5 – The Request Process

1. A school or setting can make a request for Park Community Academy support by completing a referral form and emailing it to - [email protected] A copy of the referral form can be found on the BTSA website.

If the support is for an individual pupil, please also ensure that you have discussed the referral with parents/carers at this stage and have their agreement for it to occur.

2. On receipt of the referral you will be contacted by a member of Park Community Academy staff to confirm the appropriate support arrangements.

The school may then share this report with parents/carers if they feel it is appropriate to do so. If there is a request being made for statutory assessment at a later date the report should be included in the evidence.
