INTRODUCTION
Every Industry, particularly those handling hazardous items in bulk quantities like in petroleum Industry, should strive to establish the best systems and operating procedures for ensuring all the time safety. We in Oil India have over the years established systems and facilities in such a manner that all time safety is ensured to the maximum extent. This is being done with the concept that all industrial and work accidents are avoidable.
While we continue to take steps and measures as mentioned above, we cannot rule out the probability of occurrence of an incident having disastrous consequences to our operations. The Northeast being an earthquake prone area, the disaster potential of a major earthquake or unprecedented heavy floods thereby causing major disruption of operations and distress to large number of people in and around our operational areas are examples of such crisis situations we may be called upon to confront and handle effectively.
It may be mentioned that we have successfully managed to overcome crisis of type or other on different occasions in the past, which goes to say that our systems and facilities have the inherent strength to meet such crisis situations. However, proper documentation of the existing procedures would help to streamline our preparedness to face disasters and manage the crisis effectively.
The documented plan envisages the following: 1. Proper procedure for declaration on the crisis. 2. Identification of coordinators and their role. 3. Communication of the crisis down the line to concerned personnel. 4. Setting up of control rooms. 5. Identification of disaster potential sectors and key persons involved. 6. Action plan for rescue, relief, salvage, repair and restoration taking the
assistance from all the people including Mutual Aid from nearly industries.
While the key document may not be exhaustive, it is complete with the supporting documents already in practice with us. The key factors of the plan properly updated and with regular drills would definitely go a long way in combating the crisis.
The primary objective of the plan is to organise rescue, relief, fire fighting and medical relief drills operations on top priority. At the same time, efforts to normalize production operations progressively would also continue.

The plan would cover the following types and causes of emergencies.

This plan is made as model plan for our operations at Duliajan and surrounding
areas. The coordinators and key personnel mentioned in the above plan would
work out detailed manual to cover all individual operations. Similarly the respective
Mines Managers would work out similar plans for their areas of operation.
THIS ‘ONSITE’ EMERGENCY RESPONSE plan would serve as basic document
for DISTRICT LEVEL OFF SITE EMERGENCY RESPONSE AND MUTUAL AID

EMERGENCY CONTROL ROOMS
1. In case of any fire the Central Fire Station would serve as the ‘‘CONTROL ROOM’ and Head (S&E) would take charge and coordinate activities from the Control Room depending on Severity of situation. 2. In case of any Operational contingency in the production system, MCS SCADA or Gas Field Emergency Control Room would serve as the Control Room. Head Production (Gas) or Chief Engineer Production (Gas) would take charge and coordinate the operation depending upon severity of situation. 3. In case of any operational contingency in Oil Production installation, OCS-5 would serve as Control room, General Manager Production (Oil) or Head Production (Oil-Service) would take charge and co-ordinate the operation depending upon severity of situation. 4. In case GCS-5 is affected due to Fire / Explosions Central Fire Station would serve as the Control Room for 2 & 3. 5. In case of major crisis linking many installation involving injury to personnel, relief etc. Disaster Control Room at S&E department would be declared by RCE / Head of Fields as the main Control Room and accordingly Nos. of additional phones, satellite connections would be notified. 6. In case emergency situation of Drilling / Work over well the Radio Room inside the Industrial area (near the drilling yard) would serve as Control Room.

RESPONSIBILITIES

3. Will arrange for the Reception of casualty at OPD by mobilising GDMO’s. Nurses and Attendant.
4. Will open Emergency Registration Counter to maintain the Records of all Patients
attending the Hospital.
5. Open an information centre at the OPD preferably manned bye a Doctor to
provide necessary information/help to the attendants.
6. To open a temporary morgue and arrange corpse disposal service. PHO are to be
informed.
7. Make provision for extra bed at casualty. 8. To liaise with Head (MS) and Dr. J.K. Goswami. 9. To keep casualty in the following order:
a. Minor—Advice & Discharge b. Serious c. Critical—Immediate attention d. Dead- on arrival

CONTIGENCY PLAN FOR EXTENDED HOSPITAL FACULTY:
1.If the load in the Hospital is excess or in the event of the Hospital being damaged Madhya
Zaloni Primary School to be converted into a temporary Hospital with 100 beds.
2. Alternately, Oil H.S. School to be converted to a temporary Hospital with facility to
accommodate 100 patients.
H. Action plan:
1. On Hearing the Alarm, all officers to report to Hospital Command Room. 2. CMO (Casualty/ PP will dispatch the First Aid Team and make arrangement for Reception of
casualty at OPD. 3. DGM (MS) CMO (S) will depute doctors to the wards. 4. Additional ambulance service to be made available.

I. STANDING ORDER WHEN A WELL KICKS IN A DRILLING WELL AND THE DUTIES OF PERSON EMPLOYED ON THE RIG:
SHUT-IN PROCEDURE
I) WHILE DRILLING 1. Stop Rotary. 2. Pick up Kelly to clear tool joint above Rotary table. 3. Stop mud pump and check for inflow. If yes,
- Raise Alarm - Close the Well by any the shut-in method.
II) WHILE TRIPPING: 1. Raise Alarm 2. Position tool joint above Rotary Table and set pipe on slip.

IV) To effectively kill a kick the end of the string must be closed to the bottom of the well so that
bottom hole pressure can be monitored and use of excess mud weight can be avoided. So if a kick
is detected during tripping the D. E. should take the following steps:
1. Sound alarm
2. Make up the internal BOP depending on the severity of the kick; he will take
either of the following steps:
CASE- A- In case the kick is not severe i.e. rate of inflow is negligible:
1. Run in the string to bottom as far as possible, carefully comparing the actual displacement
volume against theoretical displacement volume, while running in care should be bottom to
fill up the string with mud.
2. After reaching bottom make up the Kelly cock. 3. Open choke line 4. Close annular B.O.P 5. Slowly close the choke line valve 6. Make up Kelly 7. Take steps to record shut in drill pipe and casing pressures
CASE-B- If the kick is very severe the D.E. should take the following steps:
1. Open choke line 2. Close annular preventer 3. Slowly close choke line 4. Record annulus pressure 5. Arrange for stripping the string to bottom
The stripping procedure is briefly described as under:
1. Reduce closing pressure on the annular preventer. 2. Strip into the well bore with B.P V. on the string. Bleed off required volume of fluid into the trip
tank with hand adjustable choke. Drilling fluid volume bleed off should include volume required for
proper gas bubble expansion.
3. After reaching bottom, any standard well killing procedure may be adopted to circulate the kick
out. It is to be noted that stripping operation should always be done under proper guidance and
supervision.

II. STANDING ORDER FOR WORKOVER WLL BLOWOUT
SHUT- IN PROCEDURES
When one or more of the warning signs are observed, immediate steps should be taken to
shut- in the well. If there be any doubt, it can always be checked up afterwards as even a
small flow turn into big blowout in no time.
A) WHILE DRILLING
a) Raise Kelly until the tool joint is above the Rotary table or working platform.
b) Stop pump c) Close B.O.P. d) Inform IM
f) Read and record the shut- in tubing pressure, the shut- in casing pressure and the pit
gain.
B) WHILE TRIPPING
a) Set top tool joint on slip (If cut- of-hole run back.) b) Install and make up shut-in-valve in open position.
c) Close shut-in valve and B.O.P. d) Pick-up and make circulation gear. e) Open shut-in-valve. f) Kill outfit engine. g) Inform IM
h) Read and record the shut-in tubing pressure, the shut-in casing pressure and the pit
gain.
Installing a full opening shut in valve instead of a drop in type valve is advisable as further operation such as running in of wire line tools if become necessary can be done through shut-in valve only.
C) REPONSIBILITIES FOR SHUT-IN
Each member of the crew has different responsibilities during various shut-in/killing procedures.

v) Read & record shut-in tubing and casing pressures and pit gain. D. KILL PROCEDURES
The following variables are generally considered in killing a well.
i. Present following status ii. Magnitude of the formation pressure iii. Formation fracture pressure iv. Casing & Tubing burst pressure v. Limitations of the working string such as packer leaks, tubing holes etc. vi. Limitations of surface Equipment
vii. After studying all known & unknown variable the kill procedure an be evolved under a
given situation.
III) PRECAUTIONS TO BE TAKEN AFTER A BLOWOUT HAS OCCURRED-
a. An area within 500m. Of the well shall be demarcated as danger zone. b. An electrical installation within the danger zone shall be de- energised and an alternative gen. set should be commissioned for area lighting. c. Only approved Torches or Safety lamps shall he used within the danger zone. d. No naked light or Vehicular traffic are to be allowed within danger zone. e. A portable gas detector should be made available to indicate the presence of inflammable gas. f. Person engaged in Well control should be equipped with gas makes. Adequate fire fighting equipment must be mobilized at the well site at the earliest. It may be necessary to provide additional sources of water for fire fighting. g. An Emergency Medical unit should be arranged outside the danger zone to render prompt medical help as and when required. This has to be equipped with minimum life saving. Drugs. One ambulance or any suitable vehicle must also be kept stand-by near the site. h. temporary accommodation like tents with canteen facility should be erected near the site. i. proper Radio or Telephone communication system must be made available near the site. A Control room at Duliajan to be established & should be manhed’ round the clock by officers of Sr. E. D., Or SED level. j. Civil authorities are to be notified to warn the local inhabitants about the possible fire hazard. The danger zone should be preferably cordoned off by Security personnel. k. Transport facility to place additional pumps and tankages should also be available at site.

Control Room of PS.1. He will arrange to install one temporary telephone at site of
accident with Dy. CE (T).

ROLE AND RESPONSIBILITIES
1. WATER SUPPLY On declaration of Emergency situation. Head (FE) and his team would organise
availability of Water supply for Fire fighting and drinking water requirement. He
would check up the feasibility of maintaining water from central water supply
station, failing which he would commission available Tube wells. He would
periodically check up the ready availability of all tube wells in the housing and
other areas and the feasibility of operation them through portable generating set.
2. TRANSPORT AND SALVAGE EQUIMENT Head (Transport) would check up the fleet and drivers availability or- all Rescue,
Salvage, Transport operations and provide the services.
3. TELEPHONE COMMUNICATION On declaration of Emergency situation, In-Charge (FC) would check up the
feasibility of operation the telephone exchange on mains supply. He would keep
ready such alternate arrangements to operate the exchange with portable gen.
Sets. He would ensure that emergency telephones at declared control rooms are
made available readily and available wireless sets at critical operational centers
are in working condition.
4. ELECTRICITY One declaration of Emergency situation, Head (Elect.) and his team would ensure
that Power supply is cut off immediately where ever required so. He would try to
maintain Power supply at critical locations like Hospital, Medical relief centers,
Water supply sources etc. Head (Elect.) and Head (Mech.) would organise to
provide electricity at essential centers, in case Mains power supply from Power
station could not be made available there.
5. CIVIL ENGINEERING JOBS One declaration of crisis situation, Head (Civil) would organise gangs for debris
removal, construction of temporary camps, repair of Roads & Bridges, flood
control measures and provision of temporary Kitchen & Sanitation facilities.
NOTE:
During the critical phase of emergency, all out efforts would be made for meeting
the needs of rescue, relief and fire fighting operation. During this phase
maintenance of services like Gas, Water and Electricity for other purposes would
receive lower priority. While restoring services, high priority would be given for
maintenance of drinking water supply, then gas and electricity.

CONCLUSION
Even though the key contact personnel chart and role clarity are spelt
out, experience has shown that in critical situation like a disaster, there is
invariably some sort of panic at the beginning. At this stage, the
confidence of the leader should be of highest order, as he has to ensure
awareness of the functions, sub-functions and command system, so that
everybody stars playing their roles as per the chart and the whole
machinery starts moving in the desired direction. Each Co-ordinator
must have a manual with him which will help him to train the personnel
under him. This can be achieved if regular mock drills are undertaken.
Each and everyone will become more conscious of the type of roles they
will have to play to successfully combat the crisis situation.
