The o b j e c t i v e of t h i s program e f f o r t is t o e s t a b l i s h a group o f m u l t i d i s c i p l i n a r y cance r p r e v e n t i o n r e s e a r c h programs as a n a t i o n a l long-term r e s o u r c e i n cance r p r e v e n t i o n r e s e a r c h . The cance r c o n t r o l o b j e c t i v e s i n c l u d e :

- Reducing smoking p reva lence t o 16 p e r c e n t ;- I n c r e a s i n g t h e pe rcen tage of women who have a Pap smear e v e r y 3 y e a r s t o 90 p e r c e n t ( a g e s 20-39) and t o 80 p e r c e n t ( a g e s 40-70);

- I n c r e a s i n g t h e pe rcen tage o f women aged 50-70 t o 8 0 p e r c e n t who have an annual p h y s i c a l b r e a s t examinat i o n coupled w i t h mammography.

The scope of t h e s e program p r o j e c t s a r e intended t o i n c l u d e s t u d i e s on a l l a s p e c t s o f immunologic r e sponses aimed a t d e f i n i n g e t i o l o g i c f a c t o r s and p a t h o g e n e t i c mechanisms. Basic s t u d i e s o f t h e immunopathogenesis, t h e d i a g n o s i s and t h e t h e r a p y o f t h e s e d i s o r d e r s are encouraged.

Core r e s o u r c e s and f a c i l i t i e s t o be used by i n d i v i d u a l l y - s u p p o r t e dp r o j e c t g ran - t ees i n o r d e r t o c o o r d i n a t e t h e i r a c t i v i t i e s . I n p a r t , t h e l a c k o f p r o g r e s s i n e l u c i d a t i n g t h e dynamics of a n e s t h e t i c a c t i o n h a s been due t o t h e fo rmidab le n a t u r e o f t h e s c i e n t i f i c problem, a s it h a s been d i f f i c u l t t o a s s i g n a d i r e c t c o r r e l a t i o n between molecular e v e n t s t h a t are i n f l u e n c e d by a n e s t h e t i c s and t h e physj .ologica1 effects t h a t subsequen t ly occur .

Recent developments i n t h e b i o l o g i c a l s c i e n c e s now o f f e r t h e o p p o r t u n i t y t o a d d r e s s problems fundamental t o o u r unde r s t and ing of a n e s t h e t i c a c t i o n i n a manner t h a t was ex t remely d i f f i c u l t even a decade ago. Advances i n recombinant DNA t echno logy and molecular b io logy , i n b i o p h y s i c s and s t r u c t u r a l b io logy , i n t h e neu rosc i ences , and i n pharmacology and b iochemis t ry now p rov ide an o p p o r t u n i t y f o r i n v e s t i g a t o r s i n v a r i e d f i e l d s o f r e s e a r c h t o channel t h e i r e x p e r t i s e and knowledge toward e l u c i d a t i n g t h e a c t i o n s o f compounds t h a t e x h i b i t a n e s t h e t i c p r o p e r t i e s . Coupled t o what is p e r c e i v e d as an i n c r e a s i n g number o f i n v e s t i g a t o r s who are i n t e r e s t e d i n b a s i c r e s e a r c h problems r e l a t e d t o a n e s t h e t i c a c t i o n , t h e a b i l i t y t o i n c o r p o r a t e recent methodologica l and concep tua l developments into anesthesia r e s e a r c h offers t h e p o s s i b i l i t y o f g a i n i n g new i n s i g h t s about a n e s t h e t i c a c t i o n t h a t were h a r d l yp o s s i b l e i n t h e r e c e n t p a s t .

Dysphagia i s o f t e n a s s o c i a t e d w i t h n e u r o l o g i c d i s o r d e r s i n which t h e r e is i n t e r f e r e n c e w i t h t h e o r a l and pharyngea l phases o f swal lowing. These n e u r o l o g i c d i s o r d e r s i n c l u d e s t r o k e , m u l t i p l e s c l e r o s i s , P a r k i n s o n ' s d i s e a s e ,amyotrophic l a t e r a l s c l e r o s i s , c e r e b r o v a s c u l a r dementia , and head t rauma. The p r e v a l e n c e of dysphagia a t s p e c i f i c p o i n t s fo l lowing t h e o n s e t o f t h e s e c o n d i t i o n s remains u n c e r t a i n and t h e r e i s a p a u c i t y o f i n fo rma t ion about t h e neuropathology o f dysphagia , i t s d i a g n o s i s , and t h e e f f e c t i v e n e s s of t r e a t m e n t .
