ATTENTION, MOMS-TO-BEAsk your doctor about your Tdap shot Expectant moms may be well aware of the importance of shots to protect against infectious diseases like whoop-ing cough (pertussis), but they may not know that new recommendations advise them to have a Tdap vaccine before or during every pregnancy.

The Tdap vaccine is used to prevent three infections: tetanus, diphtheria and pertussis. Increases in cases of pertussis, a contagious respi-ratory illness, led to new recommen-dations by the Centers for Disease Control and Prevention. They call for Tdap vaccinations to be given to pregnant women, optimally between 27 and 36 weeks, or immediately postpartum. For further protection, family members and caregivers are also advised to be vaccinated with Tdap—at least two weeks before contact with the baby.

Note: Pertussis mainly affects infants and young kids and can be serious, even fatal—outbreaks have been reported in Contra Costa County schools. Newborns are especially vul-nerable since they can’t get vaccinated until two months after birth.

Staying Sober Over the HolidaysTips for those in recoveryIf you are in early recovery from an alcohol addiction, the holidays can be a challenge. “In addition to more get-togethers and more opportu-nities to abandon a healthy routine, we are faced with more family visiting, which can add stress,” says Nina Ferraris, director of John Muir Health’s Drug and Alcohol Addiction Recovery Services.

For starters, don’t feel obligated to attend every event. “People sometimes go to parties, remain abstinent but then relapse a week or two later,” Ferraris says. To avoid temptation, she advises, consider doing something different for the holidays, like going away with your spouse or sober friends during this time.

If you do attend a party, be sure to have a nonalcoholic beverage in your hand. It is wise, as well, to avoid food made with alcohol, as the taste or smell can be a trigger. And devise a strategy to manage any midparty cravings. “Some people carry a little card in their pock-et that has the 12 steps or the serenity prayer on it,” Ferraris says. “If you have a sponsor, you can use texting to have a conversation while you’re in the middle of it all.” It also can help to bring along a sponsor or another sober friend to the event. If you need someone to talk to, call the AA hotline at (925) 939-4155.

Finally, have an escape plan. “Bring your own car, even if your spouse is going,” Ferraris advises. “If you’re worried you might drink, leave early and go to a 12-step meeting. Many plan 12-step meeting at-tendance before and after the event. If you’re not comfortable disclos-ing personal information, you can always say you have a headache.”

KNOW THE WARNING SIGNS OF TEEN SUICIDE

Did you know that suicide is the third leading cause of death for young people? Dr. Yanni Rho, a board-certified child psychiatrist at John

Muir Health’s Behavioral Health Center and medical director of the Adoles-cent Outpatient Program, advises parents to watch for these warning signs:• Hints of hopelessness. Talk-

ing about feeling miserable or not being able to see his or her way out of a situation.

• Withdrawal from family, friends and school organiza-tions. “If you previously had a pretty good relationship with your child, but now he or she doesn’t want to talk, that’s a potential warning sign,” Rho says.

• Changes in behavior, such as neglecting personal appear-ance or dropping grades.

• Giving away prized possessions or other actions that indicate he or she is settling affairs or saying goodbye.

• Being preoccupied with death, either in conversation, in personal writ-ing, or in drawings or other art.

If you’re concerned, first talk to your child. “Sometimes teenag-ers are just waiting to talk with someone without feeling ashamed or judged,” says Rho. ”They want someone to care.”

It’s not always easy to tell the difference between normal teen moods and depression. “If it’s a gray area, talk to a mental health professional,” Rho advises. “You could also start the conversation with your child’s pediatrician—just somebody who can determine if there’s risk and help choose the appropriate course of action or provide reas-surance so you can put your fears to rest.”

Talk With Your Doctor

PREGNANCY AFTER 40More U.S. women in their 40s are having children, researchers say. And while there are advantages to starting a family when you’re older—and arguably wiser—40-something moms-to-be are at greater risk of complications during pregnancy and are more likely to have a cesarean delivery. That’s why getting expert ad-vice and care is important. For an overview of common concerns, check out this Q&A with Dr. Timothy Leach, an OB/GYN at John Muir Health.

What are some health risks for older women during pregnancy?Women in their 40s are more likely to develop high blood pressure, which increases risk of a low-birth-weight or preterm baby. Older women are also likelier to develop gestational diabe-tes, which, among other things, can cause the baby to grow too large, increasing the risk of injuries during delivery. So get an assessment from your OB, who can prescribe any necessary medication or care to manage the condition.

What are some other health concerns for the baby?Older moms’ babies are at higher risk of certain genetic abnormalities such as Down syndrome. John Muir Health’s experts in maternal-fetal medicine offer prenatal genetic screening, diagnosis and counseling, which are important to moms over 40.

What can women do themselves to promote a healthy pregnancy?Aim to have the healthiest weight for your height—obesity plays a role in miscarriage, gestational diabetes and other conditions. Staying up to date on immunizations— including the pertussis vaccine—is very im-portant. (See “Attention, Moms-to-Be,” page 3.) So is taking prescription prenatal vitamins, which—unlike certain trendy supplements at the grocery store—are FDA-approved.

Eat Well—and Avoid the Holiday SpreadJohn Muir Health dietary experts share tips, plus a healthful and tasty recipe

Delicious foods are part of the holiday season, but as more people are committed to health-conscious habits nowadays, it’s easier to enjoy good eats and avoid calorie creep. “People are more focused, knowledgeable and motivated than ever be-fore,” says Debbie Stevens, R.D., John Muir Health lead clinical dietitian.

One workable strategy, says Stevens, is to emphasize local seasonal produce in your holiday menus and make traditional foods the side dishes. “Make the focal point a brightly col-ored, beautiful salad that has so much eye appeal you can’t resist it,” she advises. “Fresh winter fruits and veggies, such as persimmons, dried cranberries, honeycrisp apples or firm bosc pears make side dishes more festive and incorporate seasonal favorites.”

Everyone around the table, including those with gluten-free or vegetarian diets, will welcome a selection of several veg-etable dishes, Stevens adds. A tasty option: a platter of fresh roasted veggies, such as cauliflower, string beans, asparagus and Brussels sprouts.

Orange Baked SalmonFrom Debbie Stevens, R.D., John Muir Health lead clinical dietitian

Makes 4 servings (3 ounces cooked without the skin)

Ingredients1 pound wild salmon fillet, with skin2 fresh oranges, washed, sliced thinly with peel on2 teaspoons grated orange zestFinely ground fresh pepper to taste¼ cup finely minced fresh flat-leaf (Italian) parsley1 fresh fennel bulb trimmed and cut in half vertically

Directions1. Lay a large piece of

aluminum foil in a 9-by-12-inch baking pan, with enough extra on either side to bring together on the top.

2. Lay the fennel slices and then the salmon fillet on the foil inside the pan. Sprinkle lightly with pepper to taste.

3. Sprinkle the orange zest over the fillet. Cover the surface of the fillet with orange slices and then with parsley.

4. Seal foil tightly.5. Bake at 400 F for 30 to 40 minutes, depending on the

thickness of salmon, or until salmon is cooked thor-oughly (when the flesh of the salmon turns opaque orange throughout the total thickness).

6. Cut into 4 even pieces, removing the skin as you serve. Divide orange slices among each serving.

Eat Well

More smart strategies from Stevens:

• Eat a healthy fiber-rich snack before a party. You’re less likely to overeat and should make healthier choices if you’re not too hungry.

• Beware of rich appetizers that pack fat and calories into a small package. They can put a big dent in your weight-watching efforts. Opt instead for low-fat whole-grain crackers and light feta or fresh mozzarella cheese, olives, and low-fat dips or hummus.

• Instead of prime rib, serve flavorful baked fish, pork tenderloin or tarragon shrimp.

• Keep portions small for sweet potatoes, bread stuffing or other foods that raise blood sugar.

• Bake a fresh fruit crisp instead of a traditional pie with two crusts.

• And don’t forget: Make time for physical activ-ity in all your holiday plans.

This class is designed for those considering a delivery without medication and explores cop-ing techniques that assist with labor. From breathing and positioning to relaxation, you and your partner will learn all you need to know about the special occasion of giving birth.
