I was never really concerned about my health until I became a mom to my 6-year-old daughter. Since I take care of her, I cannot afford to get sick. In order to be healthy, my husband and I take fish oil vitamins, try to cook (and eat) fruits and vegetables, and exercise.

My daughter recently started a fantastic program called “Girls On the Run.” It is a program to help encourage preteen girls to think positively and to boost their confidence levels, interspersed with running training that culminates with a 5K race. Due to her age, she must have someone who is at least 16 to run with her (and keep up with her!). I live healthy to be a good example to her and her brothers. As I age, the need to stay active and live healthy becomes more important and apparent, as a parent. As the kids get older, they are finally getting big and strong enough to do all the things that I loved to do, and I want to be healthy and strong enough to do those things with them.

Healthy living was ingrained in me from a very young age. I was a competitive swimmer since 12 years old through college and always felt very physically fit. Associate Premiums Aren’t Going Up!For the majority of associates, premium contributions for the Live Healthy My Choice Plan medical options will not increase for the coming Plan year. In other words, if you elect exactly the same My Choice Plan option and the same coverage level you had last year, your cost for coverage will not go up for the 2016 – 2017 Plan year.

Diabetic Rx deductible waived If you buy brand name diabetes medications and supplies, your deductible will be

waived (My Choice Plans only). See page 9 for details.

New incentives You will receive an incentive if you participate in the new Preventive Incentive (My Choice Plans only). See page 14 for details.

More dental options If you are a part-time associate, you will now have at least one additional dental option.

Paying for coverage After-tax contributions (except Puerto Rico) will no longer be available for medical

and dental coverage.

Enhanced Benefits You can elect new Enhanced Benefits coverage at group rates through two new benefit

options: Critical Illness Insurance and Accident Insurance. See page 19 for details.

Out-of-pocket maximum (OOPM) Due to the Affordable Care Act (ACA), starting this Plan year, you will have an individual

OOPM — in addition to your family OOPM. See page 9 for details.

Enroll on My IN-SITE : May 18 - June 1 Call HR Services for help: 1-800-234-MACY (6229) Extended hours will be available from 8 a.m. – 10 p.m. ET, Monday through Friday

beginning May 18. Representatives will also be available Saturdays and Sundays

from 11 a.m. – 4 p.m. ET.

Prepare Before Annual Enrollment: Now! o Read this Guide for an overview of your options.

o Get cost details, benefit charts and benefits highlights on the My Benefits section of My IN-SITE.

o Compare options with the My Choice Plans Cost Comparison Tool on www.cigna.com/macysinc.

o Decide who to cover. See page 6 for eligibility requirements.

o Determine your HSA and/or FSA contributions.

o Gather your dependents’ Social Security Numbers. You need them to enroll.

o Choose carefully because changes are only allowed during the Plan year if you have a qualifying life event and you report the event to HR Services within 31 days of the event.

o Visit the Save Actively page (page 20) to review your 401(k) Plan contributions.

Enroll During Annual Enrollment: May 18 – June 1 o Select the Enroll Now link under Enrollment on the My Benefits page of My IN-SITE to review your

options and enroll.

o Verify your address and opt in to receive emails.

o Make your elections and designate beneficiaries.

o Click the Submit and Confirm button to complete enrollment.

o Print your Confirmation Statement and/or request an email confirmation for your records.

o Review Enhanced Benefits options and make your election(s) on the confirmation page.

Use After Annual Enrollment o Check your pay statement regularly to verify that deductions are correct.

o Register on your carriers’ websites to access plan and account information, tools and resources.

o Carry your identification (ID) cards, HSA and/or FSA debit cards and download the carrier apps.

Know Who’s EligibleYouBased on your status, you have different benefit options.

• In general, you must work at least 30 hours a week over a specified time period to be eligible for medical coverage.

• If you’re a regular, full-time or part-time associate with standard hours of 20 or more, you have other benefits to choose from. Look for the symbol to know which options are available to you.

• If you are an executive associate, you will be eligible for benefits as long as you remain in an executive position.

Your Out-of-Pocket Maximum (OOPM)The individual OOPM is $4,500 (My Choice Plan 1) or $6,450 (My Choice Plan 2). This

individual OOPM is the most you will pay out-of-pocket for one family member’s in-network,

eligible health care expenses during the Plan year. If more than one person in the family has

health care expenses, then the combined total of those expenses count toward reaching the

family OOPM: $9,000 (My Choice Plan 1) or $12,900 (My Choice Plan 2).

PrescriptionsExpress Scripts, Inc. (ESI) is the prescription drug administrator for all Cigna and Hawaii

HMSA medical options. The prescription drug benefits for other options are administered by

the medical carrier.

For the My Choice Plan medical options, both your medical and prescription drug costs apply

toward one single annual deductible. The annual deductible applies when you purchase

most prescriptions, but for brand name diabetes medications and supplies (including injectables), the deductible is now waived, and you just pay the 20% coinsurance. Plus,

certain generic preventive drugs used to manage high blood pressure, cholesterol and diabetes are available at no-cost to you — even if you haven’t met your deductible.

Are You or a Dependent Eligible for or Going to Become Eligible for Medicare in 2016?If so, before you choose a My Choice Plan medical option for the 2016 – 2017 Plan year,

there’s something you need to consider.

If you choose a medical option that doesn’t have what is considered “creditable” prescription

coverage (does not meet Medicare’s minimum requirements), and then you later enroll in

Medicare prescription drug coverage, you will pay more for that coverage as long as you have it.

My Choice Plan 2 and the Kaiser Medical Only Plan are not considered “creditable” coverage.

My Choice Plan 1 provides creditable prescription drug coverage; so, if you enroll in this option

and later enroll in Medicare prescription drug coverage (you must join within 63 days of Macy’s,

Inc. coverage ending), you will not pay a penalty in the form of a higher premium. To find out

more about Medicare and creditable prescription drug coverage, go to www.medicare.gov

or call 1-800-MEDICARE (633-4227).

MedicalIn most locations, you have access to the My Choice Plan medical options — My Choice Plan 1

and My Choice Plan 2 — administered by Cigna. These are high-deductible health plans that are

paired with a Health Savings Account (HSA). Check out the chart below for more details on these

options. In addition to these options, the Company provides additional health plan offerings in specific geographic locations, such as Kaiser in California, or where the Company is unable to

offer the My Choice Plans — like in Hawaii, Guam and Puerto Rico.

The Health Reimbursement Account (HRA)You cannot contribute to an HSA if you are enrolled in Medicare (including Part A or Part B) or Medicare prescription drug coverage or claimed as a dependent on someone else’s tax return. If you are ineligible to open an HSA, you will receive a Cigna Choice Fund HRA. An HRA does not allow you to contribute your own funds, and you can’t take the money with you if you leave the Company. But, your Company contribution and any incentives you receive will be credited to your HRA, and the funds roll over year to year. You can also pair the HRA with a Flexible Spending Account (FSA).

The My Choice Plan medical options are paired with an HSA.

1. The Company funds your HSA with a contribution on July 1 to help pay for your qualified out-of-pocket expenses. The contribution varies based on your coverage level.

2. You can earn an HSA incentive by completing certain Live Healthy wellness programs.

See page 14 for details.

3. You can also contribute to the HSA tax-free (in most states). The amount you can

contribute varies based on your age, coverage level and the amount the Company may

contribute. Remember, you must elect your HSA contributions each and every Plan year!

Increase, decrease or stop your HSA contributions anytime by calling HR Services. There

will be a slight delay before the change takes effect.

4. Your HSA can help you pay for care you need. For example, if you go to the doctor, you

can use the money in your HSA to pay that bill. Typically, you won’t pay at the time of service,

but later, after Cigna has processed the claim and you’ve received your EOB and bill.

5. The HSA can help you save for future expenses. It’s your account, and the unused

money rolls over each year. All the money in the account is always yours, even if you retire

or leave the Company.

The Health Savings Account (HSA)

After you enroll, HSA Bank may require additional steps to establish an HSA. Be sure to respond promptly to any communication from Cigna and/or HSA Bank in order to ensure your account is established. You may need to update information or change an address since the USA PATRIOT Act regulations prohibit the use of a P.O. Box address to open an HSA.

DentalThree dental coverage options may be available to you through MetLife Dental and Aetna DMO. Hawaii, Guam and Puerto Rico associates have different options available to them.

MetLife High and MetLife BasicThere are two dental options available throughout the country: MetLife High and

MetLife Basic. Both options use the same provider network, and the only difference

between the High option and Basic option is the level of benefits. The MetLife Dental

Preferred Dentist Program (PDP) network has more than 287,000 participating dentists,

including more than 73,000 specialists. Keep in mind that you increase your benefits,

lower your out-of-pocket costs and receive the greatest cost savings by going to a dentist

who participates in the PDP.

Aetna DMOThe Aetna DMO (Dental Maintenance Organization) option is only available in certain

areas of the country. You may be eligible for this option based on the ZIP code of your

residence or work location. If you are eligible to enroll, this option will be displayed during

online enrollment.

The Aetna DMO is a managed care dental program that requires services be provided by

participating DMO dentists and specialists. When you enroll, you must designate a Primary

Care Dentist (PCD) for you and your covered dependents to access benefits, as services

are available only when provided or prescribed by a DMO participating dentist or specialist.

Quick TipThe money the Company contributes to your HSA can help you pay for your pharmacy and medical expenses until your in-network deductible and out-of-pocket maximum are met for the Plan year.

Quick TipTypically, you won’t pay your provider at the time of service. You will pay after Cigna has processed the claim and you’ve received your Explanation of Benefits (EOB) and bill from your provider. It’s important to compare any bills with your EOB.

Quick TipTo help you cover the costs of an unexpected critical illness or care you need due to an accident, you can now elect new Enhanced Benefits at a discounted group rate and pay for it through payroll deductions. Learn more on page 19.

Flexible Spending AccountsIf you want additional tax savings on eligible health care, dental, vision or dependent care expenses, you may want to sign up for a Flexible Spending Account (FSA). You may have up to

three options:

1. A Health Care FSA (HCFSA) lets you contribute up to $2,550 tax-free — but it is only available if you are not enrolled in an HSA.

2. A Limited Health Care FSA (LFSA) lets you contribute up to $2,550 for

dental and vision expenses.

3. A Dependent Care FSA (DCFSA) lets you put away up to $5,000 (or $1,000, if highly

compensated) to use during the Plan year on eligible dependent care costs.

Stretch your dollar

A Few Reminders• You must re-enroll each Plan year. When you do, you will receive a debit card for your

HCFSA/LFSA, or you can submit claims to Cigna for reimbursement. For DCFSA, you pay the cost up-front, and then submit claims for reimbursement.

• You don’t have to be enrolled in a Company medical or dental plan to enroll in an FSA.

• Up to $500 will be automatically carried to the next Plan year (except for the DCFSA program). You forfeit any money over $500 you don’t use by the end of the Plan year (June 30).

• Keep your receipts to verify your purchases. You have until Sept. 30, 2017, to request reimbursement for expenses from the Plan year ending June 30.

To help you be prepared when things don’t go according to plan, you can enroll in

disability and other insurance coverage at discounted group rates and enjoy the

convenience of payroll deductions.

Short-Term Disability (STD) (for hourly associates)STD insurance replaces a portion of your income for medically certified disabilities and

replaces a portion of your pay for up to 26 weeks. STD insurance is administered by Prudential.

For more information, including coverage calculators, visit www.prudential.com/macys. If you

are enrolling after your initial eligibility, a Statement of Health will be required.

Long-Term Disability (LTD) (for all associates)LTD insurance replaces a portion of your income during an extended period of disability, such

as for a serious injury or illness, after your STD coverage ends. Depending on your coverage

option, you may choose 40% or 60% income replacement.

How STD and LTD Work TogetherJessica was pregnant and her expected delivery date was Oct. 1; however, on April 1, she

developed complications and was put on bed rest for the duration of her pregnancy. Jessica was

enrolled in STD, which paid benefits until Sept. 30 (180 days). As of Sept. 30, Jessica had not yet

Optional Life InsuranceIn the event of your passing, optional life insurance pays money directly to your beneficiaries

to help cover their daily expenses and maintain their standard of living. You can elect ½x to 8x

your annual salary, up to $2.5 million. Optional life insurance is administered by MetLife.

You can also cover your immediate family members with benefits, payable to you, in the event of

their passing:

• Spouse/Domestic partner — $10,000 policy

• Child(ren) up to age 26 — $5,000 policy (covers all eligible children)

If you’re increasing your optional life insurance coverage, enrolling after your initial

eligibility or electing 4x your annual salary or more than $500,000 in coverage, you

may be required to provide a Statement of Health.

Will Preparation and Estate PlanningIf you are enrolled in optional life insurance, you have access to no-cost to you

will preparation and estate planning services through Hyatt Legal Plans, a MetLife

Accidental Death & Dismemberment (AD&D)AD&D coverage provides additional insurance protection in the form of a lump-sum payment

for certain accidental losses including: accidental loss of life, limbs, speech, hearing or sight,

paralysis and more. Cover yourself or your entire family for ½x to 10x your annual salary, up to

$1.5 million. AD&D coverage is administered by Zurich American Insurance Company.

New! Enhanced Benefits InsuranceTo help you cover the costs of a critical illness or care you need due to an accident, you can

elect Enhanced Benefits options at a discounted group rate and pay for it through payroll

deductions. Here’s how these options work:

Critical Illness Insurance This option pays you a lump-sum cash benefit if you have a critical illness such as a heart

attack, stroke or cancer. You can elect this coverage for just yourself, or you and your spouse.

Children are covered at 50% of your benefit amount for no additional charge.

Accident Insurance You can choose from a High and a Low Option. Both choices pay cash to you for such things as

emergency room visits, broken bones, X-rays and burns.

Auto/Home InsuranceObtain multiple competitive auto/home insurance quotes all at once from companies including:

Kemper Select, Liberty Mutual, MetLife, Progressive, Safeco and Travelers. Auto premiums are

guaranteed for six months; home premium rates are guaranteed for one policy year.

the Company — a 3.5% match — which means you will tuck away a total of 9.5%. See the

If you make pre-tax contributions, you get the added bonus of lowering your taxable income!

You can save more than 6%. In fact, you can save up to 50% of your pay — or up to the

$18,000 IRS limit, whichever is less.

To help you easily maximize your match, your contribution will be automatically increased by

Health Care Plan members who qualify for and complete the Nurse Advisor Disease Management Program are limited to one incentive per Plan year. For Plan year 2016 - 2017, the disease management incentive will be paid as a one-time contribution to your Cigna Choice Fund HSA or HRA. Collectively bargained associates or other special groups electing

a Cigna medical option other than My Choice Plan 1 or My Choice Plan 2 will receive any disease management and Know Your Numbers program incentives they earn in the form of a payroll premium reduction. These groups will not be eligible for the Company HSA/HRA contribution, $50 Preventive Incentive or Omada Prevent Program. Certain collectively bargained associates or other special groups may not be eligible for the Live Healthy program incentives.

Live Healthy program participation and results are confidential and protected by federal and state privacy laws. To receive any Company HSA/HRA contributions and/or Live Healthy program incentives, associates must be actively employed and enrolled in the appropriate Cigna medical option at time of funding; associates on leave of absence will not receive any payroll premium reduction until they return to work.
