SAFETY FIRST AND FOREMOST
WHAT CAN GO WRONG?! – Everything !!
Trucks hit trucks
Trucks verses cars . . . truck wins!
Tip-overs
Compactors/dozers verses trucks
Most dangerous job . . . on the ground truck
spotters
![Page 5: Presented by: Christopher Campman](https://reader031.vdocuments.site/reader031/viewer/2022012920/61c7915c5bcbb2399e1671b7/html5/thumbnails/5.jpg)
ANIMALS
Encounter with equipment
Equipment wins!
![Page 6: Presented by: Christopher Campman](https://reader031.vdocuments.site/reader031/viewer/2022012920/61c7915c5bcbb2399e1671b7/html5/thumbnails/6.jpg)
SAFETY FIRST AND FOREMOST
Incidents
Dump trailer tip over - trailer fell on pickup truck
and hit the driver unloading the pickup – driver lost
both legs
Roll-off container fell off rails and hit front loader
driver as he dumped his load – driver never fully
recovered – permanent disability
![Page 7: Presented by: Christopher Campman](https://reader031.vdocuments.site/reader031/viewer/2022012920/61c7915c5bcbb2399e1671b7/html5/thumbnails/7.jpg)
SAFETY FIRST AND FOREMOST
Roll off stability is not a sure thing
![Page 8: Presented by: Christopher Campman](https://reader031.vdocuments.site/reader031/viewer/2022012920/61c7915c5bcbb2399e1671b7/html5/thumbnails/8.jpg)
SAFETY FIRST AND FOREMOST
Articulated dump that is now “fully” articulated
![Page 9: Presented by: Christopher Campman](https://reader031.vdocuments.site/reader031/viewer/2022012920/61c7915c5bcbb2399e1671b7/html5/thumbnails/9.jpg)
Not paying attention can lead to other
issues . . . like stability
![Page 10: Presented by: Christopher Campman](https://reader031.vdocuments.site/reader031/viewer/2022012920/61c7915c5bcbb2399e1671b7/html5/thumbnails/10.jpg)
Tailgate issue . . . Now how to “safely”
empty the truck?
![Page 11: Presented by: Christopher Campman](https://reader031.vdocuments.site/reader031/viewer/2022012920/61c7915c5bcbb2399e1671b7/html5/thumbnails/11.jpg)
1.Roll-off handle has broken off
2.Chain pin still secures
the door, but
corrosion has
removed some
metal
3.Severe corrosion
of reinforcing
supports
4.Conclusion >>
why is this roll-off
still in service?
![Page 12: Presented by: Christopher Campman](https://reader031.vdocuments.site/reader031/viewer/2022012920/61c7915c5bcbb2399e1671b7/html5/thumbnails/12.jpg)
This is the roll-off handle that broke off
This object missed the driver, although he is a
little bit shaken . . . NOT! How about a LOT
shaken!
![Page 13: Presented by: Christopher Campman](https://reader031.vdocuments.site/reader031/viewer/2022012920/61c7915c5bcbb2399e1671b7/html5/thumbnails/13.jpg)
This is view that the driver had of
the roll-off handle.
![Page 14: Presented by: Christopher Campman](https://reader031.vdocuments.site/reader031/viewer/2022012920/61c7915c5bcbb2399e1671b7/html5/thumbnails/14.jpg)
Packer box door
latch breaks at the
working face
What is the distance
between vehicles
at the working face
for good safety?
![Page 15: Presented by: Christopher Campman](https://reader031.vdocuments.site/reader031/viewer/2022012920/61c7915c5bcbb2399e1671b7/html5/thumbnails/15.jpg)
The distance between vehicles at the
working face is not being complied with.
![Page 16: Presented by: Christopher Campman](https://reader031.vdocuments.site/reader031/viewer/2022012920/61c7915c5bcbb2399e1671b7/html5/thumbnails/16.jpg)
Fortunately, this
driver did not
compound the
problem . . . and
he can get out on
the other side of
the truck
