The following was noted:
Preparation for final excavations ongoing in area of retaining wall and parking lot expansion in the front of the site.
Excavation and compaction for the eastern corner of the site was ongoing. The foundation base for the retaining wall in this
corner was also underway. Additional fill for the rear parking lot expansion was in place. Pipes and fittings for the front
detention area were onsite. Earth was being removed from the front area of the site and being placed in the eastern corner
of the site and then it was being compacted. It should be noted that in the eastern corner of the site where the bottom of the
retaining wall will be, there was standing water. The contractor said he found soft ground and over excavated to compact
backfill this area and water came into the excavation overnight.
Follow up activities required:
Additional fieldwork was requested as follows:
Layout the new parking islands.
Layout retaining wall bottom location and elevations where we can get to.
Check benchmark at rear of site on generator pad, appears to be a few feet off.

2009I was given a set of plans and comment letter from MassDEP for a groundwaterdischarge permit for a mobile home park. I was asked to review the plans,calculations and the comment letter. These are my notes from my review.

Theleachingareamoundgradingwasrevisedtoeliminatetheneedforanimperviousbarrier. Someofthegravitysewerlineswereshiftedtoprovide10separationfromexistingmobile
homesasrequiredbytheMassStatePlumbingCode.
Moredetailwasaddedtothesystemprofiletomakeitclearerforconstruction. Additionalmaterialremovalisproposed. TheAandBsoilhorizonsshouldberemovedto
provideadequatepercolationthroughthesubsoil.
Someadditionaldetailswereaddedforclarity. Including,butnotlimitedto:o Sewerconnectiono Valvevaulto Actuatorvalveso Combinedtrench
Variousdetailswererevisedtoclarifyconstruction. Notesanddetailsindicatingthattheinteriorbottomsofthepumpchambersshouldbesloped

On October 27, 2009, I visited the City of Fitchburg Conservation Commission (Michael OHara), theBoard of Health (Steve Curry), the Engineering Department (Gary Bevilacqua), the Water Department(Dave Berger), and the Assessors office. I also visited the project site. Below is a summary of theinformation that I obtained.
Conservation Commission
I obtained the Wetlands Protection Ordinance Rules and Regulations. There were no recent plans available for this site. There was a fuel leak Michael remembered and he produced a report by Golder Associates
Semi Annual Monitoring Forge Shop Release MassDEP RTN 2-13597 #155-404 for a Tier1C W023657.
Michael suggested I go to the Board of Health to see if there was a site assignment for thislocus. Meetings are last Tuesday of the month. Submissions are due 2 weeks prior, this includes the
Town department review. Michael thought that the 125 permanent easement on the Nashua River may be an Army
Corps of Engineers easement from when the Bemis Road bridge and dam was washed out.
Board of Health
There is a site assignment for the locus. In the file there is an Actiivity and Use Limitation filed November 2007. Recorded Book 7005
and Page 250 and Book 7005 and Page 257.
Engineering Department
Gary gave me several plans showing utilities around the site. Infiltration is very important to them for drainage prior to connecting to city drainage. Sewer fees are based on water consumption so if there are losses due to processes they may
want to install a meter instead. Gary is tracking down the information on the 30-foot wide easement over the sewer and drain. Unitel is the contact point for gas and electric utilities.
2009This project involved the site design and permitting for a
Bio Fuel facility in Fitchburg within an existingmanufacturing facility parking lot. This is a researchmemorandum after I visited various City departments.

Tom Szocik would have any plans for the access road. Tom was out sick Tuesday. Gary indicated there were no sewer or drain issues in the area. There is a CSO located near the proposed cul-de-sac, CSO 033. The structures to the north
of the property are not a CSO, just close manholes.
Water Department
Dave gave me several plans showing utilities around the site as well as on the site. Fire and domestic services must be separate. Dave gave me a water service fee schedule-
they are VERY HIGH and vary by line size, make sure the appropriate size is chosen! It is important to understand that the property owner will be billed, not a leasor. Dave mentioned a potential desire to loop the water from the proposed access road through
the site up to Intervale Road. He asked if we had any offsite improvement plans. I indicated we didnt then he mentioned a
desire to see a portion of the watermain near Intervale be replaced. Pressure in the area is 95 PSI, if we need to do a hydrant test it is $100, they operate the
hydrants, we perform the test.
Assessors Office
I picked up a request for information form for Abbuter Notification. We do the notifications City does the advertising, the fee is included in the overall fee.
After the City visits, I met with Ken Myer and Charles Reitz of Simonds International. I then did a sitewalk and took pictures. The following is a summary of what I learned.

We can talk with David Witman of Simonds about the Activity and Use Limitation as well as theutility and railroad easements. He can also assist us in the landfill cap area plans.
The landfill cap is a cover of debris from the factory, mostly metal objects. The BioMass plant will get water from one of the onsite wells, mostly likely the one in the
northeastern corner of the site. I took a picture of this well and we have it located on our plans. Discharge from the plant would go through the existing Simonds discharge to the Nashua
River, requiring a modification to their current permit. Simonds will be leasing to the BioMass plant for 20 years, they will buy their electricity from the
plant. Currently they are partners in the project but may not be eventually. There may be an issue with the airport. Gale Associates is working on a proposal to expand
the airport with a new runway that may interfere with constructing smoke stacks in our area.Dan Curley of the Economic Development Commission may be able to answer our questionsabout this impact, it sounds like the airport is being directed to modify their plans since it wouldalso affect the proposed lots off the access drive.
Ken indicated that the driveway would be gravel. Ken indicated that the portion of the site containing the landfill cap will be given to the City of
Fitchburg.
