Reactions 1454 - 1 Jun 2013 Quetiapine Aseptic gingivitis: case report A 34-year-old woman with moderate mental retardation developed aseptic gingivitis while receiving quetiapine. The woman was started on retarded preparation quetiapine 400mg per day day [route and duration of therapy before reaction onset not stated] to treat the psychomotor symptoms of agitation and paroxysmal arousals. These symptoms were stable for about 1 year, when her legal guardian reported increased agitation and restlessness. Her quetiapine was slowly increased up to 800mg per day within 2 weeks. After about 1 week on the increased dose, she developed painful gingivitis that discontinuously affected the gingiva of the mandible and maxilla. The woman was treated with hygiene measures, comprising antimicrobial mouth rinses and thorough dental care after food intake. The gingivitis remained unchanged, and her quetiapine was reduced back down to 400mg per day. The gingival inflammation resolved within 1 week. Increased psychomotor agitation was treated with lorazepam. The woman’s quetiapine was again increased up to 800mg per day, and the gingivitis recurred. Her quetiapine dosage was reduced to and maintained at 400mg per day, with resolution of the gingivitis and no relapse observed within 6 months. Author comment: "Application of the Naranjo adverse drug reaction probability scale to this constellation reveals a score of at least 5, indicating a probable causation of the observed gingivitis by [quetiapine]".
