Critical care in india is growing fast and spreading its wings across the
length and breath of india which is our aim, taking critical care to places !
The preparation for the mumbai silver jubilee conference is in full swing
and no stone is been left unturned !
This conference has a excellent scientific program and involves the best
critical care faculty in the country and the world !
Dr. Vincent JL, Dr. Marin Kollef, Dr. Mitch Levy, Dr. L Brochard,
Dr. D. Cheng, Dr. Yasin Arabi, Dr. V. Rosenthal and many more have been
invited as international stars !
I’m sure the scientific content will benefit all students and fellow colleagues,
and will make them updated with what’s new in the field of critical care !
I’m happy to see good articles coming up in CCN and urge all to send
more material for the newsletter!
Various Zonal meetings were held in different parts of the country and
these conferences were very well attended and appreciated!

President's Desk...
Happy Ashtami to All!
A lot has been happening for the big days in Feb 2019! We have finalized
the scientific programme, including the workshops which look very good and
also tentatively the social programme which I shall not elaborate as it will be
a pleasant surprise!
Rest of the preparations are on full swing. We had a good response to
ISCCM day the theme of which was ‘Hand Hygiene’ and we gave prizes
to the best poster and slogan. Last Saturday we also had a webinar. We
had 6 speakers followed by a panel discussion which had very enthusiastic
participant’s response (almost 500).
I hope that we were able to generate awareness about the significance of
Hand Hygiene. INDICAPS second data collection day is nearing so I urge all
centres to enter the data 100%.
I wish you all a happy Dussehra and Diwali. Long Live ISCCM.

Dear All ISCCM Members,
A very very happy and prosperous Deepawali to you all. We are in the last quarter of this year, and we are approaching our Silver Jubilee Conference very fast, preparations are in full swing. Recently we finished our ISCCM Day celebration. Poster participation was very good and it was beyond our expectations. Webinar was viewed by a huge number of delegates, as per the feedbacks received from the office. Nearly forty branches have started working through their Branch Management System (BMS), which is nearly fifty percent of all branches, I hope in next couple of months rest all will start working with their BMS too. As we are in the process of writing the Zonal constitution, controversies related to paper presentations of IFCCM Candidates will be dealt on one to one basis. From December our IJCCM journal will be taken over by new publisher Jaypee Brothers. Scientific program for the Silver Jubilee conference is now complete and it will be visible on website very soon. Again, I request for your whole-hearted participation in CRITICARE 2019 to make it a huge success.

ISCCM foundation day is celebrated on 9th of October. We celebrated ISCCM Day this year with theme “Clean hands – safe hands, practice hand hygiene. The intention is to create a wide spread awareness amongst Doctors and community.
ISCCM Day poster/Slogan competition was conducted. Members enthusiastically participated in Poster/ Slogan Competition. 53 posters were received. The results have been put up on website. We thank all the participants and congratulate the winners. The winners will be given their prize and certificate at Annual General Meeting to be held during our Silver Jubilee conference – Criticare 2019, Mumbai. Abstract
Background The efficacy of venovenous extracorporeal membrane oxygenation (ECMO) in patients with severe acute respiratory distress syndrome (ARDS) remains controversial.
Methods In an international clinical trial, we randomly assigned patients with very severe ARDS, as indicated by one of three criteria — a ratio of partial pressure of arterial oxygen (Pao2) to the fraction of inspired oxygen (Fio2) of less than 50 mm Hg for more than 3 hours; a Pao2:Fio2 of less than 80 mm Hg for more than 6 hours; or an arterial blood pH of less than 7.25 with a partial pressure of arterial carbon dioxide of at least 60 mm Hg for more than 6 hours — to receive immediate venovenous ECMO (ECMO group) or continued conventional treatment (control group). Crossover to ECMO was possible for patients in the control group who had refractory hypoxemia. The primary end point was mortality at 60 days.
RESULTS At 60 days, 44 of 124 patients (35%) in the ECMO group and 57 of 125 (46%) in the control group had died (relative risk, 0.76; 95% confidence interval [CI], 0.55 to 1.04; P=0.09). Crossover to ECMO occurred a mean (±SD) of 6.5±9.7 days after randomization in 35 patients (28%) in the control group, with 20 of these patients (57%) dying. The frequency of complications did not differ significantly between groups, except that there were more bleeding events leading to transfusion in the ECMO group than in the control group (in 46% vs. 28% of patients; absolute risk difference, 18 percentage points; 95% CI, 6 to 30) as well as more cases of severe thrombocytopenia (in 27% vs. 16%; absolute risk difference, 11 percentage points; 95% CI, 0 to 21)
and fewer cases of ischemic stroke (in no patients vs. 5%; absolute risk difference, −5 percentage points; 95% CI, −10 to −2).
Conclusions Among patients with very severe ARDS, 60-day mortality was not significantly lower with ECMO than with a strategy of conventional mechanical ventilation that included ECMO as rescue therapy.

Strengths of the Study
• Relevantresearchquestionaboutthe effectiveness of ECMO in severe ARDS was addressed in this study.
• It was the Largest multicenterECMO RCT to date.
• ECMO for severe ARDS showedno significant benefit of mortality at day 60 as compared with a strategy of conventional mechanical ventilation, which included crossover to ECMO (used by 28% of the patients in the control group).
• Almost all patients who wererandomized to ECMO received it (unlike in CESAR).
Weaknesses of the Study
• Underpowered to answer thetrial question as the Trial was stopped early at 249/331 (75% of recruitment) due to predefined futility rules (ie unlikely to get a definitive result). A 20% absolute risk reduction sets a very large requirement for a single ICU intervention, increasing the risk of a falsely negative trial (CESAR, published in 2009, had a ARR of 16%).
• Highcrossoverrateofcontrolsi.e.a 28% cross over rate resulted in a reduction of separation between the two arms, and diluted the ECMO treatment effect. The cross over complicates the interpretation of the two arms in an intention to treat analysis. It also introduces a potential bias against the ECMO group in the secondary risk of treatment failure analysis, as ECMO was initiated much later and in sicker patients than the rest of the controls.
• The trial does not distinguishbetween the use of VV ECMO and
VA ECMO. In particular, 7 cross over control patients underwent VA ECMO for cardiac arrest.
• Some inclusion criteria mayhave been too liberal (e.g. PaO2 <80mmHg for 6 hours), which may dilute the benefit of VV ECMO and expose patients who might otherwise have lived to the complications of ECMO.
Reviewer’s Discussion
• The EOLIA trial providesinconclusive support for the benefit of ECMO in severe ARDS.
The early initiation of ECMO did not improve 60 day mortality (with ARR 20%) compared to standard care in patients.
However the data also suggest a possible clinical benefit from ECMO over standard care, especially if used early rather than late.
• Standard therapy, includingproning and NMB, had a high rate of failure requiring rescue with ECMO.

Abstract
Background Both balanced crystalloids and saline are used for intravenous fluid administration in critically ill adults, but it is not known which results in better clinical outcomes.
Methods In a pragmatic, cluster-randomized, multiple-crossover trial conducted in five inten- sive care units at an academic center, we assigned 15,802 adults to receive saline (0.9% sodium chloride) or balanced crystalloids (lactated Ringer’s solution or Plasma-Lyte A) according to the randomization of the unit to which they were admitted. The primary outcome was a major adverse kidney event within 30 days — a composite of death from any cause, new renal-replacement therapy, or persis- tent renal dysfunction (defined as an elevation of the creatinine level to

≥200% of baseline) — all censored at hospital discharge or 30 days, whichever occurred first.
Results Among the 7942 patients in the balanced-crystalloids group, 1139 (14.3%) had a major adverse kidney event, as compared with 1211 of 7860 patients (15.4%) in the saline group (marginal odds ratio, 0.91; 95% confidence interval [CI], 0.84 to 0.99; conditional odds ratio, 0.90; 95% CI, 0.82 to 0.99; P=0.04). In-hospital mortality at 30 days was 10.3% in the balanced-crystalloids group and 11.1% in the saline group (P = 0.06). The incidence of new renal-replacement therapy was 2.5% and 2.9%, respectively (P = 0.08), and the incidence of persistent renal dysfunction was 6.4% and 6.6%, respectively (P=0.60).
Conclusions Among critically ill adults, the use of balanced crystalloids for intravenous fluid administration resulted in a lower rate of the composite outcome of death from any cause, new renal-replacement therapy, or persistent renal dysfunction than the use of saline.
Reviewer’s Comments :
Strengths of the study :
• An important clinical question with sound pathophysiological and experimental basis.
• Large sample size provides power to detect small differences in clinical outcomes.
• A mixed cohort of medical and surgical patients making the result suitable for extrapolation to any critically ill patient.
• Separation between groups of fluids received indicating that the allocation was largely successful.
Weaknesses of the study :
• Patients remaining in the ICU at the end of a calendar month may have been exposed to both types of crystalloids.
• The composite outcome ‘MAKE 30’ consisted of mortality, new renal-replacement therapy or persistent renal dysfunction (creatinine 2x baseline). As against this trial, The ELAIN follow-up study, had defined persistent renal dysfunction as a decline in eGFR by 25% compared with baseline.
• Physicians were not blinded. This may introduce an element of conscious or unconscious bias.
• The trial does not tell us if Plasmalyte-A or Lactated Ringer’s is better, only that they are likely to be superior to saline in this study.
• The predicted incidence of the outcome was 15%. The result may be more obvious if a sicker
cohort of patients was studied. The subgroup of those with sepsis demonstrated a mortality of 25.2% in the balanced crystalloid group and 29.4% in the saline group (adjusted OR 0.80; 95% CI 0.67-0.97, p=0.02).
• The patients received relatively small volumes of fluid: median 1000ml for balanced-crystalloid group and 1020ml for the saline group. The difference in the primary outcome was greater in a pre-specified subgroup who received larger volumes of crystalloid.
Reviewer’s Discussion
• This large and strong methodological study adds further doubt to the safety of normal saline use in critically ill patients. My choice in sick patients is to use balanced solutions since this trial favours administering intravenous balanced crystalloids over saline to decrease a composite outcome of death, new renal replacement therapy or persistent renal dysfunction at 30 days.
• The PLUS trial results are keenly awaited as this has a solid clinically meaningful outcome (90 day mortality) and the cohort of patients has a higher baseline mortality so the effect may be more obvious.

in critical care .It suggests ever increasing role of ultrasound in modern medicine especially critical care, where the time to intervention is the only determinant of life and death and ultrasound has become an integral part of decisions making.
The scope of book encompasses wide array of indications in which ultrasound plays a major role. The preface mentions the specialties to whom it would be beneficial and plays emphasis on basics of ultrasound for the beginners.

The table of content is well organized and begins with general descriptions about POCUS, general applications in critical care medicine- giving a primer to the reader .It quickly evolves from guidelines about the usage and ultrasound physics to specifics of each organ systems starting from airway ,chest, cardiac ,diaphragm, muscular dysfunction and ends with clinical case scenarios and legal issues .This is much helpful for the students who are at the early stages of there carrier and wish to read about the role of ultrasound in intensive care. The book also throws light on topics, which are routinely not covered in other books such as infection and legal dilemma, ultrasound in muscle dysfunction, procedural ultrasound and role of ultrasound in cardiopulmonary resuscitation. Though this book is meant for beginners but it provides enough material for advance providers who wants to touch base as well as look into the future applications.
The individual chapters are informative and written in simple English .The concepts are well defined, clear convincing and the information is quite precise and accurate. However, the new field of speckle ultrasound and contrast ultrasound is untouched and I hope the author would look into these new fields in their next edition.
The images are clear and well illustrated. Chapters provide enough recent references for further read for the readers. The author list is full of academic practitioners who are actively engage in patient care, teaching and research.
I would recommend the book as ready reckoners for day-to-day critical care practitioners.

Healthcare-associated infections (HAIs, also termed “hospital-acquired infections” or “nosocomial infections”) are
infections that occur during the process of care in a healthcare facility, most commonly a hospital. They are an important
quality indicator for healthcare institutions – conceptually, one would not really wish to be treated at a hospital where
one’s risk of acquiring an infection is high, in the same way one would try to avoid eating at restaurants with a high rate of
gastroenteritis outbreaks. There is an important caveat here – HAI rates will vary according to the case mix of the hospital.
A healthcare institution that handles more complex (i.e. more ill) cases and a higher proportion of emergencies will have
a higher rate of HAI compared to one that primarily deals with obstetrics or elective orthopaedic operations, for example.
Therefore one has to be careful making any inter-hospital comparisons.
Impact- Healthcare-associated infections (HAI) are associated with increased morbidity, mortality and associated
financial burden.
HAI in the neonatal intensive care units (NICUs) are affected by many factors like infection control practices, endemic
microbial flora, invasive procedures, parenteral nutrition and antibiotic stewardship policies.2 Environmental factors,
hospital policies and unit epidemiology also play an important role.2 Neonatal infections remains a huge challenge for
pediatricians, infection control teams, quality managers and administrators. Nosocomial infections increase the cost of
neonatal intensive care, prolong hospital stay and are responsible for almost half of the deaths that occur beyond two
weeks of age.3 Emergence of resistant strains are another unwanted consequence of antibiotic selection pressure. Blood
stream infection (BSI) is considered a serious hazard because of their frequency (59%) amongst HAI and potential life-
threatening consequences.4 Aetiological agents vary and include antibiotic resistant bacteria, particularlyStaphylococcus
aureus, Gram negative bacilli and enterococci, viruses (which account for up to 20% of cases), and fungi. n addition,
multidrug-resistant (MDR) pathogens are evermore frequently isolated in ICUs [5],[6] and this hinders the initiation of
appropriate, effective antibiotic therapy, which correlates with excess mortality the increasing number of antimicrobial
agent-resistant pathogens and high-risk patients in hospitals are challenges to progress in preventing and controlling these
infections.
The active involvement and cooperation of the microbiology laboratory are important to the infection control program,
particularly in surveillance and the use of laboratory services for epidemiologic purposes. Surveillance is used to identify
possible infection problems, monitor infection trends, and assess the quality of care in the hospital. It requires high-quality
laboratory data that are timely and easily accessible.
Surveillance is a basic and effective strategy to gauge and contain HAI. Monitoring infections is increasingly regarded
as an important parameter for safety and quality of healthcare delivery.5 For many countries including India, the magnitude
of HAI’s are not exactly known or is grossly underestimated.6 This is mainly because HAI’s diagnosis is complex and
surveillance activities to guide interventions require expertise and resources.
Nosocomial infections are a big threat to modern health care. Hospital acquired infection incidence and attack rates
remain high in India. Policy making and implementation of infection control has many hurdles. Effective implementation of
infection control practices is crucial to reducing the transmission of hospital acquired infections.
Infection Control program is considered efficient if it is stringently implemented according to resources and context.
The strategies need to be individualised according to type of infection, disease burden, space, resources, attack rate, HCP
availability and many other variables. We often have to choose different techniques depending upon these variables. The
components and the whole policy per se should be easy to follow with good penetration at the grassroots level.
Effective surveillance of HAIs helps in quality improvement by identifying a problem, suggesting an intervention and
documenting the effectiveness of that intervention and thus directing future infection control practices. The risk to acquire
HAI is universal and pervades every healthcare facility and health system worldwide with some high-income countries
having a national surveillance system for HAIs. However, the true burden remains unknown in many nations, particularly

devices and antimicrobials in a large tertiary care hospital in India reported overall prevalence of HAIs as seven per cent5.
Another study carried out in north India in a burn unit of a tertiary care referral centre reported the infection density being
36.2 infections per 1000 patient-days6
Most of our current infectious outbreaks occur by person to-person transmission. Smaller outbreaks of MDRO related
infections are common. Because of the potential for healthcare-associated outbreaks, hospitals have developed policies
to prevent the spread of MDROs,
Role of fast diagnostics for screening to prevent transmission of infection?
Infection is a common complication seen in ICU patients. Given the correlation between infection and mortality in
these patients, a rapid etiological diagnosis and the determination of antimicrobial resistance markers are of paramount
importance, especially in view of today's globally spread of multi drug resistance microorganisms.

and costly HAI.
Prevention
Attention to simple preventive strategies may significantly reduce disease transmission rates. Frequent hand washing
remains the single most important intervention in infection control. However, identifying mechanisms to ensure compliance
by health professionals remains a perplexing problem. Gloves, gowns, and masks have a role in preventing infections, but
are often used inappropriately, increasing service costs unnecessarily.
Infection control programmes are cost-effective,1 2 but their implementation is often hindered by a lack of support
from administrators and poor compliance by doctors, nurses, and other health workers. Some health professionals suffer
from the “Omo syndrome”—a belief that they are always super clean and sterile. Many are visibly upset when their poor
hygiene practices are exposed and are offended when it is suggested that they may be potential vectors of disease and
are spreading virulent microorganisms among their patients. The hands of staff are the commonest vehicles by which
microorganisms are transmitted between patients.5 Hand washing is accepted as the single most important measure in
infection control. Hand washing using a spray can be accomplished in 20 seconds, compared with 40–80 seconds for
soap. Continued monitoring and educational efforts can improve hand washing habits.
Conclusions
Nosocomial infections are worth preventing in terms of benefits in morbidity, mortality, duration of hospital stay, and
cost. Educational interventions promoting good hygiene and aseptic techniques have generally proved to be successful,
but these practices are often not sustainable. Greater efforts are being made in some countries to ensure the application
of the infection control evidence base into practice. In the end, constant vigilance and attention by the individual to what
are rather simple measures is demanded.

We had one invited speaker, Dr. Bijayini Behera who spoke on preventive aspect of infection
control.
It was followed by a scintillating quiz by Dr. Alok Sahoo and Dr. Seagata tripathy.
There was a cake cutting by senior members. It was attended by 57 doctors.

The scientific committee is also burning midnight oil under the stewardship of Dr. Subhal Dixit and other colleagues to have a fantastic content, different from
the last few years. More than 20 renowned International and 100 National faculty have already confirmed their participation. We have 8 simultaneous
sessions and similar number of workshops. This time we have given a choice to delegates (students) to register only for the workshops if they wish. This will
increase the participation without straining the facilities of the venue. I would urge all to register early, as we may have to close the registrations once they
reach a threshold level.

ISCCM has certainly brought critical care into limelight in our country. Now we have to think how we can go to the next level, it is going to be more challenging; of course the leadership will have to take this forward. With every celebration our accountability increases. We are vibrant society with a motto of growing collectively. Hence, we seek a contribution in scientific feast from all corners of INDIA and world. Younger generation is our future, we believe in creating an easier pathway for them. non-profit association of Indian Physicians, Nurses, Physiotherapists and other allied health care professionals
involved in the care of the critically ill. ISCCM which was started with a small group of consultants, from Mumbai, is
now having membership of 10131, comprising of 76 city branches all across the India with headquarter at Mumbai.
ISCCM is committed for the promotion and advancement of intensive care as a specialty in India by facilitating
education and training of physicians and nurses, setting best practice standards for the care of critically ill patients
and their families and promoting research. The ultimate aim is to raise the level of critical care practice in the country

About ISCCM
Indian Society of Critical Care Medicine was established on 9th October, 1993, in Mumbai, India. It is the largest
non-profit association of Indian Physicians, Nurses, Physiotherapists and other allied health care professionals
involved in the care of the critically ill. ISCCM which was started with a small group of consultants, from Mumbai, is
now having membership of 10131, comprising of 76 city branches all across the India with headquarter at Mumbai.
ISCCM is committed for the promotion and advancement of intensive care as a specialty in India by facilitating
education and training of physicians and nurses, setting best practice standards for the care of critically ill patients
and their families and promoting research. The ultimate aim is to raise the level of critical care practice in the country
and to develop future leaders in the field of critical care.

Abstract Submission Guidelines
a) First deadline of abstract submission 30th October 2018.
b) Second deadline for intimate delegates about their selection of abstract & to complete registration process will be 30th November 2018.
c) Third deadline for submission of top 5 full presentations of abstracts selected for HN will be 15th December 2018.
d) Fourth deadline for Submission of full & final selected presentations will be 30th December 2018.
It will be irrespective of oral or poster.
No abstract will be entertained after 30th December for selection, submission or presentation.
Presenting author shall be notified of the status of the abstract after being reviewed by the committee, by email. Authors selected for oral & poster presentation will
be informed. Communication will only be made by email only on this regard. Hence including email address along with abstract is mandatory.
Registration for conference is not mandatory for abstract submission. However if abstract or paper gets selected, then only registration for the Conference is
mandatory.
Abstract must be typed and presented in English
Use font type Times New Roman, Font Size 10
The abstract must be written within 3000 words. Excluding title, and the names of the author (s) and their institutions
The submitted abstracts must contain the following:
Objectives: State the main objective of the study
Materials and methods: Briefly describe the design of the study and how it was conducted
Results: Present the main results with appropriate statistics
Conclusion: Limit the conclusions to those that are directly supported by the results.
The title of the abstract must be typed in CAPITAL LETTERS with the exception of scientific names, which should be in upper and lower case forms and italicized.
The presenting author's name must be underlined
The presenting author's name, institution, address, telephone and email address must be provided.
Presenting authors must be registered for the conference. Only Registered delegates will be allowed to present.
Language
Your abstract will be scored on the following criteria:
» Scientific merit of the abstract.
» Adequate sample size and proper statistical analysis.
» Adherence to instructions, and.
» Originality of work
