East Sutherland chosen as 1 of 5 Scottish Government test sites for the 2nd Dementia Strategy around the 8 pillar model of care

2 year study / £75,000 funding per test site

What was tested?

How the 8 pillar model of dementia care

can support potential ‘solutions’

in a remote, rural setting

by encouraging early diagnosis

to extend the self-management pathway

for as long as possible

thus delaying crisis

and care home admissions

while keeping people at home for longer

Community initiatives: Housing

The issues:

Rural housing different to urban housing

Rural houses difficult to adapt

Isolation, extreme conditions and lack of utilities

Lack of adequate housing = people with a care need forced to move away from community

Community initiatives: Housing

The solutions:

Make homes dementia friendly by making them people friendly

Cross-sectoral collaboration with local authority, NHS, social housing providers and recipients

Working with Glasgow School of Art to design a blueprint for best practice

Investigating creative use of technology

Speaking to other sectors to find any common issues (e.g. access for prams and wheelchairs)

Hosting a rural housing conference and how communities can do it for themselves

Community SupportTransport

As a result of illness a regular member of the Hub unexpectedly found herself in hospital 25 miles from home

We sent the community bus to pick her up

And be part of the Xmas celebrations

What does the future look like?

Support/funding to encourage rural community

programmes to allow families to:

dip in/out of 8 pillar solutions

stay on self-management pathway for longer

Well-being hubs in every village to support the self-management pathway?

Community working in partnership with local hospitals/housing to enable people to come home as soon as possible?

In conclusion...

All our testing tells us

8 pillar solutions should sit directly above the self-management pathway

to allow people affected by dementia (patients and carers) to dip in and out as support is needed beforethe crisis hits

And it is the community that can support families in this preventative work

To stay at home for longer

To live a life of quality for as long as possible
