FUNDRAISER WILL RUN FROM OCTOBER 6TH - NOVEMBER 5THCALENDARS ARE AVAILABLE FOR THE FOLLOWING DONATIONS. THIS IS A PRE-PAY SALE. CALENDARS WILL BE AVAILABLE FOR PICK UP AND DELIVERY MID NOVEMBER, JUST IN TIME FOR THE HOLIDAYS!

Family, Friends, and Team FUNDRAISING!

-Raise money in honor or memory of a precious child or other family member who cares about this cause and is passionate about supporting NICU, preemie, and angel families.

-Help to raise awareness about prematurity, babies born with serious health concerns, the NICU, and the need for family support before and after this journey!

-Start a team on behalf of your company or even divisions/departments within your company or organization

-Get your family and friends involved with helping to raise awareness about the NICU journey & preemies

-Submit your team name, picture of a your preemie at birth with family and friends, and other. Each team can submit up to 5 pictures for their personal fundraising page! Submit a short story (up to 200 words) about your preemie/NICU baby/or angel and why you are forming a fundraising team. Set a team goal and we will create your page and add a working thermometer that can be updated with each additional calendar donation someone makes on your personal team/family page! If you don’t want a web page then just use the order form on page 4 to help you keep track of your family/friend donations. Each team will be set up exactly like our main “Preemie Prints” page so visit www.preemieprints.org and click on the calendar to view.

-The family/team who raises the most money will win a $75 gift card. Winner will be announced AT THE OFFICIAL “CALENDAR PICK UP EVENT” ONCE THE CALENDARS ARRIVE AFTER THE FUNDRAISER IS COMPLETE. You do not need to be present to be awarded the prize. Notifications will be made via facebook and the website and prize mailed out. A form from each team/family needs to be submitted along with checks/money orders/or notification of submitted cc transactions by NOVEMBER 5TH, 2011. Forms must be returned for processing, regardless of if you decide to set up a personal webpage or not. Each team/familly should submit a form. Forms can me mailed or scanned/emailed.

Ordering & Donation Information:1. Through pay pal on your personal/TEAM page

2. Through pay pal on our main fundraising page (this is where you would order just 1 or a couple-if you are not forming a team)

3. Collect cash from family and friends using the order form on the next page and then send a MONEY ORDER for the total made out to Preemie Prints for the total amount of your calendar donations ~OR~ collect cash from family and friends using the order form on the next page and make a (1) time donation ON THE WEBSITE with a credit card ~OR~ send your family and friends to your team page or the main fundraising page to make a credit card donation. Individual check donations are excepted as well. Make sure your team name is written on the check.
