Human Sexuality & AIDS Education

Human Sexuality & AIDS Education is required instruction. Par-ents or guardians (or students eighteen years of age or older) who do not want their student involved in all, or some, of the classes may obtain an opt-out request form from the principal. Opt-out requests shall be required annually and are valid only for the school year in which they are submitted.

Drug/Smoke Free Schools & Workplace

The use of tobacco in any form is not allowed on USD 368 property, in USD 368 owned/leased/rented vehicles, in contracted school buses or at USD 368 activities at any time, by anyone. (See JCDAA). A student shall not knowingly possess, sell, use, transmit, or be under the infl uence of any narcotic drug, hallucinogenic drug, amphetamine, barbiturate, marijuana, any controlled substance, or alcoholic beverage of any kind: - on school grounds before, during, or after school - on school grounds at any other time when the school is being used. - off school grounds during a school function Use of a drug authorized by a medical prescription from a registered physician shall not be considered a violation of this rule. Immunizations/Physicals Students in Kindergarten through 5th grade must have 5 DTaP (diphtheria, pertussis and tetanus). 4 Polio, 2 MMR (measles, mumps, and rubella), 3 Hepatitis B Vaccines and 2 Varicella Vac-cines. The fi rst MMR/Varicella must be on or after the child’s fi rst birthday. The fourth DTaP and third Polio must be after the fourth birthday. Any student who has not received a DT booster within the last 10 years will be required to do so before attending class. All kindergarten students must present a signed physical exam form at the time of enrollment. Physicals are also required for students involved in sports and for those students new to the district that are 8 years of age or younger. All USD 367 special education students who attend school out of the district will need to enroll at the high school on the above dates.

TRANSPORTATION

Bus transportation is provided for students living outside the city limits of Osawatomie and within the boundaries of USD 367. Students living within the city limits of Osawatomie may walk to the at-tendance center nearest their home or a designated shuttle stop to board a bus to ride to their assigned school. Shuttle service is provided in the morning and afternoon. There is no charge for this service. More information may be obtained at enrollment.

A breakfast and lunch program is available for all students, grades K-12. All schools operate on a closed lunch system (meaning all students will remain in the school during the lunch period). Breakfast prices are FREE for grades K-5, $1.90 for grades 6-12 and $2.00 for adults. Lunch prices are $2.60 for grades K-5, $2.70 for grades 6-12 and $3.30 for adults. Students who participate in the breakfast and lunch program will be issued a card with an account number. This number should be used on all transactions with food service. Money may be deposited in your student’s account

monthly, bimonthly or weekly. Free and reduced-price lunch applications are available. Forms may be picked up at enrollment or from the manager/clerk at the school where your student attends. You also may retrieve a free and reduced-price lunch application from our Web site at www.usd367.org. Any student preferring to bring a sack lunch may do so. Students have the opportunity to purchase a second meal for breakfast or lunch or an extra entrée. These items are priced each year. IMMUNIZATIONS REQUIRED

Immunization records will be checked at enrollment to see if your child meets minimum immunization require-ments. All students must have the mini-mum immunizations for enrollment. Kansas State Immunization require-ments can be viewed at :http://www.kdheks.gov/immunize/schoolInfo All students who are attending school for the first time in Kansas are required by state law to present immunization records (available at your doctor’s office) and a health assessment form when en-rolling. Students new to Unified School District 367 are also required to show a birth certificate. Kindergarten students must be 5 years old on or before Aug. 31, 2014. First-grade students who have not attended kindergarten must be 6 years old on or before Aug. 31, 2014.

If not done so through mail-in enrollment, all enrollment fees should be paid at the time of enrollment. Special education students attending classes at Paola should enroll at their attendance center. Health Assessment Any pupil up to the age of nine years who has not previ-ously enrolled in any school in this state, prior to admission to and attendance in school, shall present to the appropriate school board the results of a health assessment. This assess-ment shall have been conducted within 12 months of school entry by a nurse who has completed the Department of Health and Environment training and certification, by a physi-cian or by a person acting under the direction of a physician as per K.S.A. 72-5214. As an alternate of the health assessment required: A writ-ten statement signed by one parent or guardian that the child is an adherent of a religious denomination whose religious teachings are opposed to such assessment; or a written statement signed by one parent or guardian that such assess-ment will be scheduled and completed within 90 days after admission to school.

Immunization Any pupil entering kindergarten and first grade for the 2014-2015 school year shall, prior to admission, be required to present to the school authorities certification from a licensed physician that he has received or is in the process of receiving immunization against polio, diphtheria, measles,

rubella, whooping cough, mumps and tetanus. Any student failing to comply shall be removed from school. Alternatives to the above requirements are (1) certification from a licensed physician stating that the physical condition of the child is such as to prevent immunization, (2) a written statement by the parent that he is an adherent of religious denomination whose religious teachings are opposed to such immunizations or tests. It is highly recommended by the State Board of Health that children who receive their mumps, measles and rubella inocu-lation before the age of 15 months be reimmunized. The State of Kansas has passed an immunization require-ment for children entering kindergarten and first grade for the 2014-2015 school year. Children who have not had the dis-ease are now required to have a total of two varicella (chick-enpox) vaccines before returning to school next year.

Age Requirements Students entering kindergarten must be age 5 on or before Aug. 31, 2014. Students entering first grade must be age 6 on or before Aug. 31, 2014. A birth certificate to verify birth date is required. Parents are required by law to present a birth certificate upon enrolling their student for the first time.

DRUG FREE SCHOOL The Board of Education of Louisburg USD 416, recognizing the responsibility to promote the health, welfare, and safety to the students of the district, does hereby adopt the following policy for the protection of the students from the danger resulting from the abuse of alcohol and other drugs. All personnel working with students of Louisburg USD 416, as well as any other student under their direct or indirect supervision, who observe the use, sale, transfer, or possession of any alcoholic beverage, drug, medication or other controlled substance, or the paraphernalia associated with the use of such substances, shall report immediately to the building principal or his designee. Any staff person who observes a student whose behavior indicates an apparent dysfunction during the school day or a school sponsored activity, shall refer such student to the building principal or his designee for further observation. If, upon observation, the administrator has reasonable suspicion that the student’s behavior may be the result of the use of alcohol or other drugs, the administrator will make a reasonable effort to contact the student’s parents or legal guardians. If, after a reasonable effort, the parents or legal guardians cannot be contacted, law enforcement officers will be consulted, and the student placed in their custody. Any student who attends Louisburg USD 416, while on school grounds or in attendance at school activities held off of the school grounds, who exhibits obvious symptoms of having consumed alcohol or is apparently under the influence of alcohol or other drugs, or who has possession of alcohol or other drugs, will face suspension and/or expulsion. While suspended or expelled, the student will be prohibited from participating in or attending school activities. Police and/or juvenile authorities will be notified of every drug/alcohol related incident.

PRIVACY ACT OF 1974NOTICE TO PARENTS AND STUDENTS OF RIGHTS UNDER THE FAMILY EDUCATIONAL RIGHTS AND PRIVACY ACTS OF 1974 On November 20, 1974, the Family Educational Rights and Privacy Act of 1974 became law. Under this law, the parents of students enrolled in any educational institution receiving federal funds are given certain rights concerning the school records of their children. The following rights are accorded to you under this act: 1. You are entitled to have access to your child’s school records upon request. This request should be directed to the custodian of the school records in question. Access must be granted to you within 45 days after receipt of the request by said custodian. 2. When your child becomes 18 years of age, all rights formerly accorded to you as parents of said student become the sole rights of the student, and you will no longer have right of access to said student’s records. 8. Certain information is designated as “Directory” information with respect to each student and will be released to the general public upon request unless a student’s parents notify their Board of Education office within 30 days of the publication. Directory information includes the following information about the students: a. the student’s name, address, telephone number, date of birth; b. the student’s major field of study; c. the student’s participation in officially recognized activities and sports; d. the weight and height of members of athletic teams; e. dates of attendance; f. degrees and awards received; and g. the most recent educational agency or institution attended by the student.

Last night I spoke with 4 different people about the future location of the Miami County Jail. All 4 had different opinions about the location, and there are only 3 choices on the table.

The decision on the location, and the design, will have been completed before I would take offi ce.

The Commissioners presented Sheriff Kelly, and the voters, with a Campus concept, on a green-site. That is what was supported, and won approval. That is what the Sheriff, County Commissioners, and the Public wanted.

Sheriff Kelly should be able to make a choice between Downtown, and the vision he was given. The Commissioners, and Paola, should do everything possible to support his choice, and fi nd ways to make things work, not come up with complications.I will not be taking part in that decision, but

I will have a part in making sure, whichever location, and design is selected, that your tax dollars are spent wisely, and without waste.

I will support, and work, with the decision made by the current Commissioners.

My opinions, and feelings, on the location are irrelevant.

My skills in effi ciency, fi nance, budgets, and more… are relevant.
