FROM THE PRESIDENTA brief bio:My name is Elizabeth Rudy, PMAC. I am currently serving my fourth year as president of the ASPMA. I was working in Podiatry for about 12 years with private practice before our practice recently joinied ventures with a busy Orthopedic practice in Metairie, Louisiana. On my off-time, I spend it with my family at home in Hammond, Louisiana.

What we do:The purpose of the ASPMA is to promote, foster, improve, sustain and advance the profession of Podiatric Medical Assistants. Our goal is to elevate the standard of education of Podiatric Medical Assistants. The ASPMA also plays a role in the advancement of the podiatric medical profession and to the improvement of public health.

Recent or future projects:We recently returned from the Midwest Podiatry Conference in Chicago, Illinois- that was held in April. I can honestly say that this past Midwest conference was one of the best ones that I have attended. We had a variety of new speakers and topics- as well as advanced techniques and workshops! We had a great response from surveys from the conference and we hope to see you all there next year!!

Additionally, we are preparing for the upcoming APMA National in July in Washington D.C. This is a truly remarkable conference that we all look forward to every year. We are happy to say that the ASPMA is also helping to sponsor the Annual Student Quiz bowl at the National and we are truly excited to be a part of this amazing event! If you plan to attend the conference, please come enjoy this experience with us! We will also be offering the PMAC exam and review course to any assistant who wishes to become certified. Our PMAC exam is the only assistant exam recognized by the APMA and is accredited.

PERSPECTIVES

It has been a great experience attending this year’s conference. I enjoyed the opportunity to meet assistants, doctors, reps, vendors & just a wonderful group of people.

As a first timer, I was nervous and didn’t know what to expect. I had a fear of getting lost in the hotel, stuck in an elevator, or oversleeping. Luckily only one of those happened, and there were people to help me find my way. Even though I was a bit scared, everything went smooth, and made the experience one to remember.

Personally, I think this conference was very well organized & successful. I truly enjoyed the lectures & workshops. The topics of the lectures were so informational & gave me great ideas on what & how I can improve my office. The variety of topics & the mix of doctors was great. Maybe next year we can have a few more doctors lecture. The hands-on workshops were great, but I will admit, I wish there were more. There is nothing better than being able to get your hands dirty a little bit. The one I enjoyed most was being able to drill screws into the model foot. It’s one thing seeing the screws on X-Rays, and seeing a patient post op, but to have that feeling of actually drilling in a screw was pretty great. Can’t wait to see what we have for next year!

I’m sure there are things that some of you would either want to change or have improved, but I personally enjoyed the conference & appreciate all the hard work that goes into it. It’s a great opportunity for assistants to learn and expand their careers.

I’m looking forward for next year & meeting more amazing people.

What Is Charcot Foot?

Charcot foot is a type of bone deformity that can lead to serious damage and disability. The bones are weakened enough to fracture, and with continued walking, the foot

eventually change shape. Charcot foot is a condition causing significant nerve damage (neuropathy)If a person suffers from especially bad neuropathy, the sensation in their feet and their sense of balance may be affected. As the disorder progresses, the joint collapse and the foot takes on an abnormal shape, such as a rocker bottom appearance.

Charcot foot can cause a visible deformity, disability and even amputation. This is very important for the patient living with diabetes – a disease often associated with neuropathy – take preventive measures and seek immediate care if signs or symptoms appear. A lot of patient sufferers continue to walk on the affected foot, due to the neuropathy preventing the foot from hurting. This can make the injury much worse.

Causes of Charcot footCharcot foot typically develops over a period of time and requires a number of factors to occur. It develops because of neuropathy, which decreases sensation and the ability to feel temperature, pain or trauma. People with neuropathy especially those who had a long time. Both factors can increase the risk of developing Charcot Foot. The trigger for Charcot foot can be a sprain or twisted ankle that goes unnoticed because of reduced feeling from nerve damage. Symptoms are warmth to touch, redness in the foot, swelling in the area, pain and soreness.

DiagnosisThe Doctor will examine the foot and ankle, x-ray and other imaging studies and test may be ordered .Charcot foot is treated by reducing pressure on the foot and wearing a plaster cast to allow to set and heal .Nonsurgical Treatment consists of immobilization ,custom shoes and bracing ,activity modification.

69th Annual Region III Conference

Ever since I became a Certified Podiatric Medical Assistant in 2011, I have attended The Region III conference in Atlantic City. One because I live in the beautiful state of New Jersey but also for the great program that the ASPMA puts together for the assistants’ year after year. This year not only was I able to attend but also lecture along with my fellow Board members Michelle Handley, PMAC and Karen Keathly, PMAC.

The assistants had a wonderful two-day program that included a Clinical and Administrative track. They were able to learn about Radiology, Empathy, the Hiring Process, ICD-10 Coding Updates, Onychomycosis, Practice Management and few other topics. Thank You to all the speakers! Each speaker did a phenomenal job with embodying their lectures and making sure all their points were reached to each assistant.

We also offered the Clinical Exam at the conference for the assistants to become Certified Podiatric Medical Assistants. The assistants who were registered enjoyed an 8-hour review course before taking the exam the next day to be able to further their careers. Congratulations to all that took the exam!

Thank You to all that attended. I can’t wait you see all next year at the 2019 70th Annual Region III!

Midwest Podiatry Conference 2018

The Midwest Podiatry Conference this year was a great success! It was held in Chicago, IL 04/19/2018 through 04/21/2018. There were attendees from

many states including Illinois, Indiana, Iowa, Michigan, Nebraska and Wisconsin to name a few.

We had a variety of educational topics to maintain and expand our knowledge of both clinical and administrative areas. Our speakers came from a diverse population including podiatrists, certified podiatric medical assistants, human resource experts, a registered dietician, a law enforcement officer and even Congressman Brad Wenstrup.

This year’s conference included a few “hands on” workshops as well. We learned about compression garments and got to try our hand at fitting these garments. Dr. Brittany Jones gave a lecture and workshop on infection control. Last but certainly not least, we had a fixation workshop. We discovered many different types of fixation and got to use the equipment and the fixation devices on saw bones. As assistants we got to experience placing screws, plates and other fixation devices. Where else would you get to experience that?!

At MPC this year, we had over 150 exhibitors. It was a great way to see the latest technologies, find quality products and services and even get a great deal on some products or equipment.

I am very pleased to have attended MPC 2018. There is always something to learn, new associates and friends to meet and fun and exciting things to see and do in the windy city! If you did not get a chance to attend MPC in 2018, mark your calendars for April 10-13, 2019. We hope to see you there!

2018 RECERTIFICATION REQUIREMENTS

In order for a Certified Assistant to maintain his/her certification, an annual total of 20 recertification

credits (10 credits if certified between August 1, 2017-December 31, 2017) obtained during 2017 must

be submitted. Please submit qualifying material accumulated during 2017, this would include any

2017 Journal questionnaires answered, and any seminars or conferences attended during 2017. The

hours for each meeting are based on contact hours offered in the meetings. Certificate(s) given to

you at the meetings will reflect hours you can use. If certificates do not reflect credit hours, a copy of

that apply, are to be postmarked by May 31, 2018 and returned to the following address.

All DPM’s who do not recertify will be dropped from the recertification database and will be unable to

use their X-ray certification credentials if it is not received before July 15, 2018.

X-ray certifications obtained between August 1, 2017 and December 31, 2017 are required to submit 10

CME. In 2019 you will renew for the full year.

All others are required to submit the full year renewal of 20 CME credits, renewal fee and any additional

fees that apply.

Once you have all qualifying material accumulated for 2017 in one packet, it is to be submitted to the

above address.

You may use the credits accumulated to renew you DPM license, as well as any of the sources listed on

the enclosed sheet. Please submit copies of your certificates from these meetings.

PERSPECTIVES

I didn’t know you treated THAT!

Scenario:Patient presents for a “routine” foot-care appointment. Upon removing her socks and shoes there is visible swelling, redness, and water filled blisters.

PMAC: “What happened?!”

Patient: “Oh, I was walking my dogs last night and I got into a pile of fire ants. I guess I’m allergic to them! They are so painful, and itchy. As soon as I leave here I’m heading over to my family doctor to see what they can do.”

PMAC: “There’s no need for that. You’re here. We can help you. The Doctor will take a look and see what’s going on, and possibly give you a prescription to help with the healing and to make sure those blisters don’t become infected. She can also prescribe you something for the itching.”

Patient: “REALLY? I didn’t know you treated conditions like this. Are you sure I don’t have to go to my ‘regular’ doctor, or maybe even a dermatologist?”

PMAC: “No ma’am...I promise we can take care of that here. The podiatrists don’t just cut toenails, they treat everything related to the foot....skin conditions included!”

Patient: “Well then, I’m glad I’m here! One stop shopping!”

This patient left the office with 3 prescriptions in hand. One for the itching, an anti-biotic to prevent infection, and an epi-pen to have for future use. (The patient mentioned to the DPM that she felt a tightness in her throat immediately after being bitten.)

PERSPECTIVES

Do you slip into Robot Mode?

Do you find yourself slipping into robot mode? Does your voice go monotone, flat, unconcerned when explaining something to a patient because this is

the hundred time today you said the same thing? It’s so important to keep the passion in our voice and actions when dealing with patients on a daily basis, either in person or over the phone. For them sometimes it’s the first time they are hearing the words or having the experience. For many of us after years of doing this, it’s just another day.

The hardest yet most important thing we can do on a daily basis is to slip ourselves into the patient’s shoes during the toughest time of dealing with them. We understand all the ins and outs of the insurance process or their problem but they don’t and this is why they are seeking our help. We need to make sure when they leave they are satisfied with their understanding of the information they were seeking or we needed to relay to them.

We have to keep ourselves and each other motivated during the day, especially when the routine gets monotonous. Take time to talk about this concept at team huddle and about what you can do to keep each other motivated. Suggest a simple smile or quick comment to break the flow helping to keep us moving. Maybe even an office code word which makes everyone smile knowing this is the time when the robot feeling could take over. Or carry a picture of a robot in your pocket and flash it to the person (not in front of a patient) to bring a smile to their face.

Let’s keep our passion showing no matter how many times we are performing the same task. Change it up a little each day or each week to keep yourself motivated. Motivate each other to keep the office vibe upbeat, happy and compassionate.

SECOND QUARTER2018 JOURNAL QUESTIONNAIREARTHRITIS AND THE FEET Arthritis is a frequent component of complex disease processes that may involve more than 100 identifiable disorders. It is characterized by ___________________________ of the cartilage and lining of the body’s joints. If the feet seem more susceptible to arthritis than other parts of the body, it is because each foot has 33 joints which can be afflicted, and there is no way to avoid the pain of the tremendous weight-bearing load on the feet.

Arthritis may be a disabling and occasionally crippling disease; it afflicts almost 40 million Americans. In some forms, it appears to have ___________________________ tendencies. While the prevalence of arthritis increases with age, all people from infancy to middle age are potential victims. People over 50 are the primary targets.

Arthritic feet can result in loss of ______________________ and independence. But that may be avoided with early diagnosis and proper medical care,

WHAT IS ARTHRITIS? Arthritis, in general terms, is inflammation and swelling of the ___________________________ and lining of the joints, generally accompanied by an increase in the fluid in the joints. Arthritis has multiple cases; just as a sore throat may have its origin in a variety of diseases, so joint inflammation and arthritis are associated with many different illnesses.

SOME CAUSES Besides heredity, arthritic symptoms may have their source in a number of phenomena:

• They can be ________________________________, having their origins in injuries, notably in athletes and industrial workers, especially if the injuries have been ignored (which injuries of the feet tend to be).

• Bacterial and viral infections can strike the joints. The same ______________________ that are present in pneumonia, gonorrhea, staph infections, and Lyme disease cause the inflammations.

• Arthritis can develop in conjunction with bowel disorders such as colitis and ileitis, frequently in the joints of the ankles and toes. Such inflammatory

bowel diseases seem distant from arthritis, but their control can relieve arthritic pain.

• Drugs, both prescription and illegal street drugs, can induce arthritis

• Arthritis can be part of a congenital autoimmune disease syndrome, of undetermined origin. Recent research has suggested, for instance, that a defective _____________________ may play a role in osteoarthritis.

SYMPTOMS Because arthritis can affect the structure and function of the feet it is important to see a Podiatrist if any of the following symptoms occur in the feet:

• Swelling in one or more joints• Recurring pain or tenderness in any joint• Redness or heat in a joint• Limitation in motion of joint• Early morning stiffness• Skin changes, including rashes and growths

SOME FORMS OF ARTHRITIS OSTEOARTHRITIS is the most common form of arthritis. It is frequently called _______________________ joint disease or “wear and tear” arthritis. Although it can be brought on suddenly by an injury, its onset is generally gradual; aging brings on a breakdown in cartilage, and pain gets progressively more severe, although it can be relieved with rest. Dull, throbbing nighttime pain is characteristic, and it may be accompanied by muscle weakness or deterioration. Gait patterns-normal walking-may grow erratic.

It is a particular problem for the feet when people are overweight, simply because there are so many joints in each foot. The additional weight contributes to the _____________________ of cartilage and the development of bone spurs.

RHEUMATOID ARTHRITIS (RA) is a major crippling disorder, and perhaps the most serious form of arthritis. It is a complex, chronic inflammatory system of diseases, often affecting more than a dozen smaller joints during the course of the disease, frequently in a ____________________________ pattern-both ankles, or the index fingers of both hands, for example. It is often

accompanied by constitutional signs and symptoms – lengthy morning stiffness, fatigue, and weight loss – and it may affect various symptoms of the body such as the eyes, lungs, heart, and nervous system.

Women are three or four times more likely than men to suffer RA, indicating a linkage to heredity. RA has a much more _________________________________ onset than osteoarthritis. It is characterized by alternating periods of remission, during which symptoms disappear, and exacerbation, marked by the return of inflammation, stiffness, and pain. Serious joint __________________________ and loss of motion frequently result from acute rheumatoid arthritis. However, the disease system has been known to be active for months, or years, then abate, sometimes permanently.

GOUT (GOUTY ARTHRITIS) is a condition caused by a build up of the salts of _____________________- a normal byproduct of the diet-in the joints. A single big toe joint is commonly the focus, possibly because it is subject to so much pressure in walking; attacks of gouty arthritis are extremely painful, perhaps more so than any other form of arthritis. Men are much more likely to be affected than women, an indication that heredity may play a role.While a rich diet that contains lots of red meat, rich sauces, and brandy is popularly associated with gout, there are other protein compounds in such foods as lentils and beans that may play a role.

DIAGNOSIS Different forms of arthritis affect the body in different ways; many have distinct systemic affects that are not common to other forms. Early diagnosis is important to effective treatment of any form, Destruction of cartilage is not ___________________________, and if the inflammation of arthritic disease isn’t treated, both cartilage and bone can be damaged, which makes the joints increasingly difficult to move. Most forms of arthritis cannot be cured, but can be controlled or brought into remission; perhaps only five percent of the most serious cases, usually of rheumatoid arthritis, result in such severe crippling that walking aids or wheelchairs are required.

TREATMENT The objectives in the treatment of arthritis are controlling inflammation, preserving joint function (or restoring it if it has been lost), and curing the disease if that is possible.

Because the foot is such a frequent target, the Podiatrist is often the first physician to encounter some of the complaints-inflammation, pain, stiffness, excessive warmth, injuries. Even _______________________ can be manifestations of arthritis.

Arthritis may be treated in many ways. Patient education is important. Physical therapy and exercise may be indicated, accompanied by medication. In such a complex disease system, it’s no wonder that a wide variety of drugs have been used effectively to treat it; likewise, a given treatment may be vert effective in one patient and almost no help al all to another.

__________________________is still the first-line drug of choice for most forms of arthritis, and the benchmark against which the efficacy of a host of therapies is measured.

The control of foot functions with she inserts (orthoses), or with braces or specially prescribed shoes, may be indicated. Surgical interventions is a last resort in arthritis, as it is with most disease conditions; the replacement of damaged joints with artificial joints is a possible surgical procedure.
