The quarterly inspection was performed on September 16, 2014. A copy of the Inspection and Routine Maintenance Report is attached. All maintenance needs identified during the inspection will be addressed as soon as possible. Routine maintenance activities for this quarter included cap mowing/trimming events, as well as periodic inspections of site fencing and monitoring wells. No non-routine maintenance activities were performed during this quarter, however a Completion Report is included for the ditch and road erosion repairs completed at the end of June. A copy of the Non- Routine Maintenance Report is also attached. The 2014 fourth quarter inspection at Neal's Landfill is scheduled for the second week of December. Re: Quarterly Inspection and Maintenance Report - Third Quarter 2014 Neal's Landfill, Bloomington, Monroe County, Indiana

This letter report summarizes the quarterly inspection and maintenance activities completed at the Neal's Landfill site during the third quarter of 2014. This report has been prepared in accordance with procedures and reporting requirements outlined in the March 2001 RCRA Cap Inspection and Maintenance Plan (CIMP) prepared by CBS Corporation.

The quarterly inspection was performed on September 16, 2014. A copy of the Inspection and Routine Maintenance Report is attached. All maintenance needs identified during the inspection will be addressed as soon as possible.

Routine maintenance activities for this quarter included cap mowing/trimming events, as well as periodic inspections of site fencing and monitoring wells.

No non-routine maintenance activities were performed during this quarter, however a Completion Report is included for the ditch and road erosion repairs completed at the end of June. A copy of the Non­Routine Maintenance Report is also attached.

The 2014 fourth quarter inspection at Neal's Landfill is scheduled for the second week of December.

Regarding: Completion Report for Ditch and Road Erosion Repairs Neal's Landfill Site, Bloomington, IN

Dottie:

The following is a summary of work completed at Neal's Landfill between 6/16 and 6/23 to repair erosion issues identified during the QI Cap Inspection. The work was completed by Alliance Services (Keith Mitchell) based on scope identified at a site meeting held earlier in June. The scope included landfill ditch erosion repairs on the north central down-chute and road erosion repairs on the east side near the top of the landfill.

Landfill Ditch Erosion Repairs

The following photographs show the top portion of the north central ditch down-chute prior to the erosion repairs. Rip rap had settled substantially at the edges of the ditch, due to washout of clay caused by failure of the geotextile fabric.

This issue was first observed in early 2013 and was only impacting the bottom portion of the down-chute at that time. Repairs were completed in June of 2013 to address this issue and the repairs were documented in the 2013 Q2 Cap Inspection report. The top portion of the down-chute was observed to be eroding in a similar fashion during the 2014 Ql Cap Inspection.

Repair work completed at the top portion of the down-chute included the following tasks:

• Removal of existing rip-rap and geotextile fabric; • Regrading of eroded clay areas; • Placement and compaction of new clay as needed; • Installation of new geotextile fabric; • Placement of new rip-rap in ditch; and • Reseeding-of any disturbed grass areas.

The following pictures document various stages of the work:

The repairs were extended down to the portion of the ditch that was repaired in 2013, so the entire down-chute has now been addressed. The south side down-chute will eventually require similar repairs and we will continue to inspect it periodically for signs of the same issue.

Road Erosion Repairs

The east access road has historically developed erosion ruts each spring during heavy rains. These ruts are generally repaired each year through re-grading and placement of additional gravel. The erosion ruts were worse this year due to a few very heavy storms that occurred over the winter and spring.

Repair work completed on the east access road included the following tasks:

• Removal of existing gravel and re-grading in eroded areas. • Placement and compaction of #2 stone and #53 gravel in eroded areas. • Placement of erosion fabric and rip-rap along sides of road for better drainage.

The completed repairs should minimize recurrence of east access road erosion issues.
