Fall 2006 Development of UCSD Medical Center Maternal –Newborn Units as a

new site for clinical education of N354 (Care of the Childbearing Family) students. This site is unique because it gives students the opportunity to see natural low risk births in a birth center with the midwifery model of care and the use of a doula (a special woman that supports the laboring woman and her family). Additional opportunities include interdisciplinary teaching and rounds with medical students and residents, and opportunities to practice in the NICU and hi-risk antepartum units. Included development of a new N354 Clinical Guidelines Syllabus specifically for students at UCSD.

Fall 2005- Instituted the routine practice of use of appropriate adjunct faculty from

the SON for thesis second or third chairs (Fall 2005) in order to engage the nursing community with the SON.

2005- Implemented the use of “Noelle” A childbirth simulator for “hands present on” education of normal vaginal and breech birth, shoulder dystopia and post

partum hemorrhage for graduate nurse-midwifery students in N640 and N742 and for undergraduate N354 nursing students to practice prenatal visits.

2003-2004 UCSD Program transfer to SDSU: As the Director of the Nurse-Midwifery Education Program was responsible for the transfer of nurse-midwife specialty

courses N222, N223 and N224 previously taught at UCSD to the following SDSU Courses N635, N640, N641, N742, N743. This required the following preparation: School of Nursing Graduate Program Committee and faculty approval of new course descriptions and objectives New catalog descriptions Class conversion time from 4 to 3 hours Conversion from Quarter to Semester Hours Development of new brochure for program advertisement Development of the Nurse-Midwife Advisor Job Description for the SON

2003-2004 N635: Development of new course, which combined a clinical practicum in

through the lived experience of midwifery situations in the clinical and classroom arenas. Outcomes (positive) for a two-year period were evaluated by an open- ended questionnaire that was also developed. (SDSU/UCSD and SDSU) (See publication 2006)

1998-2003 SDSU/UCSD Internship Program: Primary faculty for development of the

Internship Program that allowed students to continue in a clinical setting after graduation, while waiting for their certification exam results and state licensure. This program allowed students to continue to gain further expertise and for employers to begin the orientation process without incurring additional costs. (SDSU/UCSD)

1994-2003 SDSU/UCSD Bridge Program: Primary faculty for the development of a

program and curriculum to supplement the experiential background of prospective nurse-midwifery students. The program enabled RN’s without Labor and Delivery nursing experience to complete an eighty-hour clinical practicum with volunteer nurses at UCSD in order to learn the basic skills prior to program admission. Evaluation tools demonstrated the succeeds of this program (SDSU/UCSD) (See publication 1997)

Examination of Morbidities and Co-morbidities Associated with Labor 2007-2009 The purpose of this study was to explore maternal and neonatal morbidities and co-

morbidities associated with induction of labor. This was a retrospective and prospective comparative descriptive design conducted at Sharp Grossmont Hospital Women’s Center. The Labor & Delivery log was reviewed to identify patients, who had the admitting diagnosis of induction of labor, and those medical records were reviewed and data collected. The data was compared to those patients and their newborns that delivered during the same time period who began labor spontaneously. The sample consisted of 400 women who were admitted for induction of labor and

their newborns compared to 400 women who spontaneously begin labor and their newborns for a total of 1600 medical records. Descriptive statistics and frequencies were calculated for the medical information. Data was analyzed using SPSS for Windows version 15.0.1 (SPSS, Inc, Chicago, Ill). An analysis of variance (ANOVA) was conducted to test group differences. No direct benefits for participants exist in this study, yet as the rates of labor induction rise, it is imperative to determine the effects of this medical intervention. It is hoped that this research may benefit other patients by providing information regarding management of care for patients who are choosing to expedite their delivery. 2006-2008 Efficacy of Mantram Repetition during Childbirth The purpose of this study was to examine the efficacy, outcomes, and patient

satisfaction of a mind-body-spiritual intervention, mantram repetition, with respect to management and mitigation of anxiety and stress related to the childbirth experience. A convenience sample of 80 subjects attending the American Red Cross (ARC) Healthy Pregnancy Healthy Baby (HPHB) childbirth preparation class were enrolled. Demographic information and baseline scores addressing anxiety, perceived stress and coping abilities were collected prior to the introduction of the intervention. Subjects were randomly assigned to either the experimental group (n= 40) which received the mantram instruction in addition to childbirth preparation classes or the control group (n = 40), which received only childbirth preparation classes. Four to six weeks after the childbirth experience, collection of delivery statistics, anxiety, perceived stress and coping scores were conducted. Statistical comparison was accomplished. . 2004-2009 Women’s Experiences of Nurse-Midwifery Presence (“ being with woman”)

during Childbirth

The purpose of this study was to learn more about women’s experiences of nurse-midwifery “being with woman” during childbirth. A descriptive prospective correlational replication design was used to examine the effects of the childbirth setting, significant others presence, pain relief, length of labor and number of nurse-midwifery providers on the women’s experience of nurse-midwifery “being with woman” during childbirth. A 29-item seven-step response Likert scale questionnaire the Positive Presence Index (PPI) was administered to low risk postpartum women who were cared for and delivered by nurse-midwives prior to their discharge from the hospital. Data gathered from this study can be used to modify hospital settings and variables for nurse-midwifery clinical practice in order to increase the use of and amount of nurse-midwifery presence. 2004-2005 Characteristics of Effective and Ineffective Preceptors Identified by Midwifery

Students The purpose of this study was to learn about nurse-midwifery students’ perceptions of the clinical experiences and role modeling that they receive from their clinical preceptors. The qualitative portion of the study used an open-ended website questionnaire method to gather information through content analysis about preceptor behaviors (themes) that are conducive or detrimental to student learning in the clinical setting and to identify traits (themes) of good and bad clinical preceptors from the student perspective. Quasi- statistics were generated to tabulate themes, relations and insights from the data. The descriptive portion of the study examined rankings of preceptor qualities already identified from nursing literature. Data gathered from the study can potentially increase the understanding of students’ experience of being clinically precepted and help to identify traits of good and bad preceptors (PI: Lauren P. Practice, 2007 2002-2003 An Interpretive Exploration of the Meaning of Presence “Being with Woman”

during Birth for Midwives

This study explored the meaning of the phenomenon of the midwife’s experience of being present with a woman during childbirth. The value and importance of being with a woman during childbirth, specific and unique to midwifery care, is reflected in midwifery philosophies and models of care. This study looked through the lens of midwives who have written poetry about births they have attended to learn more about the experience of being with a woman. Hermeneutical phenomenology was selected for the philosophical methodology and method because it looked at both the phenomenon and the use of language to describe the phenomenon. The researcher reflected upon and interpreted 18 selected poems as data and developed themes, which described the midwives, lived experiences of being with a woman. The survey considered wait times to see nurse practitioners, satisfaction with care and reasons for clinic selection. In addition, demographic data was analyzed to compare possible difference in answers to the survey based upon language preference in filling out the Patient Satisfaction Survey (English versus Spanish) and between store locations that reflect divergent socio-economic perspectives. 2006-2009 Oral Health and Dental Care in Low Income Pregnant Women This study considered oral health care practices and status and dental care access

among low-income pregnant women in two prenatal health clinics in San Diego County. Access to dental care, current state of oral health, dietary risk factors, and personal oral hygiene practices were investigated by age and ethnicity and pregnancy outcome. Prenatal records were accessed retrospectively to verify information on the above variables. Specifically, information from the Operation Samahan Oral Health Assessment Form (Appendix 1) was utilized as data. Descriptive statistics were calculated and reported on all major variables measured in this study for the overall sample and different subgroups. Correlational analysis such as Pearson correlation coefficient, Spearman correlation coefficient and partial correlation coefficient were used to assess bivariate linear associations among the oral health practice questions and other variables. 2005-2006 Lamaze International: Understanding Childbirth Education

The proposed research to be conducted by three interdisciplinary investigators with extensive experience in the area of childbirth and women’s health, will fully explore what occurs in childbirth education classes, including informational and philosophical content, teaching strategies, and student-teacher interactions The study design is formative and qualitative; researchers will use established focused ethnographic assessment techniques to derive a rich, culturally contextualized in-depth description of what actually happens in childbirth education classes (versus what is supposed to happen, as per curricular descriptions). Participants will include a purposive sample (i.e.. a non-probability sample purposefully assembled in regard to identified variable under consideration) of teachers and their students as well as a convenience sample of pregnant women who chose not to enroll in childbirth education classes Such findings can provide the foundation for further study of both the processes and outcomes of childbirth education. 2003 NIH Planning Grant RFA RR-04-002 Creating and Sustaining Interdisciplinary Teams

Medical errors in hospitals (i.e., errors in the delivery of health care) kill more people each year than car crashes, breast cancer, or AIDS. Many others survive medical errors with serious injuries or impairments. A central cause of this safety problem is the use of inefficient and ineffective clinical teams. Considerable research addresses team effectiveness and the relationship of team functioning to safety and error reduction. But the bulk of that literature concerns industries outside of health care; moreover, it is narrow in disciplinary scope, limiting the knowledge it can generate. To overcome these present shortcomings and limitations, the main objective of this planning grant is to determine, demonstrate, and disseminate a process for sustained and systematic interdisciplinary research into clinical team functioning as this relates to patient safety. Labor and delivery (L&D) is an excellent setting for such an endeavor because: birth is the most common reason for admission to US hospitals; many disciplines are routinely involved; and breakdowns in L&D safety are very expensive and have lifelong consequences. The specific aims of this three-year planning grant follow directly from the main objective. They are: (1) In Year 1, to identify, establish, test, and document processes and structures that support and sustain effective interdisciplinary research team building and functioning; (2) In Year 2, as an interdisciplinary research team, to define a specific clinical team and patient safety problem in L&D and develop new or hybrid interdisciplinary research approaches to that problem; and (3) In Year 3 to collaborate as an interdisciplinary research team on feasibility/pilot research and then develop full-scale research proposals regarding the agreed-upon problem. The interdisciplinary research approach that is designed, and the resulting R-01 proposals developed, will yield new insights into clinical team functioning and patient safety that could never be achieved by the present practice of isolated disciplinary or multi-disciplinary inquiry.
