August 19, 2019 CALL TO ORDER The monthly meeting of the Lawrence-Douglas County Health Board was held Monday, August 19, 2019. Vice chair Vern Norwood called the meeting to order at 5:02 p.m. PUBLIC COMMENT There was no public comment. APPROVAL OF AGENDA Dan Partridge stated we need approval to pay for the video in item number 7d on Pregnant Mothers Stop Smoking Campaign. Shanda Hurla made a motion to approve the August 19, 2019, agenda knowing item 7d will be asking for approval to pay for video on Pregnant Mothers Stop Smoking Campaign. Michael Williams provided the second and the motion passed. REVIEW AND APPROVE MINUTES OF JULY 15, 2019 Michael Williams made a motion that the Minutes of July 15, 2019, be approved. Sandy Praeger provided the second and the motion passed. REVIEW AND APPROVE JULY MONTHLY FINANCIAL REPORT Dan Partridge, director reviewed the July Monthly Financial Report. Sandy Praeger moved that the July Monthly Financial Report be approved. Michael Williams provided the second and the motion passed.

PROGRAM REPORT – REVIEW PEFORMANCE MANAGEMENT REPORT Dan Partridge, director, reviewed the second quarter performance report on Positive Client Relationships, Culture and Value Alignment, Community Service Leadership and Financial Stewardship. The client satisfaction survey is at 98 percent. Clients now receive this survey the minute their visit is completed. We are also offering a gift card for clients to participate in the survey. The partner satisfaction survey was sent to 60 partners twice a year and is composed of 12 questions. Our target goal for this survey is an average score of 48 and our average was 51 in the second quarter. The goal of convening community health forums is 10 and we convened 17 this quarter. Dan stated the living wage of $16.25 per hour will be in place January 1, 2020. The goals of percent of insured clients with in-network insurance coverage is 80 percent and we reached 70 percent. The goal for clinic fee revenue is $90,948 and we collected $77,191. DIRECTORS REPORT Review of Wage Band Adjustments Included in the 2020 Budget Dan Partridge, director, reported increasing the living wage to $16.25 per hour has an affect on other wage bands. Dan stated this proposed new set of wage bands will be incorporated in the 2020 budget. City Commission Candidate Forum Dan Partridge, director, reported we have tentatively set the date of September 12th at 6 p.m. at the Lawrence Public Library for the candidate forum. More details to follow. Update on Review of Project LIVELY Program Linda Craig, clinic services director, reported meeting with community partners in senior services to review how senior needs are currently being addressed and identifying the areas we may be duplicating services or have gaps. They talked about how to work better to identify when to pass services to each other. Partners are looking to see if their role is care coordination or care management. They plan to meet again to determine how to move forward.

Review Public Perception Data Collected During Enhanced Behavioral Risk Factor Surveillance Systems (BRFSS) Data Collection George Diepenbrock, communications officer, reported the Docking Institute of Public Affairs at Fort Hays State University, conducted a survey regarding health patterns for Douglas County residents. The survey began in December of 2018 and ended in May 2019. There were 1,500 completed interviews most done by phone. The final three questions were regarding their experience with the Lawrence-Douglas County Health Department. Of the 1,500 interviews, 740 said they had no experience with the Health Department. Those 558 who had heard of the Health Department said their experience was good. Eighty-three percent who had experience with us said we do our job “well” or “very well.” Most have a positive impression of our reputation in the community, though 487 people also reported they did not know or were unsure of our reputation. We are seeing we are best known for clinic services and regulatory enforcement. George stated we still face the challenge of a large number of people who haven’t used our services and don’t recognize our work in the community. However, most people who know us think highly of us. People associate us with advocating for a healthier community and encouraging healthier behaviors. The next steps are to build on our successes, connecting people to our services, engagement on community health, equity and informatics. Tangible projects include website redesign in January 2020, updating our brand/logo and sustain momentum of Healthier Together/mySidewalk. As a brand tagline, the Board liked Health for All. George also presented a 30-second video on the Pregnant Mothers Stop Smoking Campaign. By consensus, the Board approved entering into a contract with Midco to do a 2-month commercial for $2,000. Announcements The Health Department received the Gold Premiere USDA award for breastfeeding support to WIC families. Other New Business 1. The next scheduled Health Board meeting will be Monday, September 16, 2019, at

5 p.m. in the first-floor meeting room.

Income & Expense Statement

Revenues City & County Operating Revenue: The 2nd half of City and County funding was received in the prior month, and

on track with the year to date budgeted amounts for both City and County. Contract Services: Payments increased for the month of August by $1480, low compared to budget for the month

and about 39K year to date. Program Fees: Payments continued to increase for the month of August by about $4K and on track with the year

to date budgeted amounts. Federal & State Grants: Federal grant receipts increased from the prior month by 286K and on track with the

year to date budgeted amounts. State grants increased from the prior month by 78K and on track with the year to date budgeted amounts.

Expenses Gross Salaries and Wages / Payroll Taxes: Continue to trend less than budget totaling $173k less year to date. Contractual Services: Under budget for the month of August and $54k over budgeted amount year to date, in

part due to outsourcing of the payroll, accounts payable and financial statement functions and consulting agreement with SS&C.

Other Exp./Equipment: Exceeded budget. Large purchase in the amount of $12,500 “Douglas County Administration – My Strength” grant expense.

Capital Purchases: See detail as outlined on the 2019 Funded Depreciation expenditures.

Balance Sheet General/Operating

Balance increased in August by $203,479 to $1,486,834 Board Designated Reserve Fund Balance increased in August by $117 due to recording interest Kay Kent Public Health Service Award Fund Balance increased in August by $6 due to recording interest Funded Depreciation Balance increased in August due to grant money in the amount of $13,134 transferred from the

General/Operating account to the Funded Depreciation account. Capital purchase in the amount of $4,919 was written out of the account. See detail as outlined on the 2019 Funded Depreciation expenditures.

Suicide accounts for almost 80% of all violent deaths in Douglas County (85 suicides versus 22 homicides for 2013-2017)

and 78% of all firearm-related deaths.

Like Kansas, suicide is the 8th leading cause of death in

Douglas County accounting for 85 deaths from 2013-

2017 but it is the fourth leading cause of death when

years of potential life lost is used as a metric (Figure 2).

Suicide is a public health problem both nationally and locally. Since 1999, suicide rates for males and females have increased in the U.S. and Kansas. During this time, Kansas saw an overall increase in the suicide rate of 45% - the 5th largest percent increase in the nation. In Douglas County, suicide rates have increased 84% since 1993. The differences in these rates are statistically significant (Figure 1). The purpose of this brief is to describe who in Douglas County

is dying of suicide and who is being treated in the ED for suicide

ideations and suicide attempts.

Suicide Attempts and Ideations

Suicide ideations include thoughts of dying

and may include a plan to commit suicide.

Nationally, about 4.3% of adults had suicide

ideations(2017). The group with the highest

amount of ideation are those aged 18-25 years

(10.5%).

A suicide attempt is the strongest predictor for

future suicide ideation, another suicide attempt

and death by suicide. Nationally, the

prevalence of suicide attempts in the past year

is highest among adults 18-25 years old (2017).

Age

In Douglas County half of the people who died of suicide were younger than 45 years old and half were older than 45

years (2008-2017).

In Kansas (2008-2017), the age groups 25-44 , 45-64 have statistically higher rates (21.0 .21.7 respectively) than those who

were 15-24 years old (16.4) or 65+ year old(16.5).

Suicide Attempts and Ideations by Age

Of Douglas County residents treated for suicide attempts in an ED, half were younger than 21 years old and half were

older than 21 years old (2018). The largest number of suicide attempts occurred in those 20 years of age (n=28) followed

by those 15 years old (n=25). There is no statistical difference between the age of males and females who attempted

suicide and were treated in an ED.

Gender

Like national and State findings, males account for

most suicides (76% in 2008-2017) in Douglas

County. In fact, the ratio of age-adjusted death rates

by sex suggests that Douglas County males are

almost 3.5 times more likely to die of suicide than

are women (2008-2017).

Even though female suicide rates are lower, Kansas

findings suggest that between 2007 and 2016, the

increase in suicide rates for females was more than twice the increase in male suicide rate (52.9%,

24.2% respectively). Nationally, males aged 65+ and women age 45-54 have the highest suicide

rates. Kansas data appears to show the same trend

(Figure 4).

Figure 5 shows ideation rates are higher than attempt rates for males

and females of all ages. For ideations, an almost equal number of

males and females were treated in the ED (51.8%, 48.2%

respectively), but there are statistical differences by age group. Male

ideation rates are higher than female ideation rates starting at age 30.

Compared to the 10-19 age group, more residents in their 20’s are

treated in the ED for suicide ideations (Figure 3), but the suicide

ideation rate for those 20-29 is lower than the ideation rate for 10-19

year-olds (Figure 5).

Suicide Attempts and Ideations by Gender

Unlike completed suicides, suicide attempts are

more common (68.2%) in females while males

account for about 1/3 (32%) of attempts (2016-

2018).

Male suicide attempt rates are lower than female

suicide attempt rates in all age groups (Figure 5).

More 10-19 year old residents are treated in the

ED for a suicide attempt than other age groups

(see blue circles in Figure 3) and their suicide

attempt rates are highest as well (Figure 5).

Race/Ethnicity

Most (91.9% females and 90.0% males) Douglas County

residents who die of suicide (2013-2017) are White non-

Hispanic. Suicides occurs in the other racial and the Hispanic/

Latino groups in Douglas County, but the numbers are too

small (n < 10) to do small group analyses.

In Kansas (2013-2017) suicide rates are higher for White

Kansans (16.5 per 100,000) than for Black Kansans (8.7% per

100,000). However, the highest suicide rate (27.2) in Kansas

occurs in “other racial group”. According to CDC, American

Indian, Alaska Natives have highest rates of suicide of any

racial/ethnic group in the U.S.; however, there is no evidence

that this is the case in Kansas. Nationally, suicide rates have

increased 10% for Blacks between 2015 and 2016 and 9% of

Latinos, compared to a 1% increase for Whites.

Suicide Attempts and Suicide Ideations by race/

ethnicity

Like deaths due to suicide, White Douglas County

residents comprised most (75.6%) of the suicide

attempts and suicide ideations (76.4%) treated in the

ED in 2018, followed by those who were classified as

“Other” (10.8%), Black (8.0%) and American Indian

(4.0%). Compared with the population of Douglas

County, the proportion of White suicide attempt

patients treated in the ED is lower (87.3%, 75.6%

respectively). Conversely, the proportion of Black

suicide attempt patients is higher than the proportion

of Black residents in the population (8.0%,6.1%

respectively). A similar pattern is seen for ideations

with 9.5% of the ideation patients being Black.

External Cause

Like national and State data, firearms are the most common external cause accounting for 48.7% of deaths due to suicide

in Douglas County, followed by suffocation (25%) and poisoning (16.5%). This pattern has not changed significantly

since 2008 (Figure 6).

A majority (54.8%) of males in Douglas County died from firearms, followed by suffocation and poisoning (23.5% and

13.9%). For females, an almost equal number of

women died from firearms (29.7%), suffocation

(29.7%) and poisoning (24.3%).

National data suggests that the method of suicide

differs by age group and sex. Except for those

age 10-14, firearms are the most frequent external

cause for males and are the first or second most

frequent external cause for females. Suffocation

is the primary cause of death for females 10-14

years and suffocation and firearms each account

for about 1/2 of suicides in males 10-14 years.

Suicide Attempts by External Cause

Compared to deaths due to suicides, a different

picture emerges for suicide attempts treated in

the ED. In 2016-2018, 43% of suicide attempts

in Douglas County involved poisonings,

followed by injuries due to sharp objects or

cutting (28%), and unspecified mechanism

(23.5%). Attempting suicide by other individual

methods (e.g., firearm, smoke/burn,

electrocution, blunt force) are too small to

report separately (n < 6). Figure 7 shows males

and females use similar methods when

attempting suicide.

For people 25-49 years old who die of suicide, a majority (58.5%) are

single, followed by almost equal numbers of people who are divorced

or married (20% and 26% respectively). For those 50-64 years old,

fewer people (18%) who die from suicide are single, and there are

similar numbers of married and divorced (38.5% and 30.8%

respectively). For those 65 years old or older, almost equal numbers

of people are either married or widowed (48% and 44% respectively).

Geography

Over a ten-year time period (2008-2017), 82% of people

who died of suicide were from Lawrence, followed by 9%

from Baldwin and 6% from Eudora. Baldwin City had

the highest crude rates of suicide 2.86 per 1,000

population, followed by Eudora and Lawrence (1.46 and

1.38 per 1,000 respectively.) (U.S. Census Bureau, 2014).

Suicide Attempts and Suicide Ideations by Geography

Figure 8 shows that in the three largest cities in the County,

younger residents are treated more frequently in the ED for

ideations and attempts than other age groups.

Marital Status

Most people (95%) younger than 25 years old

who die of suicide are single. This finding is not

surprising as most people in Douglas County less

than 25 years old are single.

Often referred to as deaths due to despair, suicides along with deaths from drug overdoses and alcohol poisoning are

responsible for recent decreases in life expectancy. In Douglas County, suicide is the 8th leading cause of death but is

the 2nd cause of death for those 15-44 years of age. Existing data sources allow us to look at demographics and

external causes of injury. Firearms are the most common cause of suicide in males and have become as common a

method in females as well. While Douglas County males are more likely to die of a suicide, it is interesting that they are

much less likely to attempt a suicide. At the same time, males are just as likely to have suicidal ideations as females, but

the rates of these ideations are different for males and females in different age groups.

Of particular concern are the number of suicide attempts and ideations in those age 10-19 years in Douglas County.

Nationally, there has been a marked increase in the number suicide attempts and ideations treated in the ED among

youth 10-19 years old. Risk factors for youth suicide include rape, bullying victimization, sadness or depression,

vomiting or taking laxatives to lose weight, and using substances. Moreover, Kansas Communities that Care data

suggest that students who vape or smoke may also be at risk for suicide ideations.

Conclusion

There is an urgent need to understand the root causes of suicide, attempts and ideation in our community. Suicide is usually

caused by multiple factors. In addition to mental health conditions and prior suicide attempt, other factors include social and

economic factors, access to lethal means, and poor coping and problem solving skills. Our community has embraced Zero

Suicide which sets an aspirational goal of zero suicides among people who access health services. As an adjunct to this

effective prevention approach , other community approaches should be explored. For example, in Washington County,

Oregon, their in-depth analyzes of people who died of suicide suggested that knowing the community factors associated

with suicides allowed them to institute prevention measures that complemented their Zero Suicide initiative and prevented

suicides. For example, they found that the most prevalent risk factors for suicide were depressed mood and experiencing a

crisis. In their community, an eviction crisis was common. When the police changed their protocol to have a mental health

professional with the police at the time of an eviction, the number of suicides after this type of crisis decreased. Additionally,

investigators also noticed that several people surrendered their healthy pets to the animal shelter before they died by suicide.

After training the animal shelter staff and volunteers to question (“Are you thinking of killing yourself”), persuade and refer

(Get person on the line to the suicide prevention line) (QPR), they were able in three months to intervene with 7 people who

intended to die by suicide.

Overall, it is clear that suicide, suicide attempts and suicide ideation occur across the life span. There is a continuing need to

identify and support people in need and creating a protective environment. Elderly men in particular are at high risk for

suicide and may not talk about their feelings or intentions. It is known that chronic health issues and economic or financial

strain, such as difficulty covering medical, food and housing expenses may increase a person’s risk for suicide. Lack of social

connectedness is also one of chief causes of suicide. . The current efforts to strengthen access and delivery of suicide care

will benefit those with contact with the medical care system and will address many prevention strategies including teaching

coping and problem-solving skills. And finally, preventing suicide and addressing the health care needs of persons at risk

for suicidal behavior require public health information-sharing efforts that raise awareness and provide evidence of effective

community-wide interventions.

Methods

The KDHE multiple cause-of-death research files from 2008-2017 were used to identify deaths due to suicide. Cause of

death referred to in this report is the primary or underlying cause of death, which is defined as the disease or injury that

initiated the chain of events leading directly to death, or the circumstances of the accident or violence that produced the

fatal injury. The underlying cause of death is established through a system called the International Statistical

Classification of Diseases and Related Health Problems, 10th Revision (ICD-10). This system promotes uniformity and

comparability in the collection and presentation of mortality data. All analyzes were conducted using SAS 9.4.

Suicides were defined as the underlying cause of death codes: U03, X60-84, and Y87.0(7). Suicide deaths were

categorized by external cause of suicide using the ICD-10 underlying cause-of-death codes: firearms (X72-X74),

suffocation (X70), poisoning (X60-X69), and other means (U03, X71, X75-X84, and Y87). Poisoning includes both

drug and nondrug (e.g., toxic substances or gases) intoxications. Suffocation includes hanging, asphyxiation, and other

mechanical and nonmechanical threats to oxygenation (e.g., trapped in low oxygen environment). Other means include

mechanisms such as cutting and piercing, falls and jumping, burns from fire, hot substances, and hot liquids; and

transportation. References to rates being significantly higher or lower indicate that differences are statistically significant

at the 0.05 level.

Cases of Douglas County residents who were treated in an ED for a suicide attempt or ideation were identified using

ESSENCE (Kansas Syndromic Surveillance Program). Suicide ideation was defined as ICD-10 CM codes R45.851 or

R45851. Currently, there is no CDC injury matrix for classifying the external mechanisms for non-fatal suicide attempts

and intentional self harm. The ICD-10 CM codes that were used to define suicide attempts are listed in Appendix 1.

This list was developed with a team of clinicians and coders at LMH-H in Lawrence Kansas. These ICD-10 codes were

toxic substances, poisoning plus another mechanism, cut or pierce, asphyxiation, unspecified attempt, and other.

Very few cases had both an ideation and attempt code, but when both of these codes were found, the case was coded as an

attempt. Age-adjusted rates for suicides were obtained from KDHE’s Kansas Information for Communities http://

kic.kdheks.gov/ using the 2000 standard U.S. population. Rates in Figure 1 are age-adjusted per 100,000 population.

Age-specific rates in Figure 4 are per 100,000 population. Age-specific rates in Figure 5 are per 1,000 population. When

rates are unreliable due to small numbers, this is noted in the graph footnote. Crude rates were calculated using the U.S.

Census Bureau’s population estimates as a denominator. When frequencies are reported, numbers less than 6 are suppressed

and are indicated either as a * or a 0.
