Numerous studies have shown that uncorrected voiding disorders will
negatively impact spontaneous resolution rates of reflux, increase the risk

evaluation is based upon an accurate history and physical examination

I Sintomi del Basso Tratto Urinario (LUTS: Lower Urinary Tract
Symptoms) mostrano una incidenza in aumento in tutte le età ed
hanno un notevole peso socio-assistenziale.
Spesso sottovalutati e trascurati. Importante la prevenzione e la
diagnosi precoce, per evitare che le ripetute infezioni provochino
danni come “scars” e insufficienza renale.

La ICS classifica i LUTS in relazione alle fasi di
riempimento e di svuotamento vescicale:

• SINTOMI POST MINZIONALI
DEFINIZIONI E TERMINOLOGIA
LUTS - SINTOMI DELLA FASE DI RIEMPIMENTO:
•Alterata frequenza minzionale (valutabile dopo i 5 aa d’età)
Frequenza normale 5-7 minzioni/die
Frequenza aumentata ≥ 8 minz./die
Frequenza diminuita ≤ 3 minz./die
•Incontinenza urinaria: “ogni perdita involontaria di urina”.
•Incontinenza urinaria da urgenza: “perdita involontaria di urina
accompagnata o immediatamente preceduta da urgenza”.
•Incontinenza urinaria da sforzo: “perdita involontaria di urina con lo
sforzo o l’esercizio fisico, lo starnuto o la tosse”.
•Incontinenza urinaria mista: “perdita involontaria di urina associata con
l’urgenza e anche con lo sforzo, l’esercizio fisico, lo starnuto e la tosse”.
•Urgenza: “improvviso, irrefrenabile desiderio di mingere, non differibile

RIEMPIMENTO
LUTS - SINTOMI DELLA FASE DI SVUOTAMENTO:
Esitazione: “difficoltà descritta dal paziente ad avviare la minzione”.
Straining: “sforzo muscolare necessario per avviare, mantenere o
migliorare il flusso urinario”.
Mitto debole: “percezione di flusso urinario ridotto se messo a
confronto con le performance passate o con quelle di altri”.
Mitto intermittente: “flusso più volte interrotto e ripreso durante
l’atto minzionale”.
LUTS - SINTOMI POST MINZIONALI
“Holding maneuvers”: manovre per trattenere, posticipare la minzione
o per sopprimere un’urgenza”.
Sensazione d’incompleto svuotamento vescicale: sintomo descritto
e riferito come tale non prima dell’età adolescenziale.
Post-micturition dribble: è la perdita involontaria di urina che si
verifica immediatamente dopo l’espletamento dell’atto minzionale. E’ un
sintomo riferibile a pazienti di età > di 5 anni; il reflusso vaginale ne
rappresenta la situazione tipica.
Dolore genitale o al basso tratto urinario: la possibilità di rilevare il
dolore in sede genitale o nel basso tratto urinario, teoricamente
possibile, è nella pratica clinica pediatrica di difficile localizzazione ed,
in genere, non specifica.
