History and Characteristics Provide a brief history of the institution, a description of its current mission, an indication of its geographic service area, and a description of the composition of the student population. 3. Off-Campus Instructional Locations and Branch Campuses (Please note: This section has been extensively revised.) 4. Distance and Correspondence Education Provide an initial date of approval for your institution to offer distance education. Provide a list of credit-bearing educational programs (degrees, certificates, and diplomas) where 50% or more of the credit hours are delivered through distance education modes. 1. History and Characteristics Florida Agricultural and Mechanical University (FAMU) was founded in 1887 as the State Normal College for Colored Students. In 1891, the College received $7,500 under the Second Morrill Act for agricultural and mechanical arts education and became Florida’s land grant institution for colored people. In 1953, the institution was elevated to university status, acquiring its present name. The University’s primary geographic service area is the State of Florida; 87% of the students are Florida residents. The University operates a main campus in Tallahassee, FL and a College of Law in Orlando. Additionally, the College of Pharmacy and Pharmaceutical Sciences offers the PharmD via distance education at an off-campus instructional site in Crestview, FL, and operates satellite offices to provide clinical experiences in Tampa, Jacksonville and Miami. FAMU is an 1890 land-grant institution dedicated to the advancement of knowledge, resolution of complex issues and the empowerment of citizens and communities. Expanding upon the University’s land-grant status, it will enhance the lives of constituents through innovative research, engaging cooperative extension, and public service. While the University continues its historic mission of educating African Americans, FAMU embraces persons of all races, ethnic origins and nationalities as life-long members of the university community. Admission to the University is on a selective basis. The University is part of the twelve member State University System of Florida. The University offers 53 bachelor’s degrees, 28 master’s degrees (three of which are also offered in the online format), three professional degrees, and 12 doctoral degrees. The University is composed of 14 colleges and schools: the College of Agriculture and Food Sciences; the College of Education; the College of Engineering (operated jointly with Florida State University); the College of Law; the College of Pharmacy and Pharmaceutical Sciences; the College of Science and Technology; the College of Social Sciences, Arts, and Humanities; the School of Allied Health Sciences; the School of Architecture and Engineering Technology; the School of Business and Industry; the School of the Environment; the School of Journalism and Graphic Communication; the School of Nursing; and the School of Graduate Studies and Research. In fall 2016, the University enrolled a total of 9,614 students in its undergraduate, graduate and professional programs with the following composition:

85% full-time, 15% part-time;

77% undergraduate, 19% graduate, and 4% unclassified;

84% Black, 8% White, 1% Asian, 3% Hispanic, and 4% other. The University considers as its peer institutions: Howard University, University of Memphis, University of Toledo, University of Arkansas at Little Rock, Cleveland State University and North Carolina A&T State University. 2. 5. Accreditation

(1) Table 5.1 below provides a listing of agencies that currently accredit the University and its programs, including the date of the last review by each agency.

(2) SACSCOC is the University’s primary accreditor for access to USDOE Title IV funding. (3) Since the University’s last reaffirmation in 2009, no USDOE-recognized accrediting

agency has terminated accreditation of any FAMU educational programs; the University has not voluntarily withdrawn from any USDOE-recognized agency.

(4) Within the last two years, the following sanctions or negative actions have been taken

by USDOE-recognized agencies against the University:

One of the University’s programs (occupational therapy) has received a sanction from a specialized accrediting agency within the past two years, as described below.

A) Accreditation Council for Occupational Therapy Education (ACOTE):

At its April 9-10, 2016 meeting, ACOTE reviewed the Report of On-Site Evaluation (ROSE) regarding the Occupational Therapy Master's Program at Florida Agricultural and Mechanical University, Tallahassee, Florida, and voted to Grant a Status of Accreditation for a period of 7 years (see Appendix A – ACOTE Letter April 9-10, 2016). The University was requested to develop a Plan of Correction addressing the areas of noncompliance cited below (see Appendix B – ACOTE Plan of Correction, April 2016). In December 2016, the University submitted a progress report on standard A.5.3 Program Evaluation (see Appendix C – December 2016 Progress Report).

In April 2017, the University was notified by ACOTE that the accreditation for the Master’s in Occupational Therapy program accreditation status was changed to probationary (program evaluation and certification exam pass rate) (see Attachment D – Florida A&M 4-2017 letter). On June 20, 2017, the University submitted a Plan of Correction to address the issues cited by ACOTE for standard A.5.6 and a Progress Report for standard A.5.3 (see Attachment E – ACOTE Narrative and Plan of Correction). Additionally, the University communicated with SACSCOC regarding the status of the program’s ACOTE accreditation on April 26, 2017 (Attachment F – Correspondence to SACSCOC, April 26, 2017) and June 19, 2017 (Attachment G – Correspondence to SACSCOC, June 19, 2017).

6. Relationship to the U.S. Department of Education The U.S. Department of Education (USDOE) has not imposed any limitations,

suspensions or terminations on the University in regards to student financial aid or other

financial aid programs during the previous three years. However, FAMU’s Office of

Financial Aid and Scholarships is authorized to participate in Federal Title IV programs

by the USDOE under the Program Participation Agreement (PPA) and Eligibility and

Certification Approval Report (ECAR) through September 30, 2019 with provisional

status (see Attachment H - FAMU USDOE PPA). FAMU was granted provisional status

due to the Federal Perkins Loan Default Rate condition. The Office of Financial Aid and

Scholarships has addressed the issue by offering debt counseling to Federal Perkins

Loan recipients and collaborating with the University’s Student Success Center and its

debt counselors to increase financial literacy and debt management awareness (see

Attachment I – Perkins Loan Cohort Default Rates). As a result, the University has

experienced a decline in the University’s Cohort Default Rate (CDR) for the Federal

Perkins Program and the Direct Loan Program.
