It is a blessing to have good health and access to quality health care. It is also a blessing to help those in need. Some of us help patients directly with medical skills and expertise, and some of us are employed within the health care industry or adjacent fields. So many of us are blessed to help others with our gifts of time, money and other resources. The generosity of our local communities is strong.

Together, the CHI Saint Joseph Health Foundations put humankindness into action. We acknowledge and respect that every person is worthy of care and compassion. In the spirit of our founders, the Sisters of Charity of Nazareth, the CHI Saint Joseph Health Foundations are honored to provide help for everyone we can.

The foundations help people prevent cancer with timely screenings; we give patients the resources they need when cancer can’t be stopped. Our foundations provide help and hope, services and supplies, compassion and humankindness, every single day. It is never too late to donate money, volunteer time and energy, and offer smiles and prayers, because cancer never takes a day off. Cancer is no respecter of social or economic status.

As you’ll see in this report, supporters like you rallied around the foundations to help people in the communities we serve. For example, we raised nearly $370,000 through Evening of Hope. The outpouring of support was heartwarming to witness, as so many community members took part in the record-breaking silent auction. Evening of Hope directly benefits patients being treated for cancer across CHI Saint Joseph Health, with continued improvement in technology and oncology resources.

Toyota Motor Manufacturing Kentucky, TMMK, worked two years to create a special Camry for surgery rehab patients at Saint Joseph Hospital. This in-kind donation is helping mobility remain a reality as patients practice getting in and out of a car. We believe this is the first time Toyota worldwide has taken on such a project.

Yes, Cerv! is a new program providing cervical screenings and treatment through CHI Saint Joseph Health Partners, a clinically integrated network of more than 700 providers across Kentucky, funded by the Kentucky Women's Cancer Screening Program through the Centers for Disease Control and Prevention’s National Breast and Cervical Early Detection Program. Yes, Cerv! is a complement to Yes, Mamm! that launched in 2012, which provides breast cancer services for underinsured and uninsured women and men. Our team is also honored to coordinate volunteers within the hospitals. They provide an invaluable ministry to patients. In 2020 our volunteers contributed 40,143 hours of service valued at $1.146 million to CHI Saint Joseph Health.

Supporters gathered at 50 small parties across the state as part of our first-ever Evening of Hope event. Here, guests gathered at the home of Jon Carloftis in Lexington.

Evening of Hope

Instead of hosting nearly 700 attendees in one room for a single fundraising gala, a first-time event, Evening of Hope, involved 50 small gatherings made possible by 42 sponsors for our supporters, all united for the cause of providing cancer care resources in the communities we serve. The annual fundraiser for the CHI Saint Joseph Health Foundations was spread across several counties, and gifts totaled nearly $370,000 from the generous support of donors. This support helps individuals in our communities in direct ways, through cancer care support programs such as Yes, Mamm! Yes, Cerv!

Fighting cancer is expensive. This is why CHI Saint Joseph Health Foundations work every day to relieve some of that financial burden so patients can focus on their healing. This is a tangible way we put humankindness into action.

This year alone, Kentucky is estimated to have more than 30,000 new cases of cancer and more than 10,000 cancer deaths, which is the

highest rate of cancer deaths in the country. Since 1980, CHI Saint Joseph Health has led the way in cancer treatment and diagnosis throughout central and eastern Kentucky, providing oncology care and expertise for more than 73,000 patient visits every year. Through our affiliation with the Cleveland Clinic Cancer Center, Kentucky patients now have access to second opinions and breakthrough treatments from a top 5 cancer center.

The critical dollars raised through Evening of Hope supports cancer care resources in Bardstown, Berea, Lexington, London and Mount Sterling. These funds directly benefit patients across all of CHI Saint Joseph Health, offering them hope for their healing journey.

In the spirit of our founders, the Sisters of Charity of Nazareth, we thank you for your continued support and gifts that allow us to continue to help every person, regardless of their ability to pay for cancer care.

Barbara Ermers, now cancer free, and her husband Gary

A grateful family

In 2016, Barbara Ermers of Lexington started feeling sluggish, and had aches and pains that she attributed to getting older. “I just didn’t listen to my body,” she admitted. At her annual physical exam, blood tests and an ultrasound revealed stage 4 metastatic melanoma. It had invaded her internal organs, lymph nodes and spine. This was two years after she had had a malignant mole removed from her arm.

Jessica Croley, MD, a medical oncologist with CHI Saint Joseph Health - Cancer Care Center in Lexington, was connecting Barbara with a clinical trial at Vanderbilt until a CT scan revealed that Barbara’s cancer was growing at an alarming rate and there was no time for a clinical trial. Dr. Croley started Barbara immediately on two advanced immunotherapy medications and radiation, and used high-tech genomics research and other treatments specific to Barbara’s DNA. In three months, despite some serious side effects, the cancer was gone. She has been off therapy since then and is disease-free.

“To God goes all the glory, but he sure did use Dr. Croley.” Her husband added, “And the whole team.” Gary Ermers is CHI Saint Joseph Health’s market director of business development.

“There is always that window of hope, especially now with newer technologies like immunotherapy and targeted therapy,” said Dr. Croley. “We're able to give patients that glimmer of hope.”

The Total Health Roadmap program screens patients for social determinants of health (basic needs) in the same office as their primary care physician, pediatric office or high-risk OB-GYN practice.

Connecting patients to community resources

Food availability, safe housing, quality child care, transportation, access to behavioral health services and social support—these physical, psychological and social human conditions are part of a person’s total health. An initiative called Total Health Roadmap was funded in partnership initially from the Robert Wood Johnson Foundation and CHI Mission and Ministry to the CHI Saint Joseph Health Foundations. Today, CHI Mission and Ministry supports the program.

Food insecurity is a leading cause of poor health outcomes for patients across central and eastern Kentucky. CHI Saint Joseph Health community health workers provided nearly 5,000 pounds of food to at-risk households this fiscal year. Through a partnership with God’s Pantry Food Bank, community health workers provided food-insecure patients with a supply box of emergency food.

Outreach is sometimes as close as the doctor’s

office, through integrating a universal screening tool by a community health worker. The Total Health Roadmap program screens patients for social determinants of health (basic needs) in the same office as their primary care physician, pediatric office or high-risk OB-GYN practice. In 2019, three primary care clinics conducted approximately 9,000 screenings that focused on basic needs.. Of those, nearly 2,500 patients screened positive for unmet needs. This initiative was led by Neva Francis, vice president of healthy communities, and Janell Samuels, CHI Saint Joseph Health Foundations' director of proposal development.

Saint Joseph Hospital Foundation, Saint Joseph Berea Foundation and Saint Joseph London Foundation received $945,740 in grant funding for in the continuation of the Total Health Roadmap Initiative for 2020-2023.

Violence prevention is another key social determinant of health. The violence prevention program at CHI Saint Joseph Health is funded through a grant from the CHI Mission and Ministry Fund, facilitated through the Saint Joseph Hospital Foundation.

Joyce’s story

The news that she had breast cancer was just sinking in

when Joyce Gray started to fear an even bigger battle: paying her household bills while she was undergoing

treatment.

Gray, 53, would have to take a leave from her job for six

months while she underwent a lumpectomy, radiation and

chemotherapy.

“It was just devastating,” she said. “The biggest thing was my bills. I was just worried; how am I going to pay all

those?”

Then Gray received a call that she said was God answering one of her biggest prayers. A

CHI Saint Joseph Health navigator asked if she’d like to sign up for financial support

through the Yes, Mamm! program.

“With one phone call, everything changed,” Gray

said. “All I could say was, ‘Oh Lord God, thank you.’”

Gray received support to cover her bills, erasing all her fears.

She said her faith in God, family, church, friends,

physicians and colleagues helped her through one of the

hardest times of her life.

Preventing and treating cancers

Funded by the Saint Joseph Hospital Foundation, the Yes, Mamm! program has served 2,850 uninsured and underinsured patients with more than 6,600 breast care services and supplies since 2012. Yes, Mamm! provides free mammography screenings, diagnostic mammograms, ultrasounds, X-rays and genetic testing, and supplies such as lymphedema garments and wigs.

Nonmedical services that go hand-in-hand with healing include lodging, transportation assistance, patient counseling and support groups. It is our hope at the foundations that no patient should have to choose between paying for gas to get to the hospital or paying a medical bill. We help alleviate this financial stress so they can get the care they need.

The Yes, Cerv! program started in 2020 to help prevent cervical cancer. Kentucky has some of the highest rates for cervical cancer, and Yes, Cerv! advocates Pap smears and HPV vaccinations. Yes, Cerv! provides free cervical cancer screenings, early-stage diagnoses, and oncological pelvic treatment to eligible women who are uninsured and underinsured.

Yes, Cerv! is made possible through CHI Saint Joseph Health Partners, our clinically integrated network of more than 700 providers across Kentucky. Leadership for the Yes, Mamm! Yes, Cerv! initiative was provided by Kelly Toponak, RN, BSN, director of Women’s and Children’s Services and Breast Care; Dorothy Lockhart, vice president of CHI Saint Joseph Health Partners; and Janell Samuels, CHI Saint Joseph Health Foundations’ market director of proposal development.

Thanks to Toyota Motor Manufacturing Kentucky, Saint Joseph Hospital has a Toyota Camry on the fifth floor to help patients in the rehabilitation department.

Volunteers are called to serve

They give of their time and talents, expecting nothing in return. They give because they are called to do so. They treat others the way they would like to be treated. They are volunteers, and health care settings are all the better for these people. Volunteers throughout CHI Saint Joseph Health made and donated masks, pocket prayer quilts, and goodie bags for staff and patients. We welcomed them back into buildings on April 19, after 405 days without their in-person support. Our teen volunteers returned on June 14.

Flaget Memorial Hospital was featured on the cover of the June 1, 2021, issue of Catholic Health World. Rita Carter, a volunteer at Flaget Memorial Hospital, shared her story.

Vehicles for change

What happens when patients are released from the hospital? The first thing they find themselves doing is getting into a car to go home. For those who have had total joint replacements and other surgeries, this task is more challenging.

Thanks to the Saint Joseph Hospital Foundation’s partnership with Toyota Motor Manufacturing Kentucky (TMMK), patients in the rehabilitation department at Saint Joseph Hospital can practice getting in and out of a Toyota Camry right there on the fifth floor of the hospital.

Team members of TMMK modified this Camry and assembled it at the plant in Georgetown, Kentucky. In December 2020, they reassembled it inside the hospital. Now, the therapy vehicle is helping patients on the road to recovery for a continued independent lifestyle. While there is no engine, the car simulator otherwise operates like a regular car, complete with adjustable seats and a working trunk. A hydraulic lift has the capacity to raise the car to approximate the level of each patient’s vehicle.

The license plate on the front of the car reads “SJH 1877” to commemorate the founding of Saint Joseph Hospital by the Sisters of Charity of Nazareth in 1877.

Project Hope’s completion

A renovation and expansion project for the CHI Saint Joseph Health - Cancer Care Center in Bardstown, Kentucky, began in 2019 and was opened in January 2021. Nearly $1.3 million was donated from community members for Project Hope to fund the Cancer Care Center, including proceeds from the annual Bourbon and Bluegrass Holiday Bash, fundraising events hosted by the employees of Heaven Hill Brands, and lead gifts from Bill and Mary Florence Barnes, David and Mary Stuart Haydon, the Albert and Leona Haydon Family Foundation, Haydon family members and Heaven Hill Brands, as well as numerous individuals.

The center has five additional infusion rooms, a new reception area, a new Resource Center, and more.

“In a year when hope sometimes felt bleak, this space is a shining emblem of hope,” said Jennifer Nolan, president of Flaget Memorial Hospital. “Hope is real, and it is here.”

Employee scholarships

The CHI Saint Joseph Health Foundations support our health care heroes who give so selflessly in their chosen career. Through your generous donations, we are able to offer scholarships so that employees can grow in their professional careers and provide even better care for our patients. This fiscal year we provided more than $55,000 for continuing education to 41 employees from Continuing Care Hospital, Flaget Memorial Hospital, Saint Joseph Berea, Saint Joseph East, Saint Joseph Hospital, Saint Joseph London, Saint Joseph Mount Sterling and the CHI Saint Joseph Medical Group. Your support assists with tuition to help our employees advance their education, pursuing degrees in biochemistry, bioethics & health policy, business administration, health care management, nursing, nursing administration, organizational leadership, physical therapy, psychiatric mental health, and other fields of study.

The “Telehealth Solutions for Rural Kentucky” program is helping people in 11 rural communities have access to telehealth services, right from home.

Telehealth solutions for rural Kentucky

Almost a half-million residents in 11 rural Kentucky counties now have access to expanded telehealth services, thanks to the “Telehealth Solutions for Rural Kentucky” program, whose goal is to provide improved treatment plan compliance for patients with acute conditions and improve health outcomes for all patients who participate.

The new technology also provides continuing education sessions for health care professionals at rural practice sites so they can avoid lengthy commutes to Lexington for training and demonstrations.

Through interactive monitors and mobile telehealth equipment, patients with acute and chronic conditions in infectious disease, cardiology, pulmonology, and neurology can receive a higher level of care than that available in their own communities.

Physicians at Saint Joseph Hospital in

Lexington, specializing in these conditions, offer virtual consultations to health care professionals and patients at five rural hospitals, 15 medical practices and two home health agencies.

Additionally, follow-up care is being offered via remote monitoring for approximately 2,500 patients. The 11 rural counties included in the project are Bell, Clark, Fleming, Knox, Laurel, Madison, Montgomery, Nelson, Pulaski, Taylor and Whitley.

Facilitated by CHI Saint Joseph Health Foundations, grant support totals $1,022,294 for the program ($869,604 in grant funds from the U.S. Department of Agriculture’s Distance Learning and Telemedicine program and $152,690 from the Saint Joseph Hospital Foundation).
