingworkbasedonstopworkcriteriac. Performinganadequatesurveytodefinealloftheradiologicalhazards

3.2.5 Gapsinperformancewillbecorrectedduringtrainingthroughcoaching,re-training,and/orre-testing.

3.2.6 Gapsinperformanceofcriticalsteps,whichcouldhaveresultedinasignificantissuehaditoccurredatthestation,willbedocumentedonAttachment2,TechnicianDevelopmentPlan,SectionOne,bythetrainer.

3.2.7 Attachment2,TechnicianDevelopmentPlan,SectionOne,willdescribecriticalperformancegapsandtheadditionalactionsthataretotakenbytheSiteManagertoensuretheperformanceisatacceptablelevelwhileworkingatthestation.

3.2.8 Criticalstepperformancegaps,identifiedduringtraining,willbeobservedtobeperformedcorrectlybytheSiteManagerorStationSupervision.

3.2.9 PriortothestartoftheoutagetheSiteManagerwillreviewtheapplicabledatafromAttachment2,TechnicianDevelopmentPlan,SectionOne,withthestationRPM.Itiscriticalthestationbeawareoftheperformancegapsandtheplantoimproveperformance.Thestationshouldbeencouragedtoprovideoversightofindividualsandsupporttheobservationprogram.Observationsoftrainingperformancegapsshouldbecompletedassoonaspracticable.

3.3 PerformanceAssessment3.3.1 ThegoalofPerformanceAssessmentistohelptheemployeesmaintainperformanceatthe

highestlevelpossiblebyprovidingpositivefeedbackforworkthatmeetsthestandardsforperformanceandcoachingforitemswhichneedimprovement.

3.3.2 DuringoutageworkperiodstheSiteManagerwillcompleteanevaluationoftheemployee’sperformanceanddocumenttheevaluationbycompletingAttachment2,TechnicianDevelopmentPlan,SectionThree.

3.3.3 ItistheSiteManager’sresponsibilitytocompleteanassessmentofeveryDZRPTechnicianperformanceandprovidethatfeedbacktothetechnicians.

3.3.4 ItistheSiteManager’sresponsibilitytofrequentlyprovidefeedbacktothestationsRPMsonperformanceissuesidentifiedduringfieldobservationsandthecorrectiveactionsbeingtakentoimproveperformance.Asaminimum,theSiteManagershoulddiscussperformancewiththeRPMweekly.

3.3.5 ThePerformanceAssessmentwillincludefieldobservationsandreviewofdocumentationcompletedbytheemployee.

3.3.6 ObservationsmadebyStationpersonnelshouldbeincludedintheassessmentoftheindividual’sperformance.FeedbackfromStationpersonnelmustbedocumentedonAttachment2,TechnicianDevelopmentPlan,andSectionThree.

3.3.7 AttheendofeachoutageperiodtheSiteManagerwillreviewAttachment2,TechnicianDevelopmentPlan,SectionThree,witheachemployee.Theemployeewillacknowledgethefeedbackonperformancebysigningthedocument.

3.4 ImprovePerformanceThroughRiskMitigation3.4.1 TheSiteManagercandirectlyimpactperformancebycorrectlyassessingtheriskofactivitiesand

takingactionstoensurethesuccessfullyoutcome.3.4.2 INPOusesthreeriskcategories;High,Medium,andLow,tohelpthemanagementteamfocuson

thoseworkactivitieswhichhavethehighestpotentialforunwantedconsequences.3.4.3 Highriskactivitiesarenormallythosethathavethepotentialforcreatingasignificant

radiologicalissueiftheyarenotexecutedcorrectlysuchasdiving,radiography,powerentriesorremovalofvesselinternals.

3.4.4 ThesiteworkpackageandALARAplanningprocessnormallywillassesstheriskforeachworkactivity.TheSiteManagershouldunderstandhowthestationidentifiesriskontheworkschedule.ThisgivestheSiteManagertheopportunitytohelppreparetheDZRPtechnicianstobesuccessfulinthefield.

3.4.5 TherearemanyactionstheSiteManagercantaketoensuresuccessfulcompletionofhighriskworkhowever,thefollowingshouldalwaysbeconsidereda. Doesthetechnicianunderstandthecurrentandexpectedradiologicalconditions?b. Doesthetechnicianunderstandhowtomonitorconditionsinthefieldincludingradiation,

contaminationandairborneactivity?c. Doesthetechnicianunderstandthestopworkcriteriaforthetask?d. Doesthetechnicianhavetheproperequipmentformonitoringconditionsinthefield?e. Hasthetechnicianreadandunderstoodthecontrollingdocumentsfortheworksuchasthe

RWP,ALARAplanorprocedure?3.4.6 Insomesituationsthemostrisksignificantactivitymaynotbeassociatedwithhighdoserates.

Forexamplealargeamountofequipmentbeingsurveyedforfreereleasemayhaveahigherrisktothestation.TheSiteManagershouldassessriskinabroadviewandconsideralltheactivitiesbeingperformedbyDZemployees.

3.4.7 TheSiteManagershouldavoidtheassumptionthatnorisksignificantactivityistakingplace.Therearealwaysactivitieswhichhavepotentialtocauseadverseimpactstothesiteifnotexecutedcorrectly.

ThefollowingactionsshouldbetakenbytheSiteManagerwheninvestigatingaperformanceissue.Theobjectiveoftheinvestigationistodeterminewhytheperformanceissueoccurredandwhatactionsshouldbetakentopreventadditionalerrors.Theinvestigationshouldbecompletedasquicklyaspossiblebutmustbethorough.DuringtheinvestigationtheSiteManagermustconsiderifthisisasingleeventorcouldothersimilarerrorshaveoccurredandbeenunnoticeduntilnow.Ifthepotentialexistforothererrorstohaveoccurredadditionalinvestigationactionsshouldbetaken.Thefollowingquestionscanbecompletedinanyorderasappropriateconsideringtheevent.TheyarenotallinclusivebutshouldhelptheSiteManagercollectenoughinformationtocometoaconclusiononcauseoftheeventandthenecessarycorrectiveactions.Obtainconditionreportandreviewinformation,ifapplicable.DuringtheinvestigationtheSiteManagermustdetermineiftheindividualknewthestandardtowhichhewastoperformthetask.Iftheindividualdidnotunderstandthestandardforperformancethenthecauseoftheeventmaybetheresultofalackoftrainingoreffectivecommunication.Someeventsmaybecausedbytheindividualmakingapoordecisionwhileperformingwork.Decisionmakingispartofperformance.WillfulactionsbyanemployeethatviolatescompanyorsiteproceduresasdeterminedbyDZmanagementwillbehandledinaccordancewithDZcompanypolicies.AttheendoftheinvestigationtheSiteManagerisrequiredtostatethecauseeventandthecorrectiveactionsnecessarytoimproveperformance.TheperformanceplanmayberelatedtotheindividualorcouldinvolvetheallDZemployeesatthesite.

1. Describetheeventandtheconsequences,orpotentialconsequences,whichhaveoccurred.Includethelocation,timeandindividualsinvolved.

2. Validatetheconditionhasbeencorrectedinthefieldandiscurrentlyincompliancewithstationprocedures.

3. Asktheindividual(s)toprovideawrittenstatementdescribingtheevents.4. Iftheeventwasadoseordoseratealarmaskthesitetoprovideyouthehistogramfromthe

dosimeter.Thehistogramwillprovidecriticalinformationthatcanhelpdeterminetheactionsthatwerebeingtakeninthefield.

5. Validatethekeyelementsfromtheindividual’sstatement,ifpossible.ForexampleweretheyontherightRWPnumberorhavethecorrectEADsetpointsfortheworkactivity
