Hospital Policy

1. Admission Policy

Present the Physicians Order Sheet to the admission

center of the hospital

Call our hotline to reserve a room and choose the Room

Rate you prefer.

Guardian or significant others should sign the Admission

and Discharge Record (ADR).

Pay the required deposit before transferring patient to

the ward.

Transport of patient to the Nursing Station or Ward.

2. Delivery Room Policy

Assess the patient’s preferred language for education and

care.

Assess patient’s/family’s ability to understand

information given,

need for hospitalizations and plan of care. (i.e.,

language

barriers, knowledge/sensory deficits and physical

limitations.

Patient will be placed on the external fetal monitor

(EFM) at the

time of admission. Refer to Unit Practice Manual for

application of

EFM and protocol for fetal monitoring.

An initial maternal-fetal assessment will be performed on

each

patient at the time of admission. Assessments include

maternal BP,

pain and fetal heart rate and uterine activity. Using the

L&D/OB-GYN database, conduct a patient interview, history

taking and

assessment. Examine the fetal monitor tracing for

assessment of

fetal and uterine activity.

Begin the appropriate standard nursing care plan or

carepath.

Establish IV access upon admission unless otherwise

ordered by the

physician.

Obtain necessary specimens as ordered by the physician

for admission

lab work.

Notify the physician of patient signs/symptoms that

require

immediate attention.

3. Discharge Policy

Decision made and documented in medical notes to confirm

that the patient is medically fit and risks identified

and appropriate risk assessment and plans in place.

Ensure a Continuing Care Check List has been completed

and evidence documented.

Complete Discharge Check List – give copy to patient.

Discuss prescribed medication, assess for patients to

self-medicate who will be responsible for

managing own medicines on discharge.

4. Imaging Policy

Present physicians order to the Radiology Department.

Ask patient if has allergies in any foods, medication or

contrast media that will be used in the procedure.

Ask if there are any metal implants in the patients body.

Patient should know what should be expected when they are

inside the equipment(MRI,CT-SCAN)

Call the assisting nurse to transfer the patient to their

room.

5. Laboratory Policy

Label the specimen and handle it with care before

transferring it to laboratory room.

Follow the physicians order when to deliver the specimen.

Maintain sterility when obtaining the specimen sample.

Send to the lab the specimen obtained and inform the

personnel that will handle it.

6. Nursery Room Policy

Parents must show proper wristband identification or ID

card prior to visiting their infant:baby in an

Incubator

Only two visitors per infant are allowed at any time

Siblings, 12 years or older, may visit if they

accompanied by a parent

No siblings are permitted to visit if he/she has an upper

respiratory infection, temperature, has been exposed

to a communicable disease, or has not had chicken pox.

Grandparents may visit, at anytime, if accompanied by one

of the parents

The mother may designate one "significant other" if the

father is unavailable or not involved.

Parents are responsible for making sure that all visitors

to the Nursery wash their hands before going to the

baby’s bedside.

Parents must make sure that all visitors coming into the

Nursery are healthy.

7. Operating Room Policy

It is the policy of Illustrados General Hospital that the

preoperative procedure for confirmation of correct patient

identification, operative procedure, operative site and the

informed consent process has been satisfactorily completed is

a collaborative effort on the part of nursing, anesthesiology

and surgical staff members. The roles and responsibilities of

each staff member are outlined in this policy.

The attending surgeon and attending anesthesiologist must

review and sign the patient’s consent forms for

surgery and anesthesia respectively before the patient

enters the operating room.

The surgeon must mark the surgical site with the patient

prior to the patient’s entry into the operating room.

In the case of the incompetent adult, and/or patient of

minor age, the verification of identification and

confirmation that the informed consent process has been

satisfactorily completed and consents signed is

conducted with the patient’s parent, legal

guardian/authorized representative, or health care proxy

Immediately prior to the induction of anesthesia, a final

official time out is conducted by the circulating RN,

anesthesia care provider and the attending surgeon.

8. PACU Policy

All nursing staff in the Post Anesthetic Care Unit (PACU)

will adhere to PACU Standards of

Care and will have received specific orientation and

training in the care of Post Anesthetic

patients.

All patients admitted to PACU will be extubated. All

patients are to be extubated and stabilized in the

Operating room prior to being transferred to PACU.

The PACU scoring system will be applied.

Closed monitoring of patients will be apply before

returning to the designated room.

Every patient admitted to our hospital, or the patient's

health care decision maker acting on his/her behalf, is

responsible for the following:

To provide, to the best of his/her knowledge, accurate

and complete information about present complaints, past

illnesses, hospitalizations, medications, and other

matters relating to his/her health.

To report unexpected changes in his/her condition to the

responsible practitioner.

To report whether he/she clearly comprehends the

contemplated course of treatment and what is expected of

him/her.

To report dissatisfaction with the quality of care or

service to the nurse manager, physician, or patient

representative.

To be accountable for his/her actions if he/she refuses

treatment or does not follow the practitioner's

instructions.

To assure that financial obligations of his/her health

care are fulfilled as promptly as possible.

To follow hospital rules and regulations affecting

patient care and conduct.

To be considerate of the rights of other patients and

hospital personnel by assisting in the control of noise

and the number of visitors.

To be respectful of the property of other persons and the

property of the hospital.

10. Referral Policy

Assess the patient and stabilizeas resources and skills

are available

Identify the facility where the patient needs to be

referred

Inform the patient/ next to kin regarding referral to

another facility

Contact the referring facility: Notify the doctor in

the Emergency Department regarding the patient’s

condition and get an approval to transfer

For in-patient transfer / referral requires the accepting

hospital to have an available bed, and acceptance by a

physician who has admitting privileges.

A designated hospital hotline shall be made accessible

for the purpose of patient referral

All inter-hospital transfers must be coordinated directly

between the physicians in the referring facility and the

accepting facility

Qualified medical/paramedical personnel must be present

in the ambulance when transferring the patient

It is the responsibility of the referring physician to

ensure continuity of care and patient safety during the

transfer process

Complete referral forms, legibly and accurately, include

attach any investigations and reports and send with the

patient

Documentation of the medical information must be

available in the facility ( Medical records)

11. Transfer Policy

The patients and their relatives/carers must be informed

by the nurse accountable for the patient’s care that they

are leaving and advised of where they are going.

The receiving ward must be given a verbal handover and be

told whether the patient’s relatives/carers have been

informed or not.

All of the patient’s property and valuables must be

transferred with the patient.

All of the patient’s health records must be transferred

where available. This will include medical records,

assessment documentation and therapist notes.

The transferring ward must ensure that all relevant

medications accompany the patient to the receiving ward

along with medication and associated equipment brought

into hospital.

The transferring ward must ensure patient transfer

details are entered onto the patient administration

system and ensure the PAS information transfer takes

place in line with ‘real time’ bed management system

Where a clinical need is identified observation and vital

signs should continue during the transfer process.

Only appropriately skilled personnel trained in manual

handling should transfer patients between wards.
