How to Create a Hoag Connect MyChart Account
Patients can sign up for Hoag Connect MyChart on a desktop or device. Please follow the step-by-step instructions below. COMPUTER/DESKTOP INSTRUCTIONS
1. Visit www.hoagconnect.org and click on the blue “Sign Up Now” button.
2. If you have an Activation Code, enter your information under “Step 1.” Upon completion, you will receive confirmation of your account set-up.
a. Please note, your Date of Birth and Social Security Number (SSN) must match what is in Hoag’s records. If you do not have/have not provided a SSN, please proceed to Step 3 to create an account without an Activation Code.

3. If you do not have an Activation Code, click on “Sign Up Online.”
4. Complete the requested information, which will be compared with data from a third-party identity vendor.
a. If upon completion, you receive an error message and they are unable to match your data, please email [email protected] to request an Activation Code. You will be emailed a code within 1-3 business days to activate your account.

5. Answer the series of questions from the third-party identity vendor to confirm your identity (sample shown below; questions will be related to your personal history).
6. Once your identity has been confirmed, follow the prompt to have a code sent to your email for final verification. Enter the code to continue.
7. Two-Step verification – Continue or Turn Off this additional security measure.
8. Set up a User Name and Password.

9. Confirm your Notification Preferences.
10. Account set-up is complete! a. You can now link your Hoag Hospital and Hoag Connect MyChart accounts by selecting
Menu → Link Accounts → and search/select Hoag Hospital.
DEVICE INSTRUCTIONS
1. From your device (smartphone, tablet, etc.), open or download the MyChart app.
2. Follow the same prompts as outlined above to create your account either with an Activation Code (Step 2) or without an Activation Code (begin at Step 3).
