*We will also need volunteer help at packet pick up on Friday, September 30th.

Hold Harmless: By participating in the Marty’s Mojo 5K Run/Walk for Leukemia, I agree to release and hold harmless the Marty’s Mojo Organization, it’s officers, volunteers, sponsors and the City of Menomonie from any liability , losses, claims, ac-tions, including attorney fees resulting from injury to person or damage to property arising out of my participation.
Logo/Photographic Release: I herby authorize Marty’s Mojo to utilize my photo for website, e-mail and social media communi-cations. I also grant them permission to photograph me during the run/walk and it’s related activities and to use those photo-graphs for any purpose, including but not limited to, commercial and promotional purposes.
