Support Patients through E-Servi-ce Solutions (SPES) aims at transfe-rring the positive approach and re-sults achieved in the e-Health sector to 4 cities in the Central Europe Area (see Fig. 1 below).

The SPES project is focused on 4 different pathologies in 4 different lo-cal contexts, which are: Ferrara (Ita-ly) for respiratory problems, Vienna (Austria) for dementia, Brno for peo-ple with different abilities and Kosice for social exclusion.

The SPES project wants to imple-ment a technology platform, conne-cted to different medical devices and installed at patients’ home, thus creating both a line with medical care providers or with other patients and a system able to monitor the pati-ents’ health status or to provide help to people by, for example, suggesting things to do.

On the other side, SPES wants to create a positive cooperation be-tween regional forerunners and stakeholders, international partners and decision makers to improve the awareness of tele-medicine solutions that are already available.

The SPES project is funded by the European Regional Development Fund and implemented through the Central Europe Programme of the Eu-ropean Union.

Project MissionThe SPES project wants to demon-

strate that technological solutions can improve the quality of life of patients with chronic and serious diseases and of their families. Most of these patients are aged people who have reduced move-ment capabilities and some difficulties in going to periodic medical examinations to the hospital or medical centres.

The ageing of population in the Euro-pean Area represents one of the largest challenges to be faced in the next few years.

Different government bodies, like the European Commission, the Members States and the Regions, are facing the problem and looking for solutions able to improve the quality of life of aged po-pulation.

The technological solutions in the field of tele-medicine, like the patients’ home health care, the social inclusion tools not only addressed to young pe-ople or the monitoring systems for non self-sufficient people, are examples that can be easily evaluated both by decisi-on makers and patients.

Pilots LaunchBrno

The Boskovice pilot (Brno – Czech Republic) is focused on 40 mobili-ty impaired clients of a non-gover-nmental organization DEEP. The DEEPs’ clients are often illiterate in the field of assistive devices and mo-nitoring services including e-health concept. Therefore, as the first step, courses and appropriate training will be provided in a range of registered social services motivating seniors and disabled persons in the field of communication services, education,

social and professional integrati-on. DEEP team will select 40 clients that will take part in the SPES pilot testing of services bases on monito-ring through sensors and detectors – these clients will be located pre-dominantly in the city of Brno in the Czech Republic.

Currently, Deep clients are tes-ting GPS localization technology. Mrs. Lenka Ševčíková, who is registred with Deep for social service Emergen-cy care, has been selected as a Lai-pac S-911 bracelet user. Ms. Lenka is ready to share with us her experience with this assitive technology:

First I was not happy about the watch-like Laipac bracelet , rather I took it as a burden limiting my fre-edom because the dispatcher knows all the time where I am. This device gave me one extra duty by making me responsible for charging it .

The first days with Leipac were not easy. I have been receiving a call from an operator whenever I have made a quick movement with my hand, where I wear the watch. The operator kept asking me if I am OK, if I need doctor and/or if I am able to walk home on my own.

For example during a walk in a wood with my dog the operator called me to reassure that I am alright whe-never I threw a toy to the doggie. I kept explaining that I am throwing a ball to a dog, nothing serious. I was uncomfortable and nervous until I have slipped and Allen. Suddenly the operator’s question made sense: I started to realize that the main aim of Leipac is not to control me but to

provide me with help. Once, I have been mashroom picking with my husband and we have been attacked by wild bees. I was trying to chase the bees off by waving my hands but all in vain – we were badly hurt. At that time, it was great that the ope-rator new exactly our position and he could send us a help that drove us to the nearest hospital. Now, I under-stand that GPS localization device gives me great support and I feel so safe with it that I never leave my home without it. I can even afford travelling on my own by public trans-port because I can rely on the help of the monitoring operator whenever necessary. I am happy since Leipac made me much more independent. My husband and the whole family are no more worried when they leave me alone – they know I am safe. My freedom is not restricted by the device – on the contrary it is ensured by it!Client using panic buton

About neighbouring projectsThe SPES project wants to create

a network of stakeholders involved in tele-medicine and e-Health activi-ties with the objective to increase the awareness among the population and decision makers of the problem and of possible technical solutions.

The lesson learned from the four Stakeholders Meetings organized by the SPES partners in their regions is that there are a number of organiza-tions leading different projects and successful initiatives that, normally, terminate with the end of the project.

About SPES

The motivations are normally the fo-llowing:- the decision makers, who should finance the usage of technologies in social services, are not convinced on the effectiveness of such solutions;- stakeholders and project imple-menters have not the economical for-ce and the possibilities for creating a virtual process in such fields.

The way to overcome this problem is to create a network of stakehol-ders able to increase the specific wei-ght of each project and to increase

the echoes of its results. The SPES project has already come into con-tact with more that 40 projects on the territory and will soon start the collaboration platform.



Pilots Launch

KošiceThe Košice pilot is devoted to pro-

blem of elderly social inclusion that can be supported within suitable ICT solution. The aim is not to replace ty-pical face to face communication be-tween elderly, but to stimulate their interactions within social and enter-tainment services designed based on their requirements and expectations. Their daily routine will be enriched by various communication and social fe-atures for spending leisure or media-ting psychological support.

The pilot started with initial and preparing activities which include investigation of local conditions and dependencies, identification of rele-vant stakeholders for collaboration, evaluation of elderly situation and their interest for involvement, iden-tification of relevant IT equipment, internet providers and medical devi-ces, creation of a target group, admi-nistration procedures such as nece-ssary public procurements, etc. The official start of the 1st pilot phase is

expected during autumn 2012 after all preparation activities will finish successfully. The pilot management and coordination is under responsibi-lity of Municipality of Košice in coope-ration within Technical university of Košice as a technical support.

The tailored platform for Košice pilot provides simple and end-user friendly environment resulting from detailed analyses of elderly expecta-tions and requirements, suitable existing technical solutions and me-

thods, experiences from previous projects and selected medical devi-ces. The most interesting functionali-ties (modules) are textual messaging (chat or emails), audio or video calls, contacts with user profiles, multime-dia sharing, medical records visuali-zation, calendar and list of news ba-sed on user’s interests.

The pilot in Ferrara: supporting patients with respiratory disease.

The Ferrara pilot in Italy is tar-geting patients who are followed by pneumologists for periodical clinical controls, affected by chronic respira-tory failure and requiring long-term oxygen therapy and/or non-invasive mechanical ventilation. The aim is to provide patients suffering from bre-athing problems with a system that can remotely monitor their health sta-tus, by using a pulse oximeter and (if possible) other non-invasive medical devices while staying comfortably at home; and while complementary me-dical web applications will allow clini-cians and all the other professionals involved to access the patients’ data and to report clinical observations.

The SPES users in Ferrara will have the possibility to test a solution which is easy to use, lowering their commu-ting costs and decreasing the time needed to visit the health structures (hospitals, medical centers), impro-ving their quality and their overall well-being.

The methodology implemented in the pilot in Ferrara is observational,

prospective, mono-centric, crossover and before/after study. The clinical team from the Pneumology Surgery of the Local Health Authority of Fe-rrara has identified a sample of 40 patients, and the testing phase will last a minimum of 16 months.

Starting from May 2012, the first 2 patients have been taken in charge by the experts team of the project SPES, beginning to perform their me-asurements through the pulse oxime-

ters and answering to a set of online questionnaires. To this aim, they have been provided with a tablet, connec-ted via Bluetooth to the device, and they have been properly trained on both the pro-ject aims and the use of the tools. The functionalities and the measu-ring activities have been described in an easily reada-ble handbook, alt-hough, at the same

time, both remote and on-site supp-ort has been granted.

By August 2012, the patients al-ready involved in the SPES system were 9. Regardless of some initial difficulties, which however lasted no more than a few days, all of them are presently able to perform the measu-rements and all the activities requi-red by the testing protocol conceived by the doctors.

Pilots LaunchVienna

The Vienna pilot (Austria) endea-vors to elaborate tailor-made soluti-ons for persons with dementia whose problems range from poor memory to complete disorientation. Among other health risks, many test persons are prone to acci-dents because they tend to lose their way in the streets or have problems to organize their everyday life in their homes.

Discussions with potential users, their families and caregivers as well as social workers and therapists have led to a focus being placed on loca-lization, orientation and reminder functions in everyday life as well as brain stimulation. The following five test cases were derived from the re-quirements identified:

•The test case ‘Orientation support’ provides the possibility to support persons in case of emergency with the help of GPS and GPRS locali-zation devices without restricting their movements.

•The test case ‘Preventing dange-rous situations’ refers to a system expanding the localization device by including an RFID alarm to be activated when a disoriented per-son gets into a potentially dange-rous situation, e.g. when s/he lea-ves his/her ‘personal safety area’ (garden, terrace, daycare centre, flat, house etc.).

• ‘Talking Key’ means that, with the aid of RFID tags, the person con-cerned can be reminded not to forget the keys when leaving the flat.

• ‘Finding Things again’ describes a function which helps people to find lost objects using RFID technology via a touch screen PC.

•In the test case ‘Brain Stimulation’ persons will use a touch screen PC to activate recall from their long--term memory. An individually combined selection of video clips, songs, music and photos can easily be accessed by touching the screen. Other reminders concern the daily routine of persons with dementia (reminder to take food, personal hy-giene, etc.).

Up to now, the functionality and user-friendliness of localization de-vices with GPS and GPRS in combina-tion with i-Phones and the user-fri-endliness of the touch screens for older persons with dementia have been tested. First software releases for the creation of personal electro-nic memory books together with the test persons whe-re tried out and discussed. The next step will be to test RFID sys-tems. Feedback is being collected from all people in-volved, including test persons, family members, and caregivers. The evaluation is based on internationally approved Quality of Life (QoL) measures for dementia.

On the whole, the employment of the above-mentioned forms of technology in the Vienna Pilot aims at raising the degree of security for disoriented persons both in the pu-blic space and in everyday life. The

overall objective of these attempts is to enhance the quality of life and self--determination of older persons with dementia. In the long run, the adop-tion of these technologies should lead to a decrease in the dependence on inpatient care and nursing due to dementia.

Role of technology within SPESFor seniors or people who already

encounter difficulties (cognitive, func-tional, financial, …) in their daily life, the evolution of information and co-mmunication technologies may first appear as hard to follow or even as a contributor to their isolation.

However, these technologies can also enable them to increase their autonomy and to safely stay as long as possible in their familiar environ-ment. Convinced by this and relying on its experience in ehealth-related projects, CETIC works in the SPES project a software solution adapted to the needs of those people, espe-cially those suffering from breathing problems, dementia, social exclusi-on or physical/mental disabilities. The patient application can be used on a touch screen terminal (without keyboard or mouse) or on a classical laptop. Its intuitive user interface has been especially designed to be easy to use by people who are less or even not familiar with computers. It offers numerous possibilities around:

– communication: it breaks the isola-tion of its users by enabling them to communicate with family members, friends or other caregivers using:

•a Google-based calendar that can be shared with relatives to inform them or invite them to an event ;

• integrated and easy-to-use video--conferencing or messaging (sen-ding/reception) systems ;

•an address book with the coordina-tes of all contacts ;

•shared photo albums.

– health telemonitoring:

•several wired or wireless medi-cal devices like glucometer, pulse oxymeter, sphygmomanometer or weight scaler can be connected to the software solution. The collected medical measures can be graphica-lly visualized by the patient. They are also sent to a central database to build a history of the patients’ he-alth status which can be consulted on web portal by a physician, a nur-

se or any authorized operator. The data can also be analyzed in real time to trigger alerts and detect as soon as possible potential problems.

• the patient is also regularly invited to answer questionnaires on his/her general health status and well-being but also on his/her specific disease.

– people geo-location and tracking: the real-time position of less autono-mous people who gave their consent to be equipped with a GPS/GSM/GPRS geo-location device can be obtained and tracked indoors and outdoors by operators on the web portal.

The patient software can also run on a Windows tablet to provide him/her with all functionalities in a mobili-ty context (patient outside the home or on holiday).

Project NewsKick-off metting

The SPES project started its acti-vities the 1st April 2011 and the Kick off meeting launched the project on the 18th and 19th April 2011.

The two days meeting was opened by the representative of the Provin-ce of Ferrara, where the event was hosted, Mr. Massimiliano Fiorillo, As-sessor for Health and Social Care re-presenting the interest of his organi-zation in such types of project.

The meeting was attended by a re-presentative of each partner.

The first day of the meeting allo-wed the partners to increase the knowledge of each other and to ex-change ideas on their interest and vision of the SPES project with an in-depth discussion on different aspects, from management issues to software related and pilot implemen-tation ones.

The second day started with a discussion on management issues where particular aspects of the con-tractual links were examined.

Marcella Zappaterra, President of the Province of Ferrara, Alessandro Martelli, Director of the ENEA Center of Bologna, Giulia Angeli of CUP2000 Spa and Paolo Saltari, Director Ge-neral of AUSL Ferrara presented the opening meeting of the Europe-an project SPES - Patients Support through E-services- Solutions.

The second day ended with a Press conference where Marcella Zappa-terra, President of the Province of

Ferrara, Alessandro Martelli, Director of the ENEA Center of Bologna, Giulia Angeli of CUP2000 Spa and Paolo Sal-tari, Director General of AUSL Ferra-ra presented the driving reasons for such a project in the territory of the Province and the importance of using tele-medicine solutions for increasing the quality of life of the population, in particular ageing and diseased pe-ople.

ner presented the main activities and expected results of the project and the partners had the opportunity to know better each other and to defi-ne the plans for the application of the ICT innovative solutions developed by the project.

Olga Štěpánková and Daniel No-vak (CVUT) presented the dissemi-nation main activities focused on the wide promotion of project results. First of all, an article titled “SPES: an integrated ICT platform for social

and health tele-care” has been sent to “Handbook of Research on ICTs for Healthcare and Social Services: Developments and Applications” to start the wide diffusion of the project contents.

Sébastien Rousseaux (CETIC) pre-sented the basic structure of the sys-tem to be implemented through the SPES platform, to be improved throu-gh the project. In each pilot area the responsible for the technical aspects is identified, in charge to support the identification of the technical requi-

rements as well as the devices to be selected and integrated for the im-plementation of each pilot.

Moreover, the pilot projects in each country was presented by the partners. The effectiveness of the pilots and the impact of the tool will be supported by the organisation of stakeholders’ meetings, the mapping activity, the eHealth platform and the Green and White, presented to project partners during the kick off meeting.

Project News1st Strategic Political Committee Meeting

In the framework of SPES, a Strate-gic Political Committee (SPC) has the assignment of contributing towards an area of tele-assistance in Central Europe through cross-border colla-boration, initiating a dialogue on the supraregional level and promoting innovative developments between the partner regions participating.

This first SPC Meeting took place in Vienna on 20th April, 2012. The meeting offered the opportunity for an exchange of ideas with the aim of establishing a platform towards be-tter co-operation between the regions participating in the issue of tele-as-sistance applications on the Central European level. The Strategic Political Committee was officially established at the first SPC meeting.

During the event participants got informed about the ongoing Central Europe Project SPES. They got invol-ved in the cross-border cooperation and innovative development of assis-tive technologies on the Central Euro-pean level. They were provided with information about running activities, results and challenges from SPES Project Partners. The meeting was a successful get together from Central Europe regions in the field of e- He-alth, Telemedicine, and Ambient Assi-sted Living.

At the meeting SPC members have started a discussion about three im-portant concepts that are relevant for the SPES project. These concepts are e-Health, Telemedicine, and Ambient Assisted Living (AAL). As a first step a common definition of these concepts were defined by SPC members.

• E-Health – covers aspects related to the storage of the medical data in an electronic way (normally these data are obtained from machinery equipments that are able to provide electronic version of the exams);

• Telemedicine – covers aspect of re-ducing distances between patient and medical personnel. It works well in specific contexts like small ships in navigation, little emergencies or first aid/suggestions;

• Ambient Assisted Living (AAL) – co-vers the aspect of making the hou-se, and in general the places whe-re the patients live, a better place, more comfortable and responsive.

Participating SPC members identify these as three different aspects that each pilot will cover during the pro-ject duration in different percentages. Corresponding SPES project partners define percentages according to the pilot they are re-sponsible for and thus improve the understanding of those different approaches, in each region.

The SPC mem-bers further tried to insert a general approach that can be considered as an umbrella over them. Some su-ggestions from b ra ins to rming discussion are: augmented assi-stance, tele-as-sistance, smart assistance. Some

others can be: e-Care; e-Assistan-ce, but these two have been taken in ICT sector and indicate, often, the assistance related to computer use. E--Care is used by CUP2000 as name for services devoted to about 3.000 seni-ors living in the Provinces of Bologna and Ferrara. Figure depicts for each pilot and for SPES in total how much of each topic (AAL, e-Health, and Te-lemedicine) is being addressed in the on-going work.

The participants further try to iden-tify, for each pilot, the level of aware-ness of those concepts in normal po-pulation, giving an idea of the global acceptance of them. However, during the discussion, different levels of awareness have been indicated, even by partners from the same country/city, indicating the fact that these concepts are not well defined in gene-ral, and not only in the SPES project.

Stakeholder MeetingsBrno – September 2011

The Brno Stakeholder Meeting was organized by the NGO ProDeep in cooperation with Czech Technical University in Prague. The main goal of this meeting was to interchange experience and ideas in healthcare, mainly in Moravia region. Clients and healthcare-social providers came from the following institutions: Czech National Disable Council, Czech As-sociation of Disabled People, United Organisation of the Blind and Visua-lly Impaired CR, Home for the elderly Vychodilová Brno, Southmoravia Re-gion, Habrovanský Castle.

The meeting was opened by di-rector of NGO ProDeep Adela Kolou-chova who presented the main idea of the SPES project. Several invited speakers talked about technological and social challenges in healthcare area: JUDr Jitka Tesařová from Brno Municipality explained principles of applied emergency care, Radek Fiala from CleverTech company introdu-ced Senior Inspect project, Sebas-tian Rousseaux from Cetic company described touchpad interface for el-derly, Lenka Lhotska from CVUT pre-sented various novel assistive tools ,

Adam Sporka from CVUT commented experience from EU research project I2Home, Jitka Vychodilova summari-zed her experience in technology use in senior house Vychodilova in Brno.

All participants were asked to fill in the questionnaire about assistive technology in three domains: soci-al, strategic and technology area. In total , 36 forms were collected. The participants prioritized in their an-swers remote management of hou-sehold, remote fall detection and means ensuring communication with person in critical situation.

Ferrara – October 2011SPES project plans the implemen-

tation of a series of stakeholders me-etings aiming at the involvement of local actors, as to guarantee a wider impact and exploiting of the platform as well as the project sustainability.

The first meeting was held in Fe-rrara on October 13th 2011. The stakeholders participating to the me-eting, both employed in private com-panies and representatives of public bodies (Local Health Authorities or Municipalities), stated that their pro-jects stemmed from the necessity to build more efficient networks, with greater interconnection possibilities among different professionals. The representatives from public bodies stated that the main aim of their pro-jects is to realize services for timely interventions, involving the co-parti-cipation of both social assistants, and health operators. These were the very subjects referred to by the pur-chasers, for the identification of the

instruments and processes to build the services belonging to the field of telemedicine and teleassistance.

Essentially, all the projects presen-ted during the meeting saw, before the start-up of the operative stage, the realization of a background ana-lysis on the target they would address. Epide-miological data were gathered on the popula-tion of reference, toge-ther with information on the existing supporting services; some projects provided the realizati-on of focus groups with the interested profe-ssionals, others were directly realized by the professional, so that the analysis was concentra-ted on their needs and on the experience grew in the field.

The experiences of the meeting participants will be taken into consi-deration for the SPES project succe-ssful implementation.

Stakeholder MeetingsVienna – November 2011

The Vienna Stakeholder Meeting was organized by the Vienna Social Fund in cooperation with the Vienna Umbrella Organization of Social In-stitutions. The Meeting took place on 15th November 2011. There were 48 registered participants from various organizations, including research in-stitutes, social services and private companies.

Several participants presented their latest developments and expe-

riences in the areas of Telemedici-ne, e-Health and Ambient Assisted Living. The main topics discussed include reducing stays in hospi-tal, prevention of emergency cases through intervention in time, preven-tion of health deterioration, intelli-gent housing networks for safe and self-reliant living, automatic identi-fication of critical events, acoustic, visual and multi-sensory functions, automatic fall identification, social

networks and media, playful training of cognitive functions, promotion of mobility, medical monitoring and treatment via technical aids, relief of nursing care services, relief of fami-lies and relatives, enhancement of care quality, localization of disorien-ted persons, ethical issues, financial issues, language recognition, digi-tally supported neighborhoods and innovation barriers and the lack of modular systems.

Košice – September 2012The Košice Stakeholder Meeting

was organized by the Municipality of Košice in cooperation with Technical university of Košice as involved part-ners in the SPES project. The aim of this meeting was to present main ide-as and objectives of the SPES project in strong connection with identification of similar initiatives in domains of tele-medicine, eHealth or ambient assistant living in Slovakia. The expected result of this meeting was represented by a prospective collaboration in mentioned domains in order to improve the dai-ly routine of our elderly with state of the art ICT solutions. The participants (around 50) consisted of not only peo-ple from Košice city, but this meeting also welcomed the representatives of other local governments from the regi-on of the eastern Slovakia.

The program of the meetings inclu-ded an official introduction by Mayor of the Košice city and six invited spe-akers: introductory presentation of SPES project with detailed description of Košice pilot objectives and expecta-tions; presentation of social service called S-fon operated by Municipality for local elderly; company called YMS which presented Electronic Senior Guard; TnTEL Company with Wireless communication system for nurse-pati-ent TREX; FP6 IST project MonAMi re-presented by responsible person from Technical university of Košice (Open system with monitoring services for seniors and home automation) and Lo-cal Red Cross organization with its so-cial services and projects. One of the main results of this Stakeholder mee-ting is a prospective contract between Municipality of Košice and YMS com-pany regarding the implementation of presented ICT solution in Košice city. Local WorkshopsFerrara - September 2012

The Ferrara SPES Workshop was organized by the Local Health Autho-rity of Ferrara, together with the Mu-nicipality of Copparo on September 22nd 2012.

There were 13 registered partici-pants from various organizations, such as Main City Hospital St. Anna of Fe-rrara, the Copparo Social Services, Lo-cal Health Authority of Ferrara itself.

Different categories of healthcare professionals were represented: ge-neral practitioners, pneumologists, nurses, social workers, Local Autho-rities and services commissioners. The meeting aimed at presenting the objectives and main achievements of the SPES projects and specifically the purpose of the pilot in Ferrara.

CalendarThe schedule of SPES is full with

different initiatives, due to the im-portance the European Commission, the National States and Local Gover-nment credit to the use of technical solutions, like tele-medicine or e-He-alth systems, to increase the quality of life and the well-being of popula-tion.

In the next year of the project, from October 2012 to September 2013 the planned activities are:

1) 2nd Strategic Political Committee meeting, Bologna, Italy, 19 April 2013;

2) 4th Steering Committee Meeting, Bologna, Italy, 18 April 2013;

3) 2nd Stakeholder meeting focused on active telemedicine projects, Bologna, May 2013;

4) 5th Steering Committee Meeting, Prague, Czech Republic, September 2013;

conducted by the doctor Oreste Ca-pelli, pneumologist collaborating with the Emilia-Romagna Region. He acknowledged the value of the SPES project and the potential of teleme-dicine for renewing social and health services

Due to the interest expressed by different organizations, like the Regional Service for Health and Social Services, this list will be increased with local initiatives focused on disseminating the SPES results and increasing the awareness on available technical solutions for tele-medicine.
