Life and Accidental Death - CARRIER - All fulltime employees receive a life and accidental death & dismemeberment benefit of $0. These benefits are paid by COMPANY. The benefit will double in the event death is due to an accident.
Voluntary Term Life - CARRIER - Employees can choose to purchase additional life insurance in $0 increments. Minimum $0, Maximum $0. Eligible spouses can be covered in $0 increments up to $0, and children can be covered up to $0.
Voluntary Short Term Disability - CARRIER - All active, full-time employees are eligible to enroll. Benefits begin on: The 8th day for disability due to injury or sickness. The benefit is based on 60% of predisabiliy earnings, with a maximum benefit of $0 weekly and the benefit duration is for 12 weeks.
Long Term Disability - CARRIER - COMPANY NAME provides this coverage at no cost to all eligible employees. If deemed disabled for 90 days, Principal Financial will pay a benefit equal to 60% of your basic monthly earnings up to $0.
Flexible Spending Account - CARRIER - Employees may, through payroll deductions, set aside money on a pre-tax basis in a flexible spending account and reduce taxable income. This is done in two ways: 1. Employees may reimburse themselves for eligible medical expenses up to $_____ per year for themselves and dependents, regardless of whether dependents are covered under the COMPANY NAME plans. If you are participating in the High Deductible Health Plan and you have an Health Savings Account you can only participate in the FSA on a limited basis(_________).
401(k) Retirement Plan - COMPANY NAME - You can contibute portions of your pay to the plan as pretax contributions that are deducted from pay before taxes are withheld, so no current federal or state income taxes are paid on your contributions.
In 2009, participants will be able to contribute up to $16,500 or $22,000 if attaining age 50 before December 31, 2009. You are able to make contributions in 1% increments, you can contribute 1% to 100% of your bonus to the plan on a pretax basis, rollover contributions are allowed. Matching contributions are made on a discretionary basis - COMPANY NAME matches 25% of your first $1,000 in regular contributions, to a maximum of $500 in company matching contributions per year.
