Limitaciones de los tests : PT
• Sensibilidad (inmunosupresion)
• Especificidad (BCG, MNT)
• No diferencia infección de enfermedad
• No diferencia infección reciente de infección antigua
• Bajo valor predictivo de desarrollo de TB activa
• Variabilidad
• Boosting (falsos positivos con PT repetidas)
• Se requieren 2 visitas
• Confidencialidad

“Neither IGRAs nor the TST have high accuracy for the prediction of activetuberculosis, although use of IGRAs in some populations might reduce thenumber of people considered for preventive treatment.”

To assess the non-inferiority to traditional testing (TST) of a sequential strategy using the TST followed by confirmation with QFT test when targeting preventive therapy in household contacts of patients with TB.
We hypothesized that such a strategy should narrow the target population compared with the standard TST-based strategy, without increasing the risk of subsequent active TB.
Aims OPTIMIST Clinical Trial
Open-labelled, controlled and randomized clinical trial comparing two strategies for diagnosing latent TB infection in 12 hospitals in Spain.

Methods- Design OPTIMIST Clinical Trial
Study populationHealthy adult household contacts of culture-proven active TB cases.
Sample sizen=870
Under the hypothesis of non-inferiority (1.5%) of the two-step strategy (TST/QFT) with respect to standard practice (TST alone) in terms of
preventing TB development* and detecting a 10% minimal difference in LTBI diagnoses
*An incidence of TB in the first two years of household contacts of TB patients was assumed to be 0.5%. (Diel et al, AJRCCM 2008)
Methods OPTIMIST Clinical Trial
Non-inferiority was defined as the upper limit of the one-sided, 97.5% confidence interval of the difference in TB incidence between the TST/QFT and the TST arms, being less than 1.5 percentage points.

We concluded that in low-incidence settings of tuberculosis, using QFT-GIT to confirm positive TST reactors results in a significant reduction of tuberculosis infection diagnoses and preventivetherapy prescriptions without increasing the riskof active disease.
