12,300 new BC+ Core Plan enrollees as of 1/1/2009 (Childless Adults < 200% FPL)

Access: Over 20 new FQHC net providers13% increase in Medicaid and uninsured patients served$1.6 million in new health system funding to create FQHC medical homes for uninsuredImplementation of IT supported PAP program in 10 community clinicsElimination of ED diversions for Behavioral Health patients and a significant reduction in ED wait times for IP admission

Care Coordination: Over 600 ED primary care referrals to FQHCsFunding and implementation of ED IT Linking providing health information exchange across 10 Milwaukee EDs
