Insurance Information

The NSW Junior Rugby League Association on behalf of all insured persons has arranged this plan.

Who can make a claim?

• Any person who is a registered player, coach, trainer, manager, administrator, voluntary worker, selector, referee, touch judge or ball boy of a club that is part of the NSW Junior Rugby League Districts scheme who has been in-jured whilst playing or engaging in Rugby League, training for, or travelling to and from a club match or participating in an administrative or social activity.

• Age limitations apply - Players; 4 to 65, administrative personnel up to 85.

• Registered NSW Junior Rugby League players whilst playing Rugby League for their schools.

• Players must be appropriately registered before taking part in a game.

What benefits am I entitled to?

Section A Death and Capital Benefits

Coverage for death and some permanent disabilities isprovided under the NSW Sporting Injuries Insurance Scheme arranged by your junior league. Refer to your club for details.

Section B Weekly Benefits

Weekly benefits are payable as follows whilst you aretemporarily totally disabled and wholly and continuously prevented from engaging in any occupation and/or attending school and/or studies.

Income EarnersIncome earners are entitled to 100% of gross income (net ofbusiness expenses) up to a maximum of $300 per week for 52 weeks. A 14-day excess applies.

Non-Income EarnersNon-income earners are entitled to 100% of authorised domestic help up to a maximum of $300 per week for 52 weeks. A 14-day excess applies.

Public and Products Liability and Professional Indemnity Limits

•Publicliability-$20,000,000($1,000excess)•Professionalindemnity-$10,000,000($500excess)

How do I make a claim?

1. Advise your club secretary that you wish to make a claim. They will provide you with a Sports Injury Claim Form.

2. Return the fully completed claim form, including the “Attending Physicians’ Statement” completed by your Doctor to your club secretary.

3. Your club secretary will verify the claim and forward the claim form to your District Administrator.

4. All claims must be advised to Cunningham Lindsey Australia Pty Ltd, who manage all claims on behalf of QBE Insurance (Australia) Limited, within 30 days.

5. When completing the claim form, ensure that you answer all questions and provide as much information as possible.

6. You must attend any medial examinations or other as-sessment organised by Cunningham Lindsey Australia Pty Ltd or QBE Insurance (Australia) Limited.

7. You must take all reasonable steps to recover from any injuries.

Who can I talk to about an issue or complaint?

• If you are unable to resolve any issue with Cunningham Lindsey Australia Pty Ltd, talk to QBE Insurance, ask for Accident and Health underwriting or claims staff on (02) 9375 4444.

• If the matter can’t be resolved, it will be referred to QBE’s Internal Dispute Resolution (IDR) representative.

• If the matter cannot be resolved internally, QBE will provide you with Financial Ombudsman Service (FOS) details - an independent external dispute resolution body.

Loss of Income Top Up Option Information

The Standard Loss of Income Benefit is $300 per Week(maximum).

Top up cover is recommended where you require a higher level of weekly benefits. Please review information on this summary document and complete the Application Form.

Important Notes:

• The weekly benefit payable is 100% of your average normal weekly income (net of business expenses and excluding bonuses, commissions, overtime payments and other allowances) up to a maximum of the additional benefit selected.

• The additional benefit is an extension to the NSW JRL Sports Injury insurance policy and subject to it’s terms and conditions. This is not a separate policy. Refer to the following website for full details and a copy of the policy wording - http://www.gowgatessport.com.au/ (take the link to NSW Junior Rugby League)

• The excess remains unaltered at 14 days.

• The maximum benefit period remains unaltered at 52 weeks

• In arranging this insurance it must be noted that we are providing advice only of a general nature and have not taken into consideration your or any other insured persons particular needs, requirements or circumstances.

• No additional coverage is in force until the completed application is received in our office with the payment applicable and the application is accepted by the Insurer.

Full-time StudentsFull-time students are entitled to 100% of authorised tuto-rial costs up to a maximum of $300 per week for 52 weeks. A 14- day excess applies.

Top up cover is strongly recommended. Please refer to your club secretary for an application form.

Parents Inconvenience ExpensesReimbursement of expenses incurred by parents in visiting an insured full-time student aged under 25 years hospital-ised through injury up to a maximum of $100 per day. The first 2 days are not covered. Maximum benefit is $2,000.

Section C Non-Medicare Medical Benefits

• You are entitled to a payment of 100% of ambulance costs and 80% of non-Medicare medical expenses after any reimbursement from your health fund. These expenses include private hospital accommodation, physiotherapy, pharmacy and dentistry.

• TheHealthInsuranceAct(Cth)1973 does not permit the Insurer to contribute to any Medical Expenses covered (whether fully or partly) by Medicare (“The Medicare Gap”).

• The maximum benefit payable is $3,000.• An excess of $100 applies to each claim (a $50 excess

applies if you belong to a private health fund and a $25 excess applies for ambulance only claims).

• A medical practitioner must certify all medical treat-ment necessary.

It is strongly recommended that separate private health insurance be taken out.

Section D Travel and Accommodation BenefitsReimbursement of travel and accommodation expenses for:• The injured person - expenses incurred in travelling to

hospital or a place of treatment where the travel is in excess of 100 kilometres; Emergency attendance to the injured person by the injured person’s family members which results in the need for overnight accommodation in either a registered hotel or motel.

• Limited to 80% of the actual travel and accommoda-tion expenses, with accommodation expenses not exceeding $150 per night, subject to a maximum of $1,000 per family member and a maximum of 4 family members, any one event.
