Solid- phase red cell adherence assay (SPRCA)

This assay can detect HLA, ABO, and platelet specific IgG antibodies. But weak reactions give immediate results.

Application of SPRCA

Cross match (e.g. NAITP mother’s serum +father’s platelet)

+Chloroquine: differential of HLA/HPA +drug : drug-induced thrombocytopenia +auto cell: auto antibodies

PRP preparation (ITP)

檢體收集 : 將含抗凝劑 EDTA 的全血在400 RCF 下室溫離心 10 分鐘 , 收集上層血漿部分。

病人 platelet count 檢體量＜ 10 K 40 mL

11~20K 40mL

21~50K 30mL

51~150K 20mL

＞ 150K 10mL

• 若懷疑病人有自體抗體，則病人須於最近一週內未輸過血小板濃厚液才可以抽血送檢。

• 若病人於最近三週內注射過 IVIG，則不適合此檢驗。

SB Substrate Buffer 2 mL 稀釋基質液用ST Stop Solution 2 mL 擦手紙 一 包

Procedure of Pakplus

1. 在所有 well中加入 300 μL WB 置於室溫 5~10分鐘2. 稀釋檢體， 100 μL血清加入 300 μL SD 3. 將反應盤中的清洗液倒掉，在吸水紙巾上拍乾4. 加入 50 μL稀釋好的檢體到 A~G wells；請勿加入 H

(blank)5. 貼好封膜，放入 37℃乾式培養箱，反應 40分鐘6. 配製二級抗體， 10 μL AH 加入 1 mL SD 低速

vortex避光備用 7. 反應 40分鐘後，倒掉盤內液體，在吸水紙巾上拍乾8. 清洗：每個 well加入 300 μL WB 、倒掉盤內液體9. 重複清洗三次後，在吸水紙巾上徹底拍乾10.A~G中加入 50 μL稀釋好的二級抗體

Procedure of Pakplus (II)1. 輕敲 PNPP 的玻璃瓶，減少粉末附著在瓶蓋；小心的打開。2. 加入 0.5 mL dd water ，蓋回瓶蓋，搖動混和均勻3. 取 20 μL 完全溶解的 PNPP ，加入 2 mL 的 SB 中，混和均勻

避光備用4. 反應 40 分鐘後，倒掉盤內液體，在吸水紙巾上拍乾5. 清洗：每個 well 加入 300 μL WB 、倒掉盤內液體6. 重複清洗三次後，在吸水紙巾上拍乾。7. 設定計時器倒數 30 分鐘，立即進行以下步驟8. A~G 中加入 100 μL 稀釋好的 PNPP ，勿加入 H(blank) 中9. 貼好封膜，避光室溫反應 30 分鐘

( 以 PNPP 加入第一個 well 起算 )10. 計時結束後在 A~G 中加入 100 μL ST ， H 中加入 200 μL

Result and interpretation

判讀：減去 blank 吸光度， 2X 於相對列mean of negative control 則可判定為positive ，介於 1X~2X 為 bordline.

但 positive 需至少 8X 的 mean of negative control 吸光度，否則實驗Invalid 。
