Dear Members and delegates
This is my pleasure to welcome you to Criticare2014
– the first joint meeting of ISCCM , APACCM,
SAARC and CCNS. To my knowledge this is largest
Critical Care conference in this part of world ever held . 3000
registration mark was crossed while was writing these lines
. I , on behalf of the organizing committee and the Scientific
Program committee of Criticare2014 must express heart felt
thankfulness for your massive support . The faculty both
national and International have walked an extra mile to be
with us today . We feel overwhelmed and indebted indeed.
I sincerely wish that this occasion proves not only to be
an opportunity of education and skill feast in critical Care
Medicine , it also provides all of us a rare opportunity to
build bridges between our respective Critical Care Medicine
societies across the world The weather is at its finest best .
We will try to offer you the best of Rajasthani hospitality
and cuisine which you may never be able to forget . Our
attempt will also be to introduce to you the Rajasthani art
and culture to you all .
ISCCM is poised to take the leadership role at the world
level . ISCCM was disappointed a bit after it lost its bit at
Durban World Congress last August . However , ISCCM
looks ahead and the Criticare2016 at Agra is being an almost
a joint meeting with International sepsis forum . I wish to
congratulate Team Agra Criticare 2016 for having decided
and subsequently getting a node for holding Criticare2016
in a larger shape and size by calling it a “ World summit
“. I am sure they will get overwhelming response from the
world of Critical Care Medicine across the continents . Our
next meeting is at Bengaluru – Indian silicon Valley . I a
wish Criticare2015 at Bengaluru a grand success .
This is my last write up as the President of ISCCM . It has
been most satisfying two years . My dream of Taking
critical Care to Places has been truly achieved to a
satisfactory level . Critical Care and ISCCM turely reached
Northeast , Kashmir , Sikkim , more parts of Uttarpradesh ,
Chhatishgarh , Rajasthan , , Gujrat and more of southern
India during last two years . ISCCM has become more stable
financially and has been assured of larger office in Mumbai
. The most important milestones have been making IJCCM
monthly publication and more democratization of ISCCM.
The revised constitution now assures more role for newer
generations with wider distribution witin different parts
of India of executive positons in ISCCM . Announcements
of International and national travel grants for bright young
Intensivists in India opens new horizons for promoting
higher education and training in critical Care . All ISCCM
members have been issued membership certificates . ISCCM
brochure has been prepared for information to propective
members . Indian diploma in Critical care Nursing will
prove to be a boon for speciality in coming times . We will
able to enjoy the results well within five years from now
when this course assumes dramatic huge proportions in
terms of demand and outcome. All this could not have been
possible without unstinted support of national executive
members in particular and all members of ISCCM in general.
Criticare2014 for the first time provides opportunity to 10
young Indian talented to present their presentation skills
and they have been provided with faculty status which
entitles them to travel and accommodation being provided
by the organizing committee ..
My sincere acknowledgment of support of more than 6500
members of ISCCM , my two back to back executive members
teams , my colleague Dr Manish Munjal , The editorial board
under the leadership of Dr R K Mani . I must present a vote
of thanks for office staff of ISCCM for te good work they are
doing .I also take this opportunity to thank my wife and
children to support me in my . I am very happy to retire
from any active office of ISCCM after playing innings of
10 long years . I hope Criticare2014 really sets a new trend
of Criticare by , of, and for ISCCM members

Greetings to all new and old members of ISCCM! As I write this I am looking forward to our annual gathering at Jaipur, Criticare 2014. This will be all the more exciting as
this year we combine this meeting with Asia Pacific Congress of Critical Care Medicine. The weather will be good, venue superb and from what I know of Dr Munjal’s organizational abilities, arrangement fabulous. The scientific program appears to be a learning feast in the making. I hope all of us reach the conference in large numbers and make this conference a bigger success than the previous conferences.
At Jaipur we are going to hear from Dr Singhi about the ICUs in India. You will remember my request earlier this year, to all of you to register your ICUs and give data about your ICUs. This data has now been analyzed and Dr Singhi will be showing us what types of ICUs exist in our country.

The second talk I am looking forward to is the talk on tropical fevers by Dr Rungta. For the first time we will have insight into what type of tropical fevers are more common in India and what the outcomes are.
Since we started in September, we missed the summer and rainy season where the incidence is probably highest. Therefore, I once again a request to all of you to upload the data of patients with tropical fever syndromes in your ICU on our website.
Last but not the least, members whose contact details such as mobile numbers and e-mail addresses are not updated in our list at the head office, a humble request to you to update these details.

Dr Rajesh Chawla will serve as Chancellor of the college for 2014 -16. He was Vice Chancellor in the
previous term when various new initiatives were taken. He is one of the authors of ICU protocols book based on which our society's very own Comprehensive Critical Care Course (4C) was developed for which he is the principal coordinator.
Dr N Ramakrishnan will be Vice Chancellor of the College for 2014 -16. In his previous term (2012 -14), he was the Secretary – Education of the college. He has served as Educational Coordinator of ISCCM since 2009 during which processes for our various courses have been standardized & streamlined.
Dr J V Divatia who was the first Chancellor of the College from 2012 to 2014 will continue in the College Board from 2014 to 2016 as Immediate Past Chancellor.
Dr Palepu Gopal who was a member of the board since 2012 and subsequently designated as Joint Secretary of Education will serve as Secretary, Education from 2014 to 2016 and will coordinate all activities pertaining to our courses.
Dr Deepak Govil was a member of the Board since 2012 & subsequently designated as Joint Secretary for Accreditation. During
the period of 2014 to 2016 he will serve as the Accreditation Secretary & coordinate all processes relating to accreditation of institutions & teachers.
Dr Prakash Shastri played a significant role in starting our Critical Care Nursing Diploma. He was inducted to the board as Coordinator of Nursing Program & will serve in this role for 2014 to 2016
Dr Yatin Mehta has been a member since 2012 and will also be designated as Controller of Examinations from 2014 to 2016.
Dr G C Khilnani has served as Chairperson of Credentials Committee for nominating Fellows of Indian College of Critical Care Medicine (FICCM) and will serve as a member of the Board from 2014 to 2016.
Dr Pradeep Rangappa would be on the College board as a Joint Secretary - Education from 2014 and subsequently take over as Secretary - Education of the College.
Dr B D Bande would be Joint Secretary - Accreditation from 2014 and subsequently

Impact of lung ultrasound on clinical decision making
in critically ill patientsnektaria Xirouchaki, et al. Int Care Med, Volume 40, Issue 1 / January , 2014to assess the impact of lung ultrasound (lu) on clinical decision making in mechanically ventilated critically ill patients.one hundred and eighty-nine patients took part in this prospective study. the patients were enrolled in the study when lu was requested by the primary physician for (1) unexplained deterioration of arterial blood gases and (2) a suspected pathologic entity [pneumothorax, significant pleural effusion (including parapneumonic effusion, empyema, or hemothorax), unilateral atelectasis (lobar or total), pneumonia and diffuse interstitial syndrome (pulmonary edema)].They two hundred and fifty-three LU examinations were performed; 108 studies (42.7 %) were performed for unexplained deterioration of arterial blood gases, and 145 (57.3 %) for a suspected pathologic entity (60 for pneumothorax, 34 for significant pleural effusion, 22 for diffuse interstitial syndrome, 15 for unilateral lobar or total lung atelectasis, and 14 for pneumonia). the net reclassification index was 85.6 %, indicating that LU significantly influenced the decision-making process. The management was changed directly as a result of information provided by the LU in 119 out of 253 cases (47 %). In 81 cases, the change in patient management involved invasive
interventions (chest tube, bronchoscopy, diagnostic thoracentesis/fluid drainage, continuous venous–venous hemofiltration, abdominal decompression, tracheotomy), and in 38 cases, non-invasive (peep change/titration, recruitment maneuver, diuretics, physiotherapy, change in bed position, antibiotics initiation/change). In 53 out of 253 cases (21 %), LU revealed findings which supported diagnoses not suspected by the primary physician (7 cases of pneumothorax, 9 of significant pleural effusion, 9 of pneumonia, 16 of unilateral atelectasis, and 12 of diffuse interstitial syndrome).This study shows that LU has a significant impact on decision making and therapeutic management.

the authors here assessed the safety and the impact on in-hospital and 90-day mortality of antibiotic de-escalation in patients admitted to the ICu with severe sepsis or septic shock.
they carried out a prospective observational study enrolling patients admitted to the ICu with severe sepsis or septic shock. De-escalation was defined as discontinuation of an antimicrobial agent or change of antibiotic to one with a narrower spectrum once culture results were available. to control for confounding variables,they performed a conventional regression analysis and a propensity score (pS) adjusted-multivariable analysis.
A total of 712 patients with severe sepsis or septic shock at ICu admission were treated empirically with broad-spectrum antibiotics. Of these, 628 were evaluated (84 died before cultures were available). De-escalation was applied in 219 patients (34.9 %). By multivariate analysis, factors independently associated with in-hospital mortality were septic shock, SoFA score the day of culture results, and inadequate empirical antimicrobial therapy, whereas de-escalation therapy was a protective factor [odds-Ratio (OR) 0.58; 95 % confidence interval (CI) 0.36–0.93). Analysis of the 403 patients with adequate empirical therapy revealed that the factor associated with mortality was SoFA score on the day of culture results, whereas de-escalation therapy was a protective factor (OR 0.54; 95 % CI 0.33–0.89). the pS-adjusted logistic regression models

Distress Syndromemathieu Jozwiak, Jean-louis teboul et al.AJRCCM VOLUME 188, ISSUE 12 (DECEMBER 15, 2013)the effects of prone positioning during acute respiratory distress syndrome on all the components of cardiac function have not been investigated under protective ventilation and maximal alveolar recruitment. these authors tried to investigate the haemodynamic effects of prone positioning.they included 18 patients with acute respiratory distress syndrome ventilated with protective ventilation and an end-expiratory positive pressure titrated to a plateau pressure of 28–30 cm h2o. Before and within 20 minutes of starting prone positioning, hemodynamic, respiratory, intraabdominal pressure, and echocardiographic data were collected. Before prone positioning, preload reserve was assessed by a passive leg raising test.

Inappropriate prescribing of antibiotics by healthcare pro-fessionals is a worldwide concern. the purpose of this study was to determine the pattern of antibiotic prescrip-tion among oral healthcare providers in India.
The authors sent a one-page questionnaire to 1600 oral healthcare practitioners registered under the Indian Den-tal Association by using multistage sampling; 552 (34.5%) responded to the survey. the data were analysed using the x2 test and multiple logistic regression analysis.
They analysed 552 questionnaires. 405 (73.4%) chose amoxicillin in non-allergic patients, alone [279 (50.5%)] or associated with clavulanic acid [126 (22.8%)]. The average duration of antibiotic therapy was 4.26 ± 1.26 days. The drug of first choice for patients with an allergy to peni-cillin was erythromycin [242 (43.8%) of respondents]. A majority prescribed antibiotics for irreversible pulpitis and acute apical periodontitis [395 (71.6%)] and necrotic pulp, acute apical periodontitis and no swelling [326 (59.1%)]. Five hundred and ten (92.4%) of the oral healthcare pro-viders overprescribed antibiotics.
to conclude the oral healthcare providers in India are overprescribing, which could be a major contributor to the world problem of antimicrobial resistance. As there is overprescription of antibiotics by Indian oral healthcare providers, there is an urgent need to raise public and pro-fessional awareness regarding the risks of antibiotic use.

there is a paucity of evidence to guide the best strategy for prevention of peritoneal-dialysis-related infections. Antibacterial honey has shown promise as a novel, cheap, effective, topical prophylactic agent without inducing mi-crobial resistance. We therefore assessed whether daily application of honey at the exit site would increase the time to peritoneal-dialysis-related infections compared with standard exit-site care plus intranasal mupirocin pro-phylaxis for nasal carriers of Staphylococcus aureus.
This was a open-label trial undertaken in 26 centres in Australia and New Zealand, participants undergoing peri-toneal dialysis were randomly assigned in a 1:1 ratio with an adaptive allocation algorithm to daily topical exit-site application of antibacterial honey plus standard exit-site care or intranasal mupirocin prophylaxis (only in carriers of nasal S aureus) plus standard exit-site care (control group). The primary endpoint was time to first infection related to peritoneal dialysis (exit-site infection, tunnel in-fection, or peritonitis).

Authors evaluated the efficacy and safety of oral care with povidone-iodine on the occurrence of ventilator-associat-ed pneumonia in a high-risk population.
A multicenter, placebo-controlled, randomized, double-blind, two-parallel-group trial performed between may 2008 and may 2011 at six ICus in France.
one hundred seventy-nine severely brain-injured patients (Glasgow Coma Scale ≤ 8) or cerebral hemorrhage ex-pected to be mechanically ventilated for more than 24 hours. participants were randomly assigned to receive oropharyngeal care with povidone-iodine (n = 91) or pla-cebo (n = 88) six times daily until mechanical ventilation withdrawal.
results: primary endpoint was the rate of ventilator-associated pneumonia.
Secondary endpoint included the rates of ventilator-as-sociated tracheobronchitis and acute respiratory distress syndrome and patient’s outcome.
the number of patients evaluable for the primary end-point (preplanned modified intention-to-treat population) was 150 (78 in the povidone-iodine group, 72 in the pla-cebo group). Ventilator-associated pneumonia occurred in 24 patients (31%) in the povidone-iodine group and 20 (28%) in the placebo group (relative risk, 1.11 [95% CI, 0.67–1.82]; p = 0.69). There was no significant differ-ence between the two groups for ventilator-associated tracheobronchitis: eight patients (10%) in the povidone-iodine group and five patients (7%) in the placebo group (relative risk, 1.48 [95% CI, 0.51–4.31]; p = 0.47). Acute respiratory distress syndrome occurred in five patients in the povidone-iodine group but not in the placebo group (p = 0.06). There was no difference between groups for ICu and hospital lengths of stay, as well as ICu and 90-day mortality.
So the authors conclude that there is no evidence to rec-ommend oral care with povidone-iodine to prevent venti-lator-associated pneumonia in high-risk patients. Further-more, this strategy seems to increase the rate of acute respiratory distress syndrome.
Interventions to reduce colonisation and transmission of
antimicrobial-resistant bacteria in intensive care units: an interrupted time series study and cluster

Intensive care units (ICus) are high-risk areas for trans-mission of antimicrobial-resistant bacteria, but no con-trolled study has tested the effect of rapid screening and isolation of carriers on transmission in settings with best-standard precautions. the authors assessed interventions to reduce colonisation and transmission of antimicrobial-resistant bacteria in european ICus.
They did this study in three phases at 13 ICUs. After a 6 month baseline period (phase 1), they did an interrupted time series study of universal chlorhexidine body-washing combined with hand hygiene improvement for 6 months (phase 2), followed by a 12—15 month cluster randomised trial (phase 3). ICus were randomly assigned by computer generated randomisation schedule to either conventional screening (chromogenic screening for meticillin-resistant Staphylococcus aureus [mRSA] and vancomycin-resistant enterococci [VRe]) or rapid screening (pCR testing for mRSA and VRe and chromogenic screening for highly re-sistant enterobacteriaceae [hRe]); with contact precau-tions for identified carriers.
the primary outcome was acquisition of resistant bacteria per 100 patient-days at risk, for which they calculated step changes and changes in trends after the introduction of each intervention. they assessed acquisition by microbio-logical surveillance and analysed it with a multilevel pois-son segmented regression model. they compared screen-ing groups with a likelihood ratio test that combined step changes and changes to trend.
Seven ICus were assigned to rapid screening and six to conventional screening. mean hand hygiene compliance improved from 52% in phase 1 to 69% in phase 2, and 77% in phase 3. Median proportions of patients receiv-ing chlorhexidine body-washing increased from 0% to 100% at the start of phase 2. For trends in acquisition of antimicrobial-resistant bacteria, weekly incidence rate ratio (IRR) was 0•976 (0•954—0•999) for phase 2 and 1•015 (0•998—1•032) for phase 3. For step changes, weekly IRR was 0•955 (0•676—1•348) for phase 2 and 0•634 (0•349—1•153) for phase 3. The decrease in trend in phase 2 was largely caused by changes in acquisition of MRSA (weekly IRR 0•925, 95% CI 0•890—0•962). Acquisi-tion was lower in the conventional screening group than in the rapid screening group, but did not differ significantly (p=0•06).
Improved hand hygiene plus unit-wide chlorhexidine body-washing reduced acquisition of antimicrobial-re-sistant bacteria, particularly mRSA. In the context of a sustained high level of compliance to hand hygiene and chlorhexidine bathings, screening and isolation of carri-ers do not reduce acquisition rates of multidrug-resistant bacteria, whether or not screening is done with rapid test-ing or conventional testing.
confirmed that de-escalation therapy was a protective factor in both analyses. De-escalation therapy was also a protective factor for 90-day mortality.
De-escalation therapy for severe sepsis and septic shock is a safe strategy associated with a lower mortality. efforts to increase the frequency of this strategy are fully justified.
In all patients, prone positioning increased the ratio of arterial oxygen partial pressure over inspired oxygen fraction, the intraabdominal pressure, and the right and left cardiac preload. the pulmonary vascular resistance decreased along with the ratio of the right/left ventricular end-diastolic areas suggesting a decrease of the right ventricular afterload. In the nine patients with preload reserve, prone positioning significantly increased cardiac index (3.0 [2.3–3.5] to 3.6 [3.2–4.4] L/min/m2). In the remaining patients, cardiac index did not change despite a significant decrease in the pulmonary vascular resistance.In conclusion, patients with acute respiratory distress syndrome under protective ventilation and maximal alveolar recruitment, prone positioning increased the cardiac index only in patients with preload reserve, emphasizing the important role of preload in the hemodynamic effects of prone positioning.
Of 371 participants, 186 were assigned to the honey group and 185 to the control group. The median perito-neal-dialysis-related infection-free survival times were not significantly different in the honey (16•0 months [IQR not estimable]) and control groups (17•7 months [not esti-mable]; unadjusted hazard ratio 1•12, 95% CI 0•83—1•51; p=0•47). In the subgroup analyses, honey increased the risks of both the primary endpoint (1•85, 1•05—3•24; p=0•03) and peritonitis (2•25, 1•16—4•36) in participants with diabetes. the incidences of serious adverse events (298 vs 327, respectively; p=0•1) and deaths (14 vs 18, respectively; p=0•9) were not significantly different in the honey and control groups. 11 (6%) participants in the hon-ey group had local skin reactions.
The findings of this trial show that honey cannot be rec-ommended routinely for the prevention of peritoneal-dialysis-related infections.
