Quetiapine

Aseptic gingivitis: case reportA 34-year-old woman with moderate mental retardation

developed aseptic gingivitis while receiving quetiapine.The woman was started on retarded preparation quetiapine

400mg per day day [route and duration of therapy beforereaction onset not stated] to treat the psychomotor symptomsof agitation and paroxysmal arousals. These symptoms werestable for about 1 year, when her legal guardian reportedincreased agitation and restlessness. Her quetiapine wasslowly increased up to 800mg per day within 2 weeks. Afterabout 1 week on the increased dose, she developed painfulgingivitis that discontinuously affected the gingiva of themandible and maxilla.

The woman was treated with hygiene measures, comprisingantimicrobial mouth rinses and thorough dental care after foodintake. The gingivitis remained unchanged, and her quetiapinewas reduced back down to 400mg per day. The gingivalinflammation resolved within 1 week. Increased psychomotoragitation was treated with lorazepam.

The woman’s quetiapine was again increased up to 800mgper day, and the gingivitis recurred. Her quetiapine dosagewas reduced to and maintained at 400mg per day, withresolution of the gingivitis and no relapse observed within6 months.
