What to Do – Focus on Barrier Management

▪ Barriers need to work when we need them to work▪ It is everyone’s responsibility

MY SCE IS WORKING AND I

KNOW IT

• I know the Barriers

• I know my Role

• I keep them healthy

• I can prove it

Challenges in Barrier Management

Clear line of sight to Major Accident HazardsClear roles & responsibilitiesRegular engagements and

collaboration

• Active use of e.g. Hazards & Effects Register, Bowties

• HSE Case mindset embedded into “business as usual” e.g. into Learning From Incident, Management of Change, Permit to Work

• Understand how HSE Case benefits the business

• Convince and align stakeholders

• Consistent approach and messages

• What else can go wrong? What’s new? What has changed?

• Monitor barrier effectiveness through Process Safety Performance Indicators

• Assurance on Process Safety Performance

Barrier Effectiveness: It is everyone’s responsibility

Barrier Owner

• SCE meet the design /

functional intent

• Monitor SCE compliance

• Analyze SCE performance

Executor

• Perform the assurance

task

• Report outcome

• Highlight deficiencies

Technical Authority

• Set the requirement of

SCE and acceptance

criteria as per PTS /

code and standard

What are SCEs?

Simply put: SCEs are barriers put in place to prevent and/or limit the effect of a hazard with Major Risks

SCEs are Barriers

SCEs are to create FOCUS

- the failure of which could cause or contribute substantially to Major Accident Hazard (MAH)

- a purpose of which is to prevent, or limit the effect of a major accident”

Safety Critical Elements (SCEs) are Parts of an installation and such of its structure, plant equipment and systems (including computer program) or any part thereof:

Safety Critical Elements (SCE) Identification with Bowtie and Flowchart 1

Are SCEs (Barriers) by Themselves Good Enough?

Why?

Because SCEs need to perform as required when

needed – they must be maintained to prevent the holes

from aligning.

SCE without an associated Assurance/Maintenance Activity is not a Barrier.

Good Design/Operation Integrity

• Less holes

• Minimizes time of temp holes exist

• Limits holes size

Poor Design/Operation Integrity

• Increases holes

• Increases the time of temp holes exist

• Increases holes size

The ‘holes’ in the cheese represent the weaknesses in the barriers, leaving them ineffective in the role as a risk control system.

Barrier Effectiveness

Design It Right, Construct It Right, Operate It Right, Maintain It Right

Site Specific Performance Standard

SCE Group and goal

What need to be done to ensure the

SCE is meeting the functional

requirement (technical and quality

audits)

What results are we recording to

indicate that SCE is functioning

Execute/Review and Implement Assurance

All deviation must have Risk Assessment , RCFA and etc and endorse by respective TA

3

• Centralized web based system accessible globally• Real-time information on status of SCE for each site • Integrated deviation management process

FSM retrieves SCE Maintenance PMMS Transaction Data; Deviation of LAFD need to be done using FSM 4

Verification Process Flow

• Scheduling• Team

formation• Site liaison

• Inspection• Test

• Condition assessment

• Risk Ranking• Standards
