Many of the XYZ affiliated faculty are nationally and internationally recognized experts in the social and natural sciences, including anthropology, sociology, geography and human biology. The collaborative nature of this degree links these experts with experts in the College of ZYX as well, which will situate UK at the forefront of innovative, inter-disciplinary programs in population-based alphabet disparities and public welfare. The XYZ program has recently acquired a specialized piece of equipment that is uniquely suited to the study and research required for the XYZ program.

Admission Requirements: 1. Applicants must submit a formal applicaton to the Undergraduate Admissions Office indicating XYZ as you rmajor; 2. Applicants must meet the minimum criteria for admission to the University as specified in the Bulletin, the XYZ program requires a minimum of a 2.0 gpa on a 4.0 scale for eligibility into the program; 3. Applicants must successfully complete the aptitude tensting deisnated by the XYZ program.

Retention Requirements 1. Students must maintain a 3.0 GPA on all program coursework. 2. Earning a C or lower on any program coursework results in academic probation and the student must retake the course; if a student earns a second grade of C or lower, the student may be dismissed from program.

10d Describe how the proposed program will articulate with related programs in the state. It should describe the extent to which student transfer has been explored and coordinated with other institutions. Attach all draft articulation agreements related to this proposed program. (300 word limit)

Many of the potential career opportunities will provide work-study and internship possibilities for the students. There will also be opportunities for the students to collaborate with various non-profit groups located in Lexington and the state. The students will be introduced to the many possibilities for collaborations in XYZ 201 and XYZ 202 and encouraged to begin their optional independent study activities as early as possible. After the XYZ Program is established, student exchange programs will be developed with other KY institutions. Other KY institutions have unique expertise and capabilities that would be valuable for UK XYZ students to obtain. Likewise, the XYZ Program could provide similar opportunities to students from other institutions. Collaborative exchanges with these institutions will be sought once the XYZ Program is sufficiently established to host off-campus students, and provide support for XYZ students to travel to other institutions. Collaborations with leading programs outside of KY will be important for the growth of the XYZ Program, student development and the generation of new ideas for courses and engagement activities. One opportunity would be to have one of our faculty visit the host institution to give a seminar and to observe their XYZ program. After the UK XYZ Program is established it should become a host to students from other institutions leading to a mutually beneficial two-way exchange.

10e Identify the applicant pool and how applicants will be reached. (300 word limit)

High School seniors will be provided information about the XYZ BA Program at the beginning of the fall semester before they graduate. The College maintains a list of contacts for a large number of high school advisors and teachers. The advisors will be contacted by email typically by mid-August each year. The advisors will be provided a complete listing and description of all College majors in an electronic preview book. This book will include the XYZ program, the potential careers available to XYZ graduates, and a link to the program website where more detailed information can be obtained. Additionally, College recruiters will provide information to students attending UK See Blue Preview Nights and Come See For Yourself Events throughout the fall semester and beginning of the spring semester.

The proposed program is directly aligned with the University of Kentucky’s Mission Statement and Strategic Plan by providing students with an understanding of the interrelationships between society and our global environment. UK Goal 1: The university’s strategic plan directs that programs prepare students for leading roles in an innovation driven economy and global society. In addition, the program addresses Strategy 6.1 of the statewide postsecondary education strategic agenda by focusing on the the advancement of new knowledge and enlightenment.

The state’s strategic plan for postsecondary education focuses on four areas. Please choose which area(s) your proposed program relates to along with which policy objective(s) that your proposed program will address.

College readiness – Will the program increase the number of college-ready Kentuckians entering postsecondary education? Will it increase the number of college-ready GED graduates? Will it increase the effectiveness of Kentucky’s K-12 teachers and school leaders?

Student success – Does this program increase high-quality degree production and completion rates and close achievement gaps, particularly for lower-income, underprepared, and underrepresented minority students? Does this program decrease financial barriers to college access and completion.

Research, economic, and community development: Does this program increase educational attainment and quality of life in Kentucky communities through regional stewardship, public service, and community outreach?

Efficiency and innovation: Does this program increase academic productivity through program innovations? Does this program maximize the use of postsecondary and adult education resources?

This degree program will directly address one of the state's most pressing concerns, the health and well-being of Kentuckians. The Strategic Agenda states that it wishes to (6.1.) "Support the critical role that the University of Kentucky and the University of Louisville play in the creation of new knowledge and recognize universities and faculty members for the advancement of knowledge and enlightenment." New knowledge in the field of pubic health is an urgent need for the Commonwealth, a need this program will address. Furthermore, the program meets the criteria of 6.2.: "Support collaborative research efforts that leverage university expertise, lead to research investments and commercialization in high-growth or emerging areas, and are aligned with business and industry growth." The XYZ degree is a truly interdisciplinary program that draws on the expertise of several different areas within the University of Kentucky. We believe the program will position our students at the forefront of this emerging field of study, one that promises to be high-growth throughout the nation.

We expect this program will serve students in other health and sociology areas such as health management, preventive medicine, environmental health, sociology, health society and populations. Many of the courses will be cross-listed with other departments, providing a cross-fertilization of ideas and foci for students and faculty.

12b What will be the projected “faculty/student in major” ratio? (300 word limit)

The proposed faculty-to-student ratio will be 1:6. Based on the XYZ Acceditation supervision requirements, the highest faculty-to-student ratio can be 1:10; therefore, to remain an “approved course sequence” by XYZ Acceditation, the ratio will always remain below 1:10.

12c Describe the library resources available to support this program. You may attach any documentation provided to SACS. Access to the qualitative and quantitative library resources must be appropriate for the proposed program and should meet recognized standards for study at a particular level or in a particular field where such standards are available. Adequacy of electronic access, library facilities, and human resources to service the proposed program in terms of students and faculty will be considered. Sample response: The UK Libraries offers collections, services and other learning/information resources consistent with the degrees offered at the University. UK Libraries fulfills the University's educational, research, and service missions through the acquisition, organization, and preservation of relevant information resources that support the academic and research programs. The print collections are housed in the ten libraries across the campus and in the print archives. UK Libraries’ resources include: • 4 million volumes • access to 100,000 current serials, including 70,000 electronic serials accessible on and off campus • access to approximately 450 licensed networked electronic resources/databases • 550,000 electronic books accessible on and off campus • 100,000 audio/visual materials PLUS full wireless capability in all campus libraries provides access to electronic resources within library facilities. 12d Describe the physical facilities and instructional equipment available to support this program. Physical facilities and instructional equipment must be adequate to support a high quality program. The proposal must address the availability of classroom, laboratory and office space as well as any equipment needs.

The College has committed significant facilities, technical resources, and administrative support to promote the highest levels of teaching, research, and service for its faculty, staff, and students. Currently, the College has more than 32,000 sq. ft. of space to house faculty members. All faculty members are provided with individual work stations, including PCs, printing equipment, IP telephones, filing cabinets for storing confidential records, and access to a toll-free conference line. The College makes available to faculty several new, high-capacity copy machines, fax machines, and scanners as well as a color plotting printer to produce posters for presentations and promotional efforts. Other equipment available for faculty use includes digital audio recorders, digital still and video cameras, and LCD projectors. The campus network offers licenses for office, graphics, and statistical software packages to faculty and staff, and all faculty and staff can access email, Internet, and campus libraries and databases from their work stations and from connections to the free wireless network. In addition to providing IT resources, the College supports faculty research with an administrative staff that assists with office tasks such as duplication as well as research administration, personnel management, and accounting.

Academic majors using the title of XYZ are emerging with some frequency on campuses in the U.S. Nearly 30% of schools of public health accredited by XYZ National have already established baccalaureate programs with another estimated dozen institutions working on creating a program. Additionally, twenty other colleges and universities which are not XYZ National accredited are offering undergraduate degrees. Such a major has the potential to enroll large numbers of students, as is the case at Benchmark University (enrolling more than 300 per year). Students seek opportunities for local and global application of knowledge and analytic skills, experiential learning linked to civic engagement, practice of applied ethics, problem solving, and teamwork from the “population perspective.” XYZ topics introduce a new and highly relevant approach to national and global understandings as evident by the large enrollment of students each semester in XYZ 201 Introduction to XYZ at UK.

The XYZ will be the first undergraduate public health degree offered in the state by an accredited School of Public Health. College faculty guides millions of dollars annually in collaborative research, with funding sources ranging from MNOP and EFG, to the John Doe Foundation. This extramural funding provides numerous opportunities for students to participate in some exciting, innovative and ground-breaking research. Access to faculty in five additional UK XYZ careers colleges and numerous Centers and Institutes further expands opportunities for students. Distinguished faculty members include Smith, Jones, Washington, and Green.

Although the College pursues an active evaluation and assessment plan as a routine strategy, students pursuing this minor will undergo additional assessments. The evaluation model of Donald Kirkpatrick is well suited to this purpose. The four levels of Kirkpatrick's evaluation model essentially measure: a) reaction of students (what they thought and felt about the educational experience); b) learning (the resulting increase in knowledge or capability); c) behavior (extent of behavior and capability improvement and their ability to implement or apply); d) results (the effects on the career, citizenship or career practice resulting from the student’s performance).

Reaction evaluation is how the students felt about their interactions with advisors and instructors, and personal reactions to the learning experiences. This will be done periodically (i.e., at the conclusion of each semester) by their academic advisor or the XYZ Assistant Dean for Planning and Assessment. Behavior evaluation is the extent to which the students applied their learning and changed their behavior. Students self-evaluate their competencies before and after selected courses. Results evaluation is the effect on the students’ choices after graduation. Students typically receive a one year post- graduation survey about their readiness to enter and perform well in their chosen professional work.

The end of Fall semester of Year 1 for coursework and internship experiences. The end of Spring semester of Year 1 for application and acceptance rates and advisory committee reviews. Between 6-months and 1-year of completing the program, all graduates will receive the program surveys.

Faculty of the XYZ program will be the primary data collectors. Internship supervisors will complete the internship evaluation, which will be delivered to the faculty. All data will be shared with program faculty, who will analyze and report the findings to the department and college. In addition, program faculty will share data with accrediting agency for initial accreditation, as well as continued accreditation.

If SLO data show that students are not performing well in coursework, faculty will make adjustments to the courses to better meet the programmatic requirements needed to prepare effective XYZ analysts. If passing rates of XYZ National Accreditation examination are poor, faculty will re-evaluate coursework and internship experiences to determine which areas of improvement are needed. Program survey data will be used to make adjustments to the program to better prepare graduates for the job market.

The University of Kentucky teaching evaluation is the method we will use to evaluate teaching effectiveness. Each junior faculty member is assigned a mentor (senior faculty) who will assist in evaluating instruction. Finally, the program survey for completers will ask students to evaluate the coursework and instructors following their departure from the program.

Faculty can utilize UKs Center for Enhancement of Learning and Teaching (CELT), which is designed to provide feedback and support for faculty who receive poor teaching evaluations. CELT also provides technical and instructional assistance to any faculty, regardless of evaluation, to broaden the faculty’s repertoire of effective instruction techniques. Additionally, faculty can participate in professional development activities that are directed to improvements in university teaching.
