For any temporary order, e-mails or faxes of resumes will be sent within 48 hours, interviews scheduled, or someone will be scheduled to start work, depending upon client preference and requirements.

When placing an order for a temporary employee, please provide the following information for the Managed Care Staffers account manager.

Checklist for Placing an Order with Managed Care Staffers

• List of specific instruments, methods, standards, or procedures with which the employee must be familiar.

• Level of education or certification employee should have. Would you train a fresh college graduate?

• Other skills required (computer, language skills, etc.).• Duration of assignment (indicate whether position could become a regular position

within your company).• Shift or hours of assignment (flex time?, weekends?, rotating shifts?).• Does the work require labor that may be physically challenging or cause

discomfort to some employees (heavy lifting, fast pace, etc)?

Temp-To-Hire Employee Status Policy

• If your company wishes to hire a Managed Care Staffers temporary as your regular employee, you may do so providing the employee has worked as a Managed Care Staffers temp at your firm for 640 hours of continuous employment. After this period of time there is no conversion fee.

• If you would like to hire someone on your payroll before the 640 hours expires, a conversion fee is required.

1.      Appearance - neat, clean, and dressed appropriately for the assignment.

2.      Punctuality - report to the assignment 15 minutes early and stay for the duration of the time requested.

3.      Absence - must call MCS if illness or other circumstances cause leave of absence from an assignment. MCS will make replacements if requested.

4.      Work problems - call account managers at MCS to discuss problems that may arise at the assignment.

5.      Termination - give as much notice as possible if prematurely ending an assignment. A two-week notice is expected for long-term assignments. MCS will make replacements if requested.

Employee Guidelines Our candidates are expected to follow the following guidelines:

6.      Confidentiality - all client information is confidential. Work performed on an assignment, nor the terms of their employment should be discussed. Employees are allowed to sign company confidentiality agreements when necessary. 

7.      Pay Rates - pay rates are confidential. They should not be discussed with the client or other employees.

8.      Telephone Calls - personal phone calls must be made on the employee's time. This should not take time from the assignment. The MCS telephone number should be given to friends and family in case of an emergency.

 The bottom lines must always be signed by (1) the employee and (2) the direct supervisor of the employee, or signed by someone with the authority to verify his or her hours. The signature is verification that the hours on the timecard are accurate. You are billed accordingly.

Timecards must be returned to Managed Care Staffers every other Friday. In order to complete the payroll and billing, they must be turned in at the end of the work day on the second Friday of the pay period. If the employee is working the weekend, the timecard must be faxed by the following Monday morning at 9 AM. The fax is (847) 699-0399 and is available 24 hours a day.

Do not include lunch hours and breaks as hours worked unless it is your company policy to do so. If lunch hours and breaks are added as time worked, the invoice will reflect this.

A. PERSONNEL CLASSIFICATIONS AND RATESPersonnel work duties/classification and related billing rates are agreed upon per the above. Overtime shall be charged for all hours worked in excess of 40 hours per week.

Overtime must be authorized by client. B. SUPERVISION IN CLIENT’S FACILITY

Direct supervision of assigned personnel shall be the client’s responsibility. C. TIME KEEPING

Employee shall complete and sign a bi-weekly time card, and submit to client for approval and signature. Client shall return a signed copy to MCS and a copy to the temporary employee. D. INVOICING

Using approved time sheets as a basis, MCS shall submit bi-weekly invoices to client. Invoicing terms are net 15 days. E. BENEFITS

MCS is responsible for vacation pay, holiday pay, payroll taxes, workers' compensation, public liability and unemployment compensation.

F. WORKING FACILITIESWhile working on the client’s premises, it is understood that the client shall provide working facilities such as desk, work space, utilities, telephone, etc. at no cost to MCS.

Client also is responsible for all safety equipment, which shall be the same as the equipment provided for the client’s regular employees.

G. NON-DISCLOSURE OF PRACTICES

Upon written agreement with the employee, all services to be performed for the client will be considered secret and confidential and will not be disclosed to any

unauthorized person.

H. SOLICITATION OF PERSONNEL

MCS and the client agree not to solicit or hire any personnel from the other. This includes personnel whose employment at MCS is pending (acceptance by the client) for assignment to client’s place of business.

I. TERMINATION OF AGREEMENT

Client agrees to give notice of intent of termination of this agreement and/or the services of the assigned personnel 3 days prior to the desired date of termination. Termination of the services of personnel furnished under this agreement, for reasons other than completion of a specific job, can be effected immediately by verbal notification to MCS.

J DIRECT HIRE/CONVERSION

MCS charges no fee if client wishes to hire a MCS temporary employee as a regular employee after 640 hours of continuous employment.

If the client wishes to hire the temporary employee permanently before the 640 hours expires, a conversion fee is required and is calculated as follows: 1) from 1 to 160 hours, 25% of annual salary; 2) from 161 to 320 hours, 15% of annual salary; 3) from 321 to 640 hours, 10% of annual salary.

SAMPLE Direct Hire Placement Agreement AGREEMENT: Managed Care Staffers (MCS) will refer qualified candidates to Solutions Management Services, Inc. (SMS) per your request. A placement fee will apply to candidates referred by MCS for a specified or an alternate position and employed by SMS an affiliate or any other entity as a result of subsequent referrals by SMS as either an employee, consultant or independent contractor, within twelve (12) months from the date of MCS last referral.

MCS is an equal employment opportunity employer. All applicants are referred based on their qualifications only, with total disregard for their race, sex, age, creed, place of national origin or disability. 

GUARANTEE: Provided the full fee is paid within fifteen (15) calendar days after the starting date of employment, a ninety (90) calendar day pro rata guarantee will be in effect. Otherwise, a thirty (30) calendar day pro rata guarantee will be in effect. If the employee's employment is terminated for any reason other than reorganization, takeover, material change in job responsibilities or elimination of position within the applicable guarantee period, MCS will replace that employee for the same position at no additional fee. If MCS is unable to replace the employee within a reasonable time period, MCS will credit a pro rata portion of the fee paid toward future invoices. The credit will be equal to 1/90th or 1/30th, as applicable, multiplied times the number of days remaining in the guarantee period as of the last day of employment. 

FEE: All fees are based on a percentage of the employee's first year's compensation, as agreed upon by MCS, the candidate and SMS and may include expected bonus, commission or guaranteed salary increase that is part of the initial job offer. The fee is 25% and is due within 15 calendar days of the employee's start date. When applicable, sales and service taxes will be added to the invoice. 

TERMS: If a MCS candidate accepts a position with SMS within one year of referral by MCS, the full fee (25% of the first year's compensation) for that referral is due and payable to MCS. Your acceptance of our referrals constitutes acceptance of the terms of this agreement.

CONFIDENTIALITY: All resumes and other information regarding the placement of a candidate are confidential and may be used by SMS to evaluate the employment eligibility of a candidate only.
