HUMAN SERVICE PROGRAMS FISCAL YEAR 2016

GENERAL INFORMATION

Montgomery County Department of Health and Human Services is pleased to announce the availability of grants of up to $10,000, to support health and human service projects that promote a safe, healthy and self- sufficient community. The Community Services Grants program provides one-time only grants to non-profit organizations serving Montgomery County residents. The program provides reimbursement to funded organizations for approved equipment and other capital purchases.

I. PURPOSE AND DEFINITION

Funded purchases will include capital improvements and/or equipment. Such purchases should directly contribute to the following service priorities:

1) Safety net services - Food and Shelter

2) Affordable Care Act (ACA) - Health Literacy and Insurance Literacy

Vehicles, salaries, conferences, training or operating costs, labor and/or delivery costs or funds to apply to deficits and/or losses in other funding sources are not eligible expenses under this grants program. Grant awards are distributed via reimbursement, upon receipt of proof of payment and other required documentation verifying the purchase of approved items.

All funded programs must initiate and complete purchases between July 1, 2015 and December 31, 2015.

II. ELIGIBILITY

All not-for-profit agencies, organizations, institutions, or associations providing services in Montgomery County and incorporated under 501(c) (3) of the Internal Revenue Code, and offering health and human services activities consistent with the outcomes listed in Section I (Purpose and Definition) are eligible to apply.

In order to optimize the availability and use of funding to support this priority, organizations which have received awards under this program during fiscal years 2013, 2014 or 2015 are NOT eligible to apply for funding during the Fiscal Year (FY) 2016 award period.

III. DEADLINE AND CALENDAR A. Deadline for application is 3:00 p.m. on Wednesday, December 10, 2014. Incomplete

applications will not be considered, and will be returned to the applicant. Applications received after 3:00 p.m. on Wednesday, December 10, 2014 will not be accepted. To ensure fairness to all applicants, there will be no exceptions to the deadline.

B. All Applications must be hand-delivered, and must be received by 3 p.m. Wednesday, December 10, 2014: Deliver all applications, including requested attachments, to the following location:

Applications will be accepted on the 5th Floor of 401 Hungerford Drive only, and not at any other departmental office building or County location.

C. Grant applications will be reviewed by the Department of Health and Human Services (DHHS) and funding to selected organizations will be available for use beginning July 1, 2015.

Funded projects and expenditures of monies must occur between July 1, 2015 and December 31, 2015.

IV. APPLICATION AND FUNDING A. Applications must be typed and submitted on the appropriate forms with the required

attachments. Failure to adhere to Grants Program Guidelines or provide the required attachments will result in rejection of the application.

B. Applicants are encouraged to identify in-kind services and or matching funds that are available

and will be used to augment the proposed project.

C. An organization may submit only one application per grant period and per organization. Only one group or organization may apply per year, including different branches, divisions, locations and/or units of same organization.

D. Applicants must provide six (6) collated copies of the following information as attachments to their

applications:

1. Proof of applicant’s not-for-profit and incorporation status (IRS not-for-profit designation);

2. Financial statement for applicant’s last complete fiscal year;

3. Complete project budget, including all needed equipment, quantities, specifications, manufacturer’s details, drawings, photographs or other renderings.

6. Grant Application Checklist, which should be attached as the cover page of each copy.

All attachments should be numbered, stapled and included in each copy of the grant application.

E. Narrative should clearly list all proposed items to be purchased, explain nature and purpose of items, and provide brief explanation of how the proposed purchase will contribute to the outcomes delineated in Section I, Purpose and Definition.

V. OTHER CONSIDERATIONS A. The requested award should not duplicate or supplant funding for any existing activities or efforts.

VI. SUPPORT RESTRICTIONS

A. Grants will be awarded for projects implemented in Montgomery County only. Organizations must operate in, and provide direct service to residents of Montgomery County. Organizations may have headquarters and/or administrative offices outside of Montgomery County, so long as the organization demonstrates that client populations to be served by the award reside in Montgomery County.

B. The program will NOT fund:

1. Projects that have an existing deficit from a previous year or a previous project. 2. Organizations that received previous awards, and did not comply with the terms of the

award, including submission of project reports and/or participation in a monitoring visit. 3. Projects that will require more than a one-time grant award. 4. Projects of an ongoing nature. 5. Projects that are implemented before the start or after the close of the granting period. 6. Vehicles, personnel, operating or salary expenses of the organization, including

training and conferences. 7. Labor, delivery costs and related fees associated with the proposed purchase. 8. Replacement of lost and/or reduced Federal, State, United Way or other funding. 9. Organizations that have an outstanding contractual obligation with the Department

and/or the County.

VII. EVALUATION CRITERIA Applications will be reviewed against the following criteria: Applications will be judged on how well the grant funds will contribute to the outcomes identified in

Section 1, Purpose and Definition.

Requested funds must be used for a capital expenditure (renovations, equipment, or technology improvements) or purchase of goods.

Applicants must demonstrate the proposed project will impact the priority areas, and the organization’s experience, capability and strategy for implementing the proposed project.

Applicants must demonstrate the effective use of volunteers.

Applicants must provide a program overview which indicates how the agency’s services fit into the

overall health and human services delivery system of Montgomery County and maintain a funding base which does not rely solely on County grant funds.

Applicants must demonstrate sound financial management and effective resolution of any problem identified in previous financial audits.

VIII. REVIEW PROCESS Applications are subject to the following levels of review:

A. Acceptance – All applications will be subject to an initial review, including:

1. Completeness of application. 2. Legibility and clarity. 3. Compliance with applicable guidelines, including the one-time only nature of the project. 4. Fiscal accuracy.

Incomplete applications will be returned to the applicant without consideration.

B. Grants Review Panel – The Grants Review Panel includes public and private representatives.

The Panel will be chaired by the Director of the Department of Health and Human Services or a designee. The Review Panel will review applications based on the Evaluation Criteria listed in Section VII.

. C. Recommendations to the County Executive – The Review Panel will present recommendations

for awards to the Montgomery County Executive, who determines final approval and inclusion in the FY16 Recommended Operating Budget, based on the availability of funds. The final determination of awards is subject to County Council approval during its annual review of the FY16 budget.

IX. CONDITIONS OF AWARD

A. Awardees will be required to: 1. Submit an organizational invoice, requesting reimbursement of funds, along with

documentation confirming payment and receipt of the approved items.

2. Assure the County that the organization is compliant with Title VII of the Civil Rights Act of 1964, indicating that no person will be excluded from participation or be denied the benefits of any program, activity or service on the basis of race, sex, sexual preference, color, religion, ancestry, age, national origin or handicap.

Applicants must submit six (6) complete, collated copies of the application. Applicants are encouraged to retain one additional complete copy for their files and reference.

C.

Grant applications will be reviewed and grants announced by July 1, 2015.

D.

Grant funds will be disseminated consistent with the terms listed previously. No funds will be available prior to July 1, 2015. If your agency is funded, you are not permitted to be reimbursed

for purchases made prior to July 1, 2015, even if those items are consistent with requests made in your organization ’s g ran t a p p lica tion .

3. Acknowledge Montgomery County in all publicity and in all promotional or informational materials used in connection with the funded project, i.e., programs, handbills, posters, radio and TV spots, etc.

4. Submit to the County within 30 days of the completion of the project, a brief (not more than 3

pages) summary of how the grant award was used and how the award contributed to the stated outcomes.

5. Assure item(s) will be used solely for purpose outlined in application for a period up to two

years following the award of funds.

6. If the awardee fails to comply, the Department and/or the County may seek return of all items purchased under this award.

This PSA series will be aimed at helping the ACA’s newly insured to better comprehend and to navigate the primary health care system in Montgomery County and to access health care services that are needed. The campaign will launch with the ACA Open Enrollment season beginning in November, 2015, and conclude on June 30, 2016, with content that focuses on post-enrollment content and calls-to-action.

b. Affordable Care Act (ACA) - Health Literacy and Insurance Literacy

On Thursday, October 30, 2008, Daniel Valentine published in “The Gazette Newspaper” an article entitled, “County has highest percentage of uninsured residents in Maryland: Incomes, health care costs cited as reasons for high rate.” His words resounded with alarming and startling revelations that “Montgomery County's uninsured population was the fifth-highest [among Maryland’s 23 counties] at 17 percent.” Moreover, “local health officials said the high number of people living without a plan to help cover routine doctor visits and prescriptions contributes to high rates of chronic problems like obesity, heart disease and diabetes and strains the county's public hospital system…” The Affordable Care Act (ACA) was signed into law by the President on March 23, 2010, to directly ameliorate this growing crisis of the number of uninsured and underinsured Americans. Although the ACA substantially improved the number of Montgomery County residents with access to health care, it does not resolve the challenge of our individual residents understanding and awareness of the medical or insurance terminology needed to fully engage the allied health community’s resources, the complex labyrinth that is too often encountered in selecting a primary care provider, the actual services and pharmaceuticals covered by the insured’s qualified health plan (QHP), the reimbursement terms, open enrollment, and much more. The ACA improves health care access, but it does not improve our residents’ understanding of their options and their need for self-direction in accessing their heath care resources. Pages 16 – 17 of the “Survey of Health Insurance Marketplace Assister Programs: A First Look at Consumer Assistance under the Affordable Care Act” revealed that 90% of the USA’s newly insured reported post-enrollment problems with their insurance. This survey discovered that 44% of newly insured reported not knowing how to use their insurance (http://kaiserfamilyfoundation.files.wordpress.com/2014/07/8611-survey-of-marketplace-assisters2.pdf). Simply stated, having personal ownership and possession of an insurance card is NOT accessing health care! Montgomery Community Television dba Montgomery Community Media (MCM) has demonstrated prior successful experience in partnering with the Montgomery County government and Montgomery County’s health care community to address the County’s call-to-action health awareness and education goals. During 2012 and 2013 MCM - with its collaborative healthcare partners - conceptualized and successfully launched on both broadcast and broadband channels “Healthy Montgomery Family Check-Up.” This

Toward addressing the need to improve our residents’ understanding of their health care options and their need for self-direction in accessing their heath care resources, Montgomery Community Media - in partnership with the Montgomery County Department of Health and Human Services and the Primary Care Coalition of Montgomery County, Maryland, Inc. – will further conceptualize, develop and launch “the Card to Better Living” campaign series of public service announcements (PSAs). This PSA series will be aimed at helping the ACA’s newly insured to better comprehend and to navigate the primary health care system in Montgomery County and to access health care services that are needed. While this PSA series is specifically designed to target the ACA’s newly insured and those with chronic or complex health conditions, the content is also concurrently informing and better educating Montgomery County’s general population at-large. It is most important to note that this PSA series can be

used to support the Montgomery County Health and Human Services Departmental efforts in its administration of the Prince George’s County ACA programs. Through the successful efforts of the Montgomery County Department of Health and Human Services and its partners, 75,000 low-income residents in Montgomery and Prince George’s Counties gained Medicaid coverage under the Affordable Care Act (ACA); and, an additional 32,500 residents of the two counties gained insurance through a qualified health plan (QHP). Although there is new health care coverage for these residents, these newly insured neighbors quickly discover that accessing their coverage remains a significant challenge. The too often unaddressed realities of the barriers of substantial health illiteracy and high health insurance illiteracy – meaning the inability to comprehend and to take self-direction upon personal health and health insurance information – remain without regard to the improved access to health care for the newly insured. The ACA provides only the initial step in the health care access ladder. The newly insured must become health literate and health insurance literate to fully access their health care. Towards that aforementioned goal Montgomery Community Media will develop and launch “the Card to Better Living” campaign series of public service announcements (PSAs). This PSA series will improve the newly insured’s understanding of their newly acquired health insurance and will empower their self-direction in engaging the allied health community and accessing their health care for the services that they personally need. Montgomery Community Media will advance its ongoing partnerships with the Primary Care Coalition of Montgomery County, Maryland, Inc., the Montgomery County Department of Health and Human Services and the Montgomery County’s health care community towards these ends. Our partnership with the Primary Care Coalition of Montgomery County (PCC) is strategic. PCC’s mission is to promote health care access and equity. They will use their resources and expertise to guide us in the development of the messaging that addresses the topics of health care plan benefits, payment structures, how to use health insurance and accessing primary care to maintain good health. During the spring, 2014, the Primary Care Coalition of Montgomery County conducted focus groups to study the ACA newly insured’s experiences enrolling in their selected qualified health plan. Most focus group participants struggled to comprehend the differences between the health care plans, were unfamiliar with health care and medical insurance terminology, and were ignorant of how to actually engage and to use their health care insurance. PCC’s focus group results are consistent with the ACA’s post-enrollment challenges cited within a recent “Washington Post” article. That article entitled, “New Challenges for Obamacare: Enrollees who don’t understand their insurance plans,” was published on July 16, 2014, and reported that participating nonprofit organizations in at least 40 states are responding to the newly insured whom do not know what to do after receiving their health insurance cards (http://www.washingtonpost.com/national/health-science/new-challenge-for-

ACA-eligible residents are seeking-out any source that can help them enroll in an ACA qualified health care plan and provide them with knowledge on accessing their health care. Unfortunately, this effort to access health care knowledge is only confounded by health care scams that appear similar to valid ACA health care exchanges. On Thursday, November 21st, 2013, Elaine Rigoli posted on PrivateWiFi.com the article entitled, “BBB Offers Tips to Help Consumers Avoid Phony Affordable Care Act Websites” (http://www.privatewifi.com/bbb-offers-tips-to-help-consumers-avoid-phony-affordable-care-act-websites/). ACA participating organizations continue to scramble to develop content to educate and inform the newly insured with the relevant information they need. Our collaborative partnership to launch “the Card to Better Living” campaign series of public service announcements (PSAs) will help to fill that ongoing community need. “The Card to Better Living” campaign series of public service announcements (PSAs) will follow the pattern of Montgomery Community Media’s prior success with the “Healthy Montgomery Family Check-Up” health care campaign. It is a campaign series that delivers clear, concise, culturally and linguistically relevant health access information to the newly insured. There is a high demonstrated need for content that provides this information. Montgomery Community Media will harness its leadership within the Montgomery County PEG Governance Board of eight (8) cable broadcast channels (http://www.mymcmedia.org/peg-channels/) serving over 873,000 viewers in over 351,000 cable households to broadly distribute the “The Card to Better Living” PSAs throughout Montgomery County with significant frequency and county-wide reach. MCM will also distribute the PSA campaign to Gaithersburg Television which serves the residents of Gaithersburg, Maryland. These PSAs will also be made available to Prince George's Community TV (PGC TV), Prince George's County Public Schools TV (PGCPS-TV96)and Prince George's Community College Video Enhanced Online (VEO) for their 802,000 viewers. Our ongoing partnership with Telemundo and Univision also provide opportunities for enhanced broadcast television synergies. Montgomery Community Media will also distribute these PSAs on its broadband channels: mymcmedia.org which annually serves over 500,000 unique web users, YouTube.com/mymcmedia which has exceeded 300,000 video views and Facebook.com/mymcmedia which attains 100,00 views per week! primary care provider, and provide tips on successfully using access to health care to achieving and maintaining a healthy life. Our goal is to produce content in at least two languages: English, Spanish, Amharic, and Korean are targeted languages. 2. What population will be served by the proposed project? This project will target Montgomery County’s low and moderate income whom recently acquired health insurance coverage through the Medicaid program or from a Qualified Health Plan (QHP). Prior to the implementation of the Affordable Care Act and through the subsequent ACA enrollment processes, the uninsured population in Montgomery County remains heavily minority: 23% African American, 46% Hispanic, and 14% Asian. The demographics of the newly insured through the Affordable Care Act will naturally reflect a similar distribution of minority individuals. This latter mentioned demographic is the Montgomery County community target for this project. The Affordable Care Act catalyzes access to health care through establishing income-based premium assistance. Current Medicaid eligibility regulations require that the insured individual’s household income is below 138% of the federal poverty level (FPL). This standard contrasts with and compliments those of the ACA. The eligibility threshold for individual coverage by an ACA Qualified Health Plan is such that insured individuals may have higher incomes than Montgomery County residents whom are Medicaid eligible. Because of this income variability, the ACA provides an eligibility for premium assistance for those individuals whose income is between 139% and 400% of the federal poverty level. The ACA has truly shifted the health care access paradigm. The ACA continues to make historic progress by providing previously unaffordable health services to those Montgomery County residents with pre-existing health conditions. Montgomery Community Television dba Montgomery Community Media will further conceptualize and fully develop - in partnership with the Montgomery County Department of Health and Human Services and the Primary Care Coalition of Montgomery County, Maryland, Inc. – “the Card to Better Living” campaign series of public service announcements (PSAs). 3. Describe your organization’s mission and goals related to the priority area. Include details regarding the programs, services and populations served by your organization, and describe how these services fit into the health and human services delivery system in Montgomery County. Since our 1984 founding as the county’s community media center and region's largest public access station, our aim remains to "inform, connect and educate" the nearly one million residents of Montgomery County. Our mission is to encourage and sponsor “first-voice “multimedia content that meets community needs, engage our communities, and offer forums for free expression on both broadcast and broadband platforms. Our vision is to educate and empower county residents by providing a means to creatively engage and more effectively connect them through our multimedia platforms. We serve the community through content distribution, educational training, economic development, nonprofit partnerships and collaborations, and production services. Towards the fulfillment of those goals by innovative and informative means, we foster and sustain programming and offerings that both reflect and promote the culturally and socioeconomically inclusive communities that comprise the mosaic that is us. Our multimedia content is community-centric and overwhelmingly volunteer produced. We broadcast to nearly 873,000 viewers in over 351,000 cable TV households. Our broadband platform, mymcmedia.org, annually serves over 500,000 unique web visitors. Our nearly 700 active volunteers annually complete 600+ projects, engage the humanities through weekly blogs and daily tweets, and provide over 14,000 hours of on-air content. Our volunteers and presenters contribute 120 program series each quarter, including: "Backpack Beat," "Teen Point of View,” "Musica Y Algo Mas," "Selam Ethiopia," "Traveltelevision, and the "Punjabi Community Hour." Our volunteer bloggers include interests in health and wellnessm psychology, the culinary arts interior design, public policy, creative writing, etc. Our community-based multimedia participators engage us through Facebook.com/myMCMedia, @myMCMedia on Twitter and Linkedin.com/myMCMedia, YouTube.com/myMCMedia, YouReport.com, Pintrest.com, Instagram.com, Eventbrite.com, eVite.com and Constant Contact. On Facebook we have nearly 8,000 Friends and it attains an average of 100,00 views per week. On YouTube we have exceeded 300,000 video views. We publish electronic newsletters to nearly 34,000 readers through Constant Contact. LinkedIn.org named us as one of the top 5% most viewed LinkedIn profiles out of over 200 million! We have demonstrated prior successful experience in partnering with the Montgomery County government and Montgomery County’s health care community to address the County’s call-to-action health awareness and education goals. During 2012 and 2013 MCM conceptualized and successfully launched on both broadcast and broadband channels “Healthy Montgomery Family Check-Up.” This engaging educational series of short-videos that address Montgomery County’s highest ranked health care challenges teaches Montgomery County residents vital tips to incrementally improving their

MCM also regularly disseminates community content sent to us by the African American Health Program funded by the Montgomery County Department of Health and Human Services and we also broadcast PSA delivered to us by the US Health and Human Services agencies. Our partnerships with ACA participating nonprofits such as Interfaith Works, the Nonprofit Roundtable and the Primary Care Coalition of Montgomery County span several years and are ongoing in the common, mission-driven efforts to “engage, inform and educate” the residents of Montgomery County, especially on critical issues such as health, wellness and access for affordable healthcare. 4. Describe the project for which these funds will be used. Clearly describe the goals, strategies and timeline for implementation, and how the proposed project will address the priority area identified in number 1. Montgomery Community Media - in partnership with the Montgomery County Department of Health and Human Services and the Primary Care Coalition of Montgomery County, Maryland, Inc. – will use the $10,000 FY2016 Community Service Grant to further conceptualize, develop and launch “the Card to Better Living” campaign series of public service announcements (PSAs). This PSA series will be aimed at helping the ACA’s newly insured to better comprehend and to navigate the primary health care system in Montgomery County and to access health care services that are needed. While this PSA series is specifically designed to target the ACA’s newly insured and those with chronic or complex health conditions, the content is also concurrently informing and better educating Montgomery County’s general population at-large. This PSA campaign series will improve the ACA’s newly insured comprehension and navigation of the primary health care system in Montgomery County – and, by default, Prince George’s county - and their self-direction in accessing the health care services that they need. The initiative will use both qualitative and quantitative metrics to assess campaign impressions (how effectively the PSAs reached the target audience) and impact (whether messages resulted in improving their understanding and/or changing their behavior). Within this collaborative partnership the PCC may gather data and prepare quarterly reports following the campaign’s roll-out. Metrics to assess impressions may include: number of views of broadband channel played PSAs/video vignettes, number of airings of broadcast PSAs, number of partner organizations using the content. The assessment of the impact and behavioral change may require retrospective pre-post survey data from the newly insured to measure their understanding of health insurance and their improved capacity to navigate the health insurance system. 6. Describe how the project will expand access to and/or availability of services to the targeted population?

Our strategic partner, the Primary Care Coalition of Montgomery County, prides itself on collaborating with a wide array of public agencies and community organizations. The PCC serves as the performance manager for the Maryland Health Benefit Exchange's Capital Region Connector Entity, which conducts the outreach and enrollment process for Montgomery and Prince George’s Counties. PCC’s experience as the performance manager allows is to continue to develop strong partnerships with community organizations that enroll consumers in qualified health coverage plans. PCC has already conducted focus groups to solicit input and insight from ACA newly insured individuals. Together - Montgomery Community Media, the Primary Care Coalition, the Montgomery County Department of Health and Human Services and our partners – will distribute this highly demanded health care content to the outreach and enrollment professionals working throughout Montgomery and Prince George’s Counties to re-enroll and newly enroll consumers and to “engage, inform and educate” our communities about health, wellness and the basics of accessing health care insurance. 7. Describe how your organization will use volunteers under the proposed project (if applicable). Montgomery Community Media may use ACA newly insured volunteers in the casting of the “The Card to Better Living” campaign series of public service announcements (PSAs). This strategy will give a relevant, community-based face to this effort. Montgomery Community Media has successfully used this “community faces” strategy in its award winning partnership that recognizing outstanding educators in both Montgomery and Prince George’s Counties in FY 2014 (http://www.mymcmedia.org/mcm-named-natao-award-winning-finalist-in-two-categories/ and http://www.mymcmedia.org/nominate-your-favorite-teacher-by-oct-6-video/). 8. Describe new partnerships and service innovations associated with the proposed project (if applicable). This project directly arises from the most recent two-consecutive fiscal years of dialogue and conceptualizing among the Montgomery County Department of Health and Human Services offices and the Primary Care Coalition of Montgomery County. It is an ongoing collaborative partnership between us three. Montgomery Community Media will innovate and invigorate this collaborative partnership by harnessing its leadership within the Montgomery County PEG Governance Board of eight (8) cable broadcast channels (http://www.mymcmedia.org/peg-channels/) serving over 873,000 viewers in over 351,000 cable households to broadly distribute the “The Card to Better Living” PSAs throughout Montgomery County. The FY 2016 $10,000 Community Service grant represents less than 1% of our annual budget. The “the Card to Better Living” campaign series of public service announcements (PSAs) cannot be created without the FY 2016 funding from the Community Service Grant. 10. How would you implement the project with a 50% reduction in the requested amount of funding? The number of PSAs and/or the ability to provide any linguistically relevant content beyond English would be proportionately reduced if full funding is not provided. 11. Has your organization requested funding for this project from other sources? If yes, please list other solicitations under consideration. Montgomery Community Media’s collaborative partner, the Primary Care Coalition of Montgomery County, has unsuccessfully sought funding for a substantially broader, more robust, comprehensive campaign that included posters, factsheets, and radio and television public service announcements and as well as four video vignettes explaining four different stages in using health insurance produced in Spanish, Amharic, Korean, and English. Subsequently, the Primary Care Coalition of Montgomery County and the Maryland Women’s Coalition for Health Care Reform discussed health and health insurance literacy issues with a committee of health care funders from the Washington Regional Association of Grantmakers. Those committee members included the Consumer Health Foundation, the Healthcare Initiative Foundation, and others. Currently, no funding solicitations from the Primary Care Coalition’s effort remain under consideration. Montgomery Community Television dba Montgomery Community Media only has this grant proposal under consideration, and we fully intend to use this $10,000 FY 2016 Community Service Grant as the seed-funding to attract additional funding resources – in collaboration with our partners - from grant providers and foundations during FY 2016.
