Score at dashed lines. Cut out pocket along solid black outline.

Fold at scored lines and glue side flap to back of pocket. Then glue bottom flap to back of pocket.

Glue onto inside front cover or onto the flyleaf of the book of your choice.

Hand-write call number/cutter number and book title in spaces indicated. Alternatively, you could type the call number and title on a sticker and attach it in the indicated space.
