When am I Eligible for Benefits?
Contact your local Human Resource Department for the dates
and times of the one-on-one enrollment sessions with a
BenefitHelp™ Enrollment Counselor.
Waiving Medical Coverage
Under the employer responsibility provisions of the
Patient Protection and Affordable Care Act (PPACA)
eligibility for health benefits is determined through the
use of an initial or standard measurement period.
The standard measurement period (2/1 – 1/31) of each
year is used to determine who will be eligible to enroll
for coverage during the Open Enrollment period. An
11-month initial measurement period will be used for
variable hour employees throughout the plan year.
Full-Time employees expected to average 30 or
more hours per week - if you are hired as a full-time
employee, your benefits will become effective for the
medical and prescriptions drugs after 90 days. You
must go through the enrollment process timely for
coverage to be effective.
Variable Hour employees – if you are not hired as
a full-time employee, then you will be considered a
Variable Hour employee. Using an 11-month initial
measurement period, beginning the first of the month
following your date of hire, your hours will be reviewed
to determine if a 30 hour average was achieved.
If so, you would be considered full-time for purposes
of health benefit eligibility. You will be contacted
regarding your enrollment rights.
You may choose not to elect medical coverage under
the Isle of Capri Casinos group medical plan. But keep
in mind that the individual mandate provision of the
Patient Protection and Affordable Care Act (PPACA)
requires you, your children and anyone else that you
claim as a dependent on your taxes to have health
insurance in 2016 or you will be required to pay a
penalty when you file your federal income tax return.
Health Coverage under the Isle of Capri Casinos, Inc.
medical plans meets minimum essential coverage
guidelines and is deemed to be affordable using the
IRS federal poverty line safe harbor definition. This
means that if you are eligible for coverage under the
Isle of Capri Casinos, Inc. health plans you likely will
not qualify for a tax credit if you purchase your
coverage through the Health Insurance Marketplace.
If you choose to purchase health coverage through the
Marketplace, you will not be able to elect coverage
under the Isle of Capri Casinos, Inc. health plan until
our next open enrollment.

Adding Dependents
If you are adding dependents to the medical, dental
or vision plans for the first time during this open
enrollment, you must present the following dependent
verification documentation to your Human Resources
Department immediately following your enrollment.
If proper documentation is not provided by you by the
end of the open enrollment period, these newly added
dependents will not be enrolled for coverage during
the new plan year unless you experience a qualifying
life event.

Spouse Marriage License and the first page of your most recent joint tax return. (financials should be blackened out)
Natural Children Birth Certificate
Step-Children Birth Certificate and Marriage License showing both parent’s names
Dependent Child(ren): Legal guardian, adoption or foster
Birth Certificate, Final Court Order of legal guardianship with judge’s signature and/or final adoption decree with judge’s signature
ENROLLMENT PROCESS
You can enroll the following dependents into the same medical, dental and vision plans you choose for yourself.
Your legal spouse (including same-sex spouses);
Your under age 26 child, stepchild, legal ward or any child required by a Qualified Medical Child Support
Order (QMCSO). Under age 19 dependent children can be enrolled into the dental or vision plans.
Q. If my spouse also works for the Isle of Capri Casinos, what coverage level should we select?
A. If both you and your spouse work
full-time for the Company and are both eligible
for coverage you may both enroll in employee-only
coverage, or one of you can enroll in employee +
spouse coverage.
NOTE: If both enroll in employee-only coverage,
only one of you may cover eligible dependent
children.

To make any CHANGES to your current elections, you must participate in the enrollment process.
OPEN ENROLLMENT INSTRUCTIONS
2016 Open Enrollment Open Enrollment for the 2016-17 plan year will be held from March 1 - March 31, 2016. During this time, you
have the opportunity to make your benefit elections or make any changes to your current benefit enrollments
that will be in effect for one full plan year - May 1, 2016 through April 30, 2017. You will not be able to make
any mid-year changes to your medical, dental, vision, or FSA unless you have a Qualifying Life Event such as
a marriage or birth of a child.
It’s Time to Enroll in Your Benefits... The Isle of Capri Casinos, Inc. Benefits Package is designed to protect you and your family from catastrophic
losses. Keeping our annual healthcare increases affordable year-over-year takes a joint effort. We depend on
our employees to become educated healthcare consumers. Visit your primary care physician annually for your
free preventive screenings that are included in all of our medical plan options. When medical tests or surgical
procedures are recommended, use Anthem’s on-line quality and cost comparison tools. This will provide you
with the estimated out-of-pocket expenses for certain services and helps manage the cost to you and the plan.
Do I have to Attend an Open Enrollment Meeting? There are some changes to the Isle of Capri Casinos, Inc. medical plans effective May 1, 2016. We feel it is
important for all employees to review these plan changes, it will be necessary for you to attend one of the Open
Enrollment meetings being held at your property and to make an appointment to review the benefit plans with an
Enrollment Counselor if you wish to make any changes to your current benefit enrollments. Check with your local
Human Resources Department for available dates and times.
What happens if I do nothing during this Open Enrollment period?
For this upcoming new plan year that begins May 1,
2016 there will be 2 medical plan options. The
former $500 deductible plan is being eliminated.
If you are now enrolled in the current Medical Plan 1
($500 deductible plan) and you do nothing during
Open Enrollment, you will be automatically moved to
the new Medical Plan 1 ($1,000 deductible plan).
Current enrollments in the $1,000 and $2,000
deductible medical plans will carry over into the
same plan in the new plan year. If you would like
to participate in the health care or dependent care
flexible spending accounts you must make an active
election every new year. By law, flexible spending
accounts cannot roll forward into the next plan year.
To enroll, you must make an active election through
Employee Access. See page 16 for information on
our NEW Flexible Spending provider, Ameriflex.
All of your other current benefit plan elections (dental,
vision, and other voluntary plans) will carry over into
the next plan year.

Enroll for your health and welfare benefits using our online intranet site at
https://islenet.net by clicking on the Employee Access link at an Isle of Capri computer.
Once you’ve enrolled, your benefit elections will remain in effect until April 30, 2017.
VERY IMPORTANT! You cannot change or enroll in the Accident, Critical Illness, Cancer or Hospital Income
Plans without the assistance of an Enrollment Counselor.
HOW TO USE EMPLOYEE ACCESS
1. ►
Log on to https://islenet. Click on Employee Access. Your user name is the first initial of your first name and the first five letters of your last name and the last four digits of your social security number. If this is your first time logging into Employee Access, your password is your social security number. You will be prompted to assign a new password once you have answered security questions. (User Name Example: jsmith1234)
2. ► Click Benefits, then My Dependents to verify or to add spouse and/or child(ren) dependent(s). You will need a social security number and date of birth for each dependent(s) added. Proper documentation must be provided to your HR department following enrollment of any new dependent(s). (See page 4).
3. ► Click Benefits, then My Beneficiaries to verify or to add beneficiary information to your Life Insurance plans. Ensure the information is correct for the beneficiaries you wish to add/change including social security number, date of birth and address.
4. ► Click Benefits, then New Hire/Benefit Changes and/or Open Enrollment (as applicable) to begin making elections for your benefits.

Enter the Benefit Change Date and Benefit Change Reason. Benefit Change Date is the effective date of your benefits and/or the lifestyle change date. Benefit Change Reason is the reason for the event and you will select one option from the drop down. For Open Enrollment, review/change your Benefit Type plan elections and proceed to step #7 below.
6. ► Select the Benefit Type plan elections you wish to enroll. Be sure to select the Opt Out if you choose not to enroll in any of the medical, dental and/or vision plans.
7. ► Select Continue which is the right arrow on the top of page. Check the box next to the dependent(s) you choose to add to a medical, dental or vision plan. Check the beneficiaries you want to assign to your
Life Insurance plan. Select Continue which is the right arrow on the top of the page.
8. ► Assign Beneficiary, primary and/or contingent allocation(s) and percentage(s). Select Continue which is the
right arrow on the top of page.
9. ► Review your benefit elections. Read the statement and confirm your benefit elections by entering your
password twice. Submit your enrollment by selecting the disc icon located on the top of the page.
10. ►
A confirmation of your benefit enrollment will be placed in your Inbox My Messages. New Hire/ Benefit Changes and Open Enrollment will be reviewed and approved if all applicable documentation is received in a timely manner. Before closing, print your benefit elections and retain a copy for your records and Logout of Employee Access.

Step therapy is a program that helps you and your doctor choose drugs that are right for you.
After studying many drugs, Anthem has chosen certain ones to be the first drugs to try when
treating some conditions. Trying drugs in a step-by-step way is called step therapy. When your
doctor prescribes a drug that requires step therapy, a message is sent to your pharmacy’s
computer. This lets the pharmacist know you must first try a different, similar drug that’s
covered by your plan. The pharmacist will call your doctor to get a prescription for the new

Prescription Drug Coverage. Many of your prescription maintenance drugs can be filled through the Mail Order service - saving you one monthly copay (get a 90-day supply for only 2 monthly copays).
MEDICAL OVERVIEW
Terms and Definitions...
Deductible This is the amount of money you pay for health services before your medical insurance begins paying. For some services you have to pay the deductible before the plan pays. Your deductible starts over each May 1st.
Copay This is the amount of money that you pay each time a particular service is utilized.
Coinsurance This is the rate that you will essentially be splitting the cost of your healthcare. For instance, if your health plan has an 80/20 coinsurance rate, your insurance plan pays for 80% of your eligible medical expenses and you’re responsible for the remaining 20%.
Out-of-pocket Coinsurance Maximum This is the most you will have to pay under your medical plan each year. Includes copays, deductible & coinsurance. This protects you from the financial drain of high medical expenses. Copays do count towards your out-of-pocket maximum and still apply even if you meet your maximum.
In-Network / Out-of-Network If your medical plan has “in and out of network” coverage, this means you can see any provider you wish. However, if you choose to see a provider that is not on the approved in-network list, you will pay a greater share of the cost. Determining whether or not a provider is in-network is your responsibility. Please check with the provider to see if he or she is in the network before services are rendered, preferably when making the appointment.

Isle of Capri offers dental protection through
Delta Dental of Missouri. With Delta Dental of Missouri, you can receive care from any
dentist. However, Delta Dental has contracts with a large network of
dentists who have agreed not to charge more than a specified amount
for particular services. If you use one of these network dentists, you
won’t have to worry about being charged for additional amounts above
the allowable amount covered by the plan.

No member cards are needed with the Vision Service Plan. You may receive care from any vision care provider. However, Vision Service Plan has an extensive nationwide network of doctors who provide eye care and eye wear. When you receive care through these vision care providers, you simply pay the appropriate copayment. Flexible Spending Accounts (FSAs) enable you to put aside money for important expenses and help you reduce your income taxes at the same time. The Isle of Capri offers two types of Flexible Spending Accounts: a Health Care FSA and a Depend-ent Care FSA. These accounts allow you to set aside pre-tax dollars to pay for certain out-of-pocket health care or depend-ent care expenses.
How Flexible Spending Accounts Work 1. Each year during Annual Enrollment, you decide on how much to set aside for health care and/or
dependent care expenses. 2. Your contributions are deducted from your paycheck on a pre-tax basis in equal installments
throughout the calendar year. 3. File your claims through Ameriflex. Please note that these accounts are separate and you may choose to participate in one, both or neither. You cannot use money from the Health Care FSA to cover expenses eligible under the Dependent Care FSA or vice versa.
How do I file a claim?
Ameriflex offers a number options when it comes to submitting your eligible, unreimbursed expenses for payment through
your flexible spending account.
Ameriflex Convenience Card – The Ameriflex Convenience Card can be used for your eligible expenses at the time
of service. This card works like a debit card and pays the provider’s office directly. You may request multiple debit
cards to be used by your eligible dependents. You will want to keep copies of your receipts in the event that
verification is required.
MyPlanConnect – register your insurance carrier information through the Ameriflex member portal so they can
receive copies of your EOBs (explanation of benefits) directly from Anthem or any other health insurance provider.
Funds are directly debited from your FSA for distribution to you. There is no need for you to provide additional
documentation.
Mobile App – use your smart phone or tablet to take a picture of your claim/receipt of your unreimbursed
expenses and submit directly to Ameriflex for payment. Ameriflex will process the payment from the submitted
information.
Member Portal – log into your secure, online account through any internet connection to upload and submit
your documentation for payment from your FSA.
Payment is provided via a paper check or you may elect to have funds placed into your banking account through
a direct deposit option.

Company Paid Basic Life As an employee, you will receive a company paid life insurance and AD&D benefit equal to one times your base annual salary rounded up to the next thousand. The below noted maximums apply by each class:

Optional Term Life How Much Coverage Can You Buy?
Salary Employee: You can select supplemental life insurance in amounts equal to 1, 2, 3, or 4 times your annual compensation.
However, your total optional life insurance amount cannot exceed $750,000 (company provided and optional life combined).
Hourly Employee: You may purchase additional term life insurance in $10,000 increments to a maximum of $100,000.
Spouse: You may purchase term life insurance for your spouse in the amount of $10,000. You must have coverage for yourself
under the optional term life plan to have coverage on your spouse.
Child/Children: Life insurance in the amount of $5,000 is available for your children provided that you have optional life insurance

Guaranteed Issue:
Guaranteed issue means that you are guaranteed coverage without having to answer any health questions
or provide evidence of good health.
Salary Employee: You may purchase up to 3 times your annual compensation to a maximum of $750,000 (company provided and
optional combined) without providing Evidence of Insurability.
Hourly Employee: Guaranteed issue up to $100,000.
Guaranteed Issue for your Spouse is $10,000 and $5,000 for your children. For Salary Employees If you elect coverage equal to 4 times your salary (not to exceed a combined total of $750,000) you will be required to answer health questions on an Evidence of Insurability form. Cigna must approve all coverage amounts above the guarantee issue amount. You will be covered at the guarantee issue amount until a determination is made by Cigna at which point your coverage will increase to the requested amount or will remain at the guarantee issue amount (if denied). You may receive a request for additional information, if you fail to provide the additional information you will only be covered for the guarantee issue amount.
Reductions in Insurance: By the time you or your spouse reach age 70, chances are that your children will be grown and your mortgage paid.
At age 70, providing you are still employed, your coverage will decrease to 65% of the benefit amount. It will decrease to 50% at age 75.

Salary Employee: If you are currently enrolled, you may elect 1 additional times your salary (not to exceed 3 times your salary) in optional life coverage without medical questions during the 2016 open enrollment period.

Short Term Disability Insurance (STD) - CIGNA Your Short Term Disability plan is administered through Cigna. The plan requires that you be out of work, and certified disabled by your doctor, due to illness or injury for a minimum of 7 days before your weekly benefit is paid. Your weekly benefit will continue for 12 weeks provided that you remain disabled. Benefits are based upon the following class schedule:
