Dear Friends, Liberty is about relationships and it is with the help of many that we are able to Connect Communities and Transform Lives. The core elements of this work are the result of careful and strategic planning, gaining a deeper appreciation for the benefits of technology and enhancing our relationships with congregations and communities – our valuable partners in service.

The Liberty Lutheran 2014 Annual Report is an opportunity for you to hear about the many ways your generous support, the work of the Liberty Lutheran Board of Directors, a dedicated management team, and our compassionate staff are making a difference in the lives of those in need.

Liberty’s vision for the future includes a wide range of lifestyle choices for residents living in our four senior communities, offering a greater number of children and youth educational opportunities, and providing the most vulnerable people in Pennsylvania new community services through our ministry with congregations and other non-profit organizations.

When Karen toured The Hearth at Drexel with the Executive Director, Betty Bebian, she believed she found a perfect solution for the care and support that her parents needed. Karen says, “Although Mom and Dad had lived independently all their lives, their needs for care and daily support were increasing. We didn’t want a clinical environment with restrictions and call- bells, but a place they could call home.”

Opened in June, The Hearth represents a new approach to Assisted Living and features the shared warmth of households and the opportunity to enjoy their meals and conversation with neighbors around the dining room table. Betty Bebian says, “It’s unlike the typical personal care environment. Each of our four households are fully equipped and beautifully furnished to enhance the daily lives of our residents. Our goal is ‘resident – centered’ care –to nurture each person’s dignity, and encourage them to use their abilities and preferences when choosing their daily activities and meals.” Moving is overwhelming at any age- it’s a big adjustment. Karen recalls, “I wasn’t sure how a move like this would go for my dad – but it was really fine. In the warmer weather we sat on the porch – he loved that – and my mother has improved so much.”

Although Karen’s mother had experienced a series of falls before moving to The Hearth, she now enjoys walking through the halls and participating in some group activities.

Karen says, “I appreciate that The Hearth is affiliated with a faith-based organization – its core underpinning is reflected in the loving way the staff have been, from the housekeepers and maintenance, to the nurses and administration. Add to that, it’s beautiful here, and we believe our parents are in the best place possible at this stage of their lives.”

Liberty Lutheran has adopted a cutting- edge technology to reduce hospital admissions for short-term and rehabilitation patients and long-term residents. This technology, known as eSNF telemedicine, allows residents to remain in their “home.” If there is a change in the resident’s condition, the nurse and eSNF doctor are able to have a face-to-face video conversation about a presenting medical issue. This service provides immediate diagnostic services which are available after hours, weekends, and holiday - times when the attending physician may not be available to see the resident in person.

For the elderly, a trip to the hospital often means spending uncomfortable and intimidating hours in the emergency room. By implementing eSNF telemedicine, this year 92% of the residents who were “seen” by an eSNF physician were treated on-site at Artman and Paul’s Run Retirement Community.

The mobile telemedicine equipment includes a video camera, monitor and digital stethoscope. Both parties can speak to and see each other and the eSNF physician can examine and treat the patient.

This innovative technology demonstrates Liberty Lutheran’s commitment to delivering the highest quality of care to our residents and positively impacts our relationships with referring hospitals.

Artman, Paul’s Run Retirement Community, The Hearth at Drexel and corporate partner Sodexo enjoy working together to give residents the very best. Tom Rogers, District Manager for Sodexo Senior Living Division believes that “Creating quality of life in senior care means creating a home.”

Sodexo, the provider of dining services for Artman and Paul’s Run has demonstrated their commitment to residents as a sponsor of the Artman Golf Outing for the past 10 years. Tom says, “We’re very happy to support the Artman Benevolent Care Fund. By giving back to the communities we serve we’re fulfilling our mission.”

For more than three years, staff have been enhancing resident services The Liberty Way. Katrina Wise, Executive Director of Paul’s Run Retirement Community says, “We’ve made significant changes – changes that empower residents to make choices, something they have been doing all their lives. Resident – directed care is a real change for long term care, and Sodexo has learned along with us.”

“Life Happens at the Kitchen Table” is one of the ways the Sodexo and the Liberty team have worked together. Katrina continues, “Our residents have lived during a time that everybody cooked every day. The kitchen, no matter how big or small is where everyone gathers, and is an important feature as we continue our renovations.”

Signs of this new model of care are plentiful at Liberty. Residents at Paul’s Run are enjoying the aromas of fresh coffee brewing and goodies baking in the new kitchen area. At The Hearth, fireside chats are just one of the ways new relationships are formed, and after months of planning, Artman has launched extensive renovations that will enhance resident-centered care.

Brothers Joel and Narcisse Bakutibe, students at Norristown High School, are settling into a new school and a new life in Pennsylvania. With support from Lutheran Children and Family Service Refugee Resettlement staff and many volunteers from Trinity Lutheran Church, Fairview Village, these boys are enjoying the typical activities of ninth grade students. The brothers arrived in May from the Democratic Republic of the Congo (DRC). Volunteers, led by Yolanda Zimmerman and Joan Fichtner, began working together to prepare for their arrival in October 2013. Yolanda recalls, “We had ample time to prepare but there was much to do. We needed to locate a house, gather furniture and household items, investigate school registration, and medical care. Each person on the team took responsibility for one area and enlisted the help of others to provide meals, clothing and teaching the ways of our culture.”

Over the summer months the boys visited the Norristown High School where they met the soccer coach and made new friends. Seeing their potential the coach purchased the shoes, shin guards and uniforms for the boys. “Having new friends and a sport they both love was a great way to start the school year. Parents got to know them too and have been supportive to the whole family. It’s been great to see this,” Yolanda says. “This experience has been amazing. I’ve learned how much I take for granted and how important literacy is in our daily lives. By working together to support this family, our spiritual life has been enhanced and our relationships with each other have strengthened.”

Giving Kids a BO

OST for Life

Shayne Brock, an eighth grade student at Bryant Middle School in West Philadelphia, has his friends to thank for enrolling in our Best Out of School Time (BOOST) program.

“Instead of just running the streets and not being good kids, we just decided to join this and have fun,” Shayne says.

Quickly, the 14 year old boy realized the afterschool program is much more than a place to hang out with his friends. BOOST, he says, is inspiring him to set goals and carve out a path to achievement. Shayne is among more than 1,100 youth in elementary, middle and high school enrolled in BOOST’s before-and-after school programs across the city of Philadelphia. Administered by Liberty’s Lutheran Children and Family Service, BOOST incorporates academic enrichment, activities that promote social and emotional growth, as well as a wide array of recreational activities in an adult-supervised, safe environment. This includes homework assistance, a project-based learning curriculum, recreation, arts and crafts, computer lab, field trips and more.

This past year, through grant and community support, the BOOST program built upon its tools and curriculum by obtaining iPads at some sites that did not have computer labs, creating new and innovative educational projects, and adding additional staff members.

Shayne credits the extra guidance he receives from BOOST staff with making a difference in his life. “I would like to go to college. I want to get my education and at one point be a CEO. I’d like to have my own business, maybe my own clothing line,” he says.

ELCA Support extends Liberty’s Mission

With support from the Evangelical Lutheran Church in America (ELCA) over the past year, the Liberty Lutheran Family has extended its services to hungry children, individuals and families recovering from a disaster, and to older adults who are motivated to remain active.

In January 2014, Lutheran Disaster Response – Eastern PA (LDR-EPA) received a two year grant of $14,000 from the ELCA Disaster Response to pilot a Spiritual Support Partners for Disaster Recovery program. Chaplains, who are trained in disaster preparedness and recovery, will accompany pastors and other faith-based leaders into disaster affected areas and provide coaching and spiritual support. The Spiritual Support Partners project will teach pastors about the stages of disaster recovery, ways to best serve the community, and how to care for themselves while working in these challenging situations. As a recipient of a $3,500 ELCA Domestic Hunger Grant, Lutheran Congregational Services is working with Backpack Pals/Peanut Butter Project of Easton to ensure a greater number of children, who are participating in school lunch and breakfast programs, will have food for long weekends and holiday breaks from school.

Liberty Lutheran’s Wellness Program, “Liberty Lifestyles,” has been supported, in part, through a grant from the Lutheran Services for the Elderly Endowment at the ELCA Foundation. The $60,000 award will help to support wellness initiatives for more than 900 residents in Liberty’s four senior communities.

Village at Penn State resident, Percy Pierce, and many of his neighbors are thinking a lot about their future. Liberty Lutheran is committed to the needs of our residents and to responding to the changing trends in senior living. By listening to our customers and investigating trends, Liberty is transforming all of its senior communities to “households” – a new model of care. “The new Personal Care Household is important to us as we age. We will have the support we need - right here at the Village,” says Percy, who has called The Village “home” since 2004. Since 2012, Liberty Lutheran has been planning for the future of The Village at Penn State. As plans evolved, resident feedback helped revise and shape the plan. As a result, the residents identified a priority for personal care accommodations.

The construction of a new Personal Care Household will accommodate 12 to14 residents. This new approach to senior living is a transformational shift from the institutional medical model of care to one that gives residents greater autonomy and choice. Each household will feature a fully functional kitchen, living room with hearth area and dining room. Plans for The Village will mirror those put into place at newly-renovated Hearth at Drexel, Liberty’s Assisted Living community in Bala Cynwyd and extensive renovations at Artman and Paul’s Run that are currently underway.

The residents are eager for the new construction of the Personal Care Household to begin in the spring of 2015 with an anticipated completion in 10 to 12 months.
