Please refer to the above Request for Proposal (RFP) document in your possession and be advised of the following:
I. REVISIONS
i. The closing date has been revised from April 3, 2017 to April 5, 2017 ii. The pricing form has been revised. Bidder must use the revised price form dated March
31, 2017. Failure to use the revised price form will result in your proposal being declared non-compliant.
iii. Line Item 6 of the Price Form Should Read "Contingency (Line 5 x 10%)"

All items include all costs to perform the work including, mileage, a site report issued every day
with all material testing results from 7 am until 7 pm Monday to Saturday and a written report for
all off site tests, stockpile pick up and plant inspections. In addition the unit prices quoted must
include all necessary office support, overhead costs and disbursements. All pick-ups to be noted
by e mail or other paper receipt given to inspector. All Laboratory Test results to be submitted in
writing, after review and being stamped by a Professional Engineer for each test at no additional
cost. Frequency as per Field Services Manual and Material Specifications as contained in the
latest edition of the City of Toronto Specifications for Roadworks.
