RATING & DEFINITIONS

The Homeowners Policy Program provides property and liability coverages using the forms andendorsements referred to in this manual.

The rates, rules, forms and endorsements of Heritage Property & Casualty Insurance Company (Heritageor the Company) shall apply to all cases provided for in this Manual. This Manual contains the rules andclassifications governing the writing of the Homeowners Policy.

101. LIMITS OF LIABILITY AND COVERAGE RELATIONSHIPS

The limits of liability provided under the Homeowners policy are as follows:

102. DESCRIPTION OF COVERAGES

B. Section II Coverage - Liability - All Forms

Coverage E - Personal LiabilityCoverage F - Medical Payments to Others

1. Personal Liability - Covers payment on behalf of any insured for all sums which the insured willbecome legally obligated to pay as damages because of bodily injury or property damage arisingout of an insured’s premises or personal activities.

2. Medical Payments to Others - Covers medical expenses incurred by persons, other than theinsured, who sustain bodily injury caused by an accident arising out of an insured’s premises orpersonal activities.

104. ELIGIBILITY

Note: No Homeowners policy form may be issued in the name of a corporation, partnership orassociation. Limited coverage may be provided under the Dwelling Program.

A. FORM HPC HO-3 – A Homeowners Policy may be issued:

1. To the owner-occupant of a dwelling which is used exclusively for private residential purposesand contains not more than 2 families and with not more than 2 boarders or roomers per family;or

2. To the purchaser-occupant who has entered into a long term installment contract for thepurchase of the dwelling and who occupies the dwelling but to whom title does not pass fromthe seller until all the terms of the installment contract have been satisfied. The seller retainstitle until completion of the payments and in no way acts as a mortgagee. The seller’s interest inthe building and premises liability may be covered using Endorsement HPC 04 41 - AdditionalInsured; or

3. To the occupant of a dwelling under a life estate arrangement when the Coverage A amount is atleast 100% of the dwelling’s replacement cost. The owner’s interest in the building and premisesliability may be covered using Endorsement HPC 04 41 - Additional Insured; or

4. When a 2-family dwelling (duplex) is occupied by co-owners, each occupying distinct livingquarters with separate entrances. Given these circumstances, a Homeowners Policy providingbuilding coverage may be issued to only one of the co-owner occupants of the dwelling. Thepolicy may be endorsed to cover the interest of the other co-owner in the building and forpremises liability.

The Homeowners Policy may be extended, without additional premium charge, to cover theinterest of a non-occupant joint owner in the building and for premises liability. UseEndorsement HPC 04 41 - Additional Insured.

B. FORM HPC HO-6 - A Unit Owners Homeowners Policy may be issued to the owner(s) of acondominium or cooperative unit which is used exclusively for residential purposes, and is notoccupied by more than one additional family or more than two boarders or roomers.

C. Subject to all other sections of this rule, a Homeowners Policy may be issued to cover a seasonaldwelling.

D. A Homeowners Policy shall not be issued to cover any property located on a farm, ranch, orchard orgrove.

107. CONSTRUCTION DEFINITIONS

A. Frame — exterior wall of wood or other combustible construction, including wood ironclad, stuccoon wood, fiber cement siding or plaster on combustible supports.

Note: Includes aluminum or plaster / vinyl siding over frame.

B. Masonry Veneer — exterior walls of combustible construction veneered with brick or stone.

C. Masonry — exterior walls constructed of masonry materials such as adobe, brick, concrete gypsumblock, hollow concrete block, stone, tile or similar materials and floors and roof of combustibleconstruction (disregarding floors resting directly on the ground.)

D. Superior Construction

1. Non-Combustible — exterior walls, floors and roof constructed of, and supported by metal,asbestos, gypsum, or other non-combustible materials.

2. Masonry Non-Combustible — exterior walls constructed of masonry materials (as described in C.above) and floors and roof of metal or other noncombustible materials.

3. Fire Resistive — exterior walls, floors and roof constructed of masonry or other fire resistivematerials.

Note: Mixed (Masonry/Frame) — a combination of both frame and masonry construction shall beclassed and coded as frame when the exterior walls of frame construction (including gables) exceed33 1/3 % of the total exterior wall area; otherwise class and code as masonry.

109. SINGLE BUILDING DEFINITION

A. All buildings or sections of buildings which are accessible through unprotected openings shall beconsidered as a single building.

B. Buildings which are separated by space shall be considered separate buildings.

C. Buildings or sections of buildings which are separated by:

1. A six (6)-inch reinforced concrete or an eight (8)-inch masonry party wall; or

2. A documented minimum two (2) hour noncombustible wall that has been laboratory tested forindependent structural integrity under fire conditions; which pierces or rises to the underside ofthe roof and which pierces or extends to the inner side of the exterior wall shall be consideredseparate buildings. Accessibility between buildings with independent walls or through masonryparty walls described above shall be protected by at least a Class A Fire Door installed in amasonry wall section.

301. TOTAL POLICY PREMIUM CALCULATION

The rating order in this manual generally follows the order of the PREMIUM CALCULATIONWORKSHEETS. For further assistance refer to the HPC HO-3 and HPC HO-6 Premium CalculationWorksheets at the end of this manual.

A. To determine the TOTAL POLICY PREMIUM follow the steps below.

1. Calculate the INDIVIDUAL PERIL PREMIUM for each individual peril by multiplying the BASERATE for each individual peril by a series of rate factors, then round the product to the nearestwhole dollar. The rate factors used to modify the BASE RATE are applied based on coverageoptions selected and certain individual risk characteristics. When a coverage option or riskcharacteristic does not apply to an individual peril, use a factor of 1.000.

The number of INDIVIDUAL PERIL PREMIUMS varies by policy type. There are 7 INDIVIDUALPERIL PREMIUMS for the HPC HO-3 policy (Fire, Water, Liability/Medical, All Other Perils,Sinkhole, Hurricane, and Other Wind) and 4 INDIVIDUAL PERIL PREMIUMS for the HPC HO-6policy (Theft, All Other Perils, Hurricane and Other Wind).

Note: Calculation of the Hurricane and Other Wind peril premiums is not applicable when theoptional coverage for Windstorm or Hail is excluded from the policy. This applies to all Heritage

HO policy types. Calculation of the Sinkhole peril premium is not required when the optionalcoverage for Sinkhole Loss is not selected. Optional Sinkhole coverage ONLY applies to HPC HO-3 policy type. The remaining individual perils on all policy forms are not optional and cannot beexcluded from the policy.

2. Calculate the BASE PREMIUM by determining the sum of the INDIVIDUAL PERIL PREMIUMSdeveloped in the previous step.

3. Calculate the sum of any ADDITIONAL COVERAGE OPTIONS AND ENDORSEMENTS premium.

4. Calculate the TOTAL MANDATORY ADDITIONAL CHARGES. First, determine each individualmandatory surcharge. This is done by multiplying the GRAND SUBTOTAL by each mandatorysurcharge factor separately and rounding the resulting product to the nearest whole dollar. Thenadd the individual surcharges together to produce the TOTAL MANDATORY ADDITIONALCHARGES.

The mandatory surcharge factors are provided on the HPC HO-3 and HPC HO-6 PremiumCalculation Worksheets.

Exception: The Emergency Management Preparedness & Assistance Trust Fund surcharge is awhole dollar amount and does not require a multiplication step. Refer to the PremiumCalculation Worksheet.

5. Finally, calculate the TOTAL POLICY PREMIUM. Add the GRAND SUBTOTAL to the TOTALMANDATORY ADDITIONAL CHARGES developed in the previous step.

B. Rate Factor Determination (Interpolation)

When the desired limit of liability or coverage percentage chosen is between the limits shown on anyrate table, determine the rate factors using the following procedure.

1. Procedure (For each peril)

a. From the applicable rate factor table, determine the next lower limit and the next higher limitthan the desired limit.

b. Subtract the lower limit from the desired limit and divide this result by the differencebetween the higher limit and the lower limit.

c. Determine the rate factors for each of the higher and lower limits.

d. Subtract the lower limit’s rate factor from the higher limit’s rate factor and multiply by theresult from b.

e. Add the result from d. to the lower limit’s rate factor. Round to the third decimal. The resultis the rate factor for each peril for the desired limit.

2. Example

The factors in the following example are for illustration only and are not necessarily factorsshown in the Coverage A Rate Factor Table of this manual.

Assume the desired Coverage A limit is $215,000, and that this limit is not in the Coverage A RateFactor Table. Also, assume that the next lower limit in the table is $200,000 and the next higherlimit is $225,000. The Factors for these limits for a particular peril are 1.820 and 2.022,respectively.

Following the steps above:

a. The next lower and next higher limits are $200,000 and $225,000

b. ($215,000 - $200,000) / ($225,000 - $200,000) = $15,000 / $25,000 = 0.60

c. The rate factors are 1.820 for $200,000 and 2.022 for $225,000

d. (2.022 – 1.820) x 0.60 = 0.1212

e. 0.1212 + 1.820 = 1.941 = Rage Factor for $215,000

303. BASE RATES

The first step in calculating the INDIVIDUAL PERIL PREMIUMS is to determine the BASE RATES. TheBASE RATES are as follows:

Note: When an individual peril insured against does not apply (e.g., Optional Sinkhole coverage is notselected or Windstorm or Hail is excluded) the base rates for that peril are not applicable. The individualperil base rate will default to zero (0) and individual peril rating factors will default to 1.000.

304. WINDSTORM OR HAIL EXCLUSION

A. The peril of Windstorm or Hail may be excluded at renewal for existing policies. No mid-termendorsement requests to add, change or remove this coverage will be accepted. Requests to excludeWindstorm or Hail for a property must be submitted with Form HPC WE. Use Endorsement HO 04 94- Windstorm or Hail Exclusion.

B. When Windstorm or Hail coverage is excluded, the Hurricane and Other Wind individual peril baserates and individual peril rating factors are not applicable.

305. TERRITORY RELATIVITIES

From the Territory Relativities Table, select the factors for the territory that apply. Apply the factors tothe INDIVIDUAL PERIL PREMIUM calculation described in the Total Policy Premium Calculation rule.

306. COVERAGE A LIMIT OF LIABILITY – FORM HPC HO-3

A. From the Coverage A Rate Factor Table select the rate factors for the Coverage A limit of liability thatapplies. If the desired limit of liability for Coverage A is not provided in the table, see theInterpolation example on developing the appropriate rate factor for the desired limit of liability.

B. Upon determining the Coverage A rate factor for each peril, apply the rate factors to theINDIVIDUAL PERIL PREMIUM calculation described in the Total Policy Premium Calculation rule.

309. OTHER STRUCTURES (COVERAGE B) COVERAGE OPTIONS – FORM HPC HO-3

The base rates include a Coverage B - Other Structures blanket limit of 10% of the Coverage A limit ofliability.

A. Optional Reduction Limit

1. The Coverage B blanket limit may be reduced to 5%, 2% or 0% of Coverage A for a premiumcredit at renewal.

2. Select the appropriate factor from the Coverage B Rate Factor Table and apply it to theINDIVIDUAL PERIL PREMIUM calculation described in the Total Policy Premium Calculation rule.

3. When Coverage B is excluded (0%) use Endorsement HPC 04 12 – Exclusion of Section I -Coverage B Other Structures Coverage.

B. Specific Structure Increased Limits

Increased limits on specific structures may be purchased up to 70% of Coverage A when theCoverage B blanket limit has not been reduced or excluded.

Note: For rating Permitted Incidental Occupancies in Other Structures see Permitted IncidentalOccupancies Residence Premises rule.

1. For structures not rented to others the following additional premium applies:

$4 per $1,000 of insurance

Use Endorsement HO 04 48 - Other Structures Increased Limits

2. For “Structures Rented to Others - Residence Premises” the following additional premiumapplies:

a. $6 per $1,000 of insurance, andb. A premium of $38 for the increased Coverage E and Coverage F exposure.

Use Endorsement HO 04 40 - Structures Rented to Others - Residence Premises.

3. Apply these premiums to the ADDITIONAL COVERAGE OPTIONS AND ENDORSEMENTS premiumcalculation described in the Total Policy Premium Calculation rule.

Mid-term Coverage B changes will be considered. Endorsement requests must be submitted toUnderwriting for approval and must include documentation that supports a mid-term change todecrease (or increase) the coverage limit (e.g. remodeling, loss, additions, etc.).

401. PERSONAL PROPERTY (COVERAGE C) COVERAGE OPTIONS

A. HPC HO-3 provides Coverage C in the amount of 50% of Coverage A.

1. Reduction in Limit

2. Exclusion of Personal Property

Coverage C may be excluded on new policies or at renewal for existing policies for a premiumcredit. No mid-term endorsement requests to add or exclude contents coverage will be accepted.Any request to exclude contents coverage must be submitted with form HPC CE for the insuredsstatement.

4. Required Documentation

For burglar and fire alarms, a legible copy of the certificate of installation that includes amonitoring agreement or a current legible copy (not more than one {1} year old) of themonitoring subscription agreement is required.

For fire sprinkler systems, a copy of the certificate of installation from a sprinkler contractorlicensed per Section 633.021, Florida Statute is required. The sprinkler contractor must holdeither a Class I, II or IV license and must certify that the installation substantially complies withNational Fire Protection Association (NFPA) sprinkler standard 13, 13D or 13R. Alternatively,documentation that the building has been specifically rated by the I.S.O. will suffice.

D. Age of Home / Year Built Premium Adjustment – Form HPC HO-3 Only

All risks insured on a Homeowners Form are subject to an Age of Home / Year Built premiumadjustment.

1. For the Hurricane and Other Wind INDIVIDUAL PERIL PREMIUMS the age of home rate factor isapplied based on the year built as provided in the Age of Home Rate Table.

2. For all of the other INDIVIDUAL PERIL PREMIUMS, the age of home rate factor is determined bysubtracting the year built from the policy effective year. The rate factor applies to the year duringwhich the construction of the house was completed, regardless of the date of first occupancy.

3. Premium Determination

To apply the Age of Home / Year Built factor, determine the appropriate rate factor on the Age ofHome / Year Built Rate Table. Then apply the rate factors to the INDIVIDUAL PERIL PREMIUMcalculation described in the Total Policy Premium Calculation rule.

E. Seasonal Property

1. Definition

A seasonal property is a dwelling that has continuous unoccupancy of three (3) or moreconsecutive months during any one (1) year period. A dwelling that has continuous unoccupancyof six (6) consecutive months during any one (1) year period is subject to a surcharge.

2. Premium Determination

From the Seasonal Property Factor Table determine the applicable rate factor. Then apply therate factors to the INDIVIDUAL PERIL PREMIUM calculation described in the Total PolicyPremium Calculation rule.

3. Hurricane Deductible Changes

Hurricane deductible options may only be amended effective at the normal policy renewal date.

601. WINDSTORM MITIGATION FEATURES

A. For policies that cover the peril of Windstorm or Hail, a premium credit may be available if one ormore of the following loss mitigation features or construction techniques exists:

1. Roof Covering;2. Roof Deck Attachment;3. Roof Wall Connection;4. Opening Protection;5. Roof Shape;6. Secondary Water Resistance;

The credit recognition and description of the loss mitigation features listed above are described inthe Loss Mitigation Credits Table contained in Paragraph F. below.

Note: For townhouse or row-house, use 1 to 4 unit tables regardless of number of units in thebuilding.

B. Proof of Compliance

The company requires proof to substantiate the existence of loss mitigation features displayed in theLoss Mitigation Credit tables, except roof shape that is verifiable via a photograph. Use Form OIR-B1-1802, Uniform Mitigation Verification Inspection Form (“UMVIF”). The applicant/insured isresponsible for the expense associated with the use of this form or any other costs for substantiatingthe existence of the mitigation features.

Heritage will return the least amount of premium credit based on the applicable mitigation featuresdisplayed in the Loss Mitigation Credit Table if the loss mitigation feature(s):

1. Is unknown; or2. Cannot be verified; or3. Is not substantiated by a mitigation affidavit/form.Exceptions to use of forms listed above:1. Year built on or after January 1, 2002: Structures built on or after January 1, 2002, are eligible

for the minimum FBC 2001 credit by providing documentation that validates the year ofconstruction. Acceptable documents include: certification of occupancy, copy of propertyappraisal or any other document the Company deems acceptable. Completion of the UMVIF isnot required to receive the minimum FBC 2001 credit. Additional credits may be obtained bycompleting the UMVIF.

2. Year built 2002 or later (Dade and Broward County ONLY): Type II and Type III structures builton or after January 1, 2002 in Dade or Broward County are eligible for Opening Protection ClassA credit by providing documentation that validates the year of construction. Acceptabledocuments include certification of occupancy, copy of property appraisal or any other documentthe Company deems acceptable. Completion of the UMVIF is not required to receive this Class Acredit.

C. Wind Design – 1 to 4 Units built on or after January 1, 2002.

FBC 2001 establishes base (e.g. minimum) Wind Design Speeds (WDS) for each FBC Wind Speedzone. When WDS of the structure (e.g. 1-4 unit buildings built on or after January 1, 2002) exceedsthe basic speed design of the wind speed zone, certification is required for premium credit.

Predominant Roof Covering Rule

Roof covering should be determined by using the roof covering that comprises greater than50% of the roof surface of the dwelling, excluding entrance ways, porches, and decorativenonstructural gables.

4. Roof Shape

Hip - roof having sloping ends and sloping sides down to the eaves line. Gable – The portion of the roof above the eaves line of a double-sloped roof; the end section

appears as an inverted V. Flat – A horizontal roof with a pitch less than 10 degrees.

Predominant Roof Shape Rule

The predominant roof shape should be determined based upon the type of roof structure thatexceeds 50% of the roof surfaces subject to the following:

A Hip roof must be comprised of no other roof shapes greater than 50% of the exterior walllength.

Any exterior wall with a Gable end exceeding 50% of the exterior wall length shall beclassified as Gable.

5. Roof Deck Attachmentsb. For 1 to 4 unit buildings and 5+ unit – Type I buildings:

602. BUILDING CODE COMPLIANCE (BCEGS)

Note: Not applicable when the peril of Windstorm or Hail is excluded.

A. General

1. The Building Code Effectiveness Grading Schedule (BCEGS) develops a grade of “1” to “10” for acommunity based on the adequacy of its building code and the effectiveness of its enforcementof that code. Policies which cover the perils of Windstorm or Hail may be eligible for specialrating treatment, subject to the criteria in the following paragraphs.

2. In some communities, two BCEGS grades may be assigned. One grade will apply to one and twofamily dwelling buildings and/or personal Property contained in such buildings. The other gradewill apply to all other buildings occupied for residential, commercial and/or manufacturingpurposes including personal and business property contained therein.

3. A Building Code Compliance rating factor does not apply when the peril of Windstorm of Hail isexcluded from the policy.

B. Community Grading

1. The BCEGS grade applies to any building that has an original certificate of occupancy/completiondated in the year of the effective date of the community grading, or later.

2. If a community is re-graded, the factor for the revised grade applies to buildings that have anoriginal certificate of occupancy dated the year of the revised grading, or later.

3. If, due to an addition or alteration, the original building is changed to comply with the latestbuilding code, the factor for the community grading applicable at the time the reconstruction iscompleted will apply to such building.

4. Communities that decline to participate in the BCEGS Program will be identified as notparticipating and will receive a premium surcharge, unless they qualify for Individual Grading.This surcharge will apply to any building that has an original certificate of occupancy dated in theyear of the effective date of the community evaluation that indicates the community as notparticipating.

C. Individual Grading

Where buildings have been built in full conformance with the natural hazard mitigation elements ofone of the nationally recognized building codes, even though the community grade is greater than“1”, or the community is not participating in the program, exception rating procedures may apply.

Any building may be classified as Grade one “1” for Windstorm/Hail upon certification by a Floridalicensed architect or structural engineer, based on an on-site inspection, that such building is incompliance with one of the three nationally recognized building codes with respect to mitigation ofthe Windstorm and/or Hail hazard. This classification is effective only from the date of thecertification.

Any costs associated with this provision are to be paid by the insured.

Note: Submit “Individual Property Certification” Form HPC 95.

D. Ungraded and Non-Participating Risks

Buildings which do not qualify for Community or Individual Grading are classified as ungraded ornon-participating risks. Do not classify as Grade “10”.

E. Premium Computation

Compute the premium as follows:

For replacement policies issued to policyholders assumed from Citizens Property InsuranceCorporation, sinkhole coverage will be endorsed to the Company’s replacement policy and theapplicable premium will apply if the assumed Citizens policy provided sinkhole loss coverage. If theassumed Citizens policy did not provide sinkhole loss coverage, the Company’s replacement policywill provide catastrophic ground cover collapse coverage but will not provide sinkhole loss coverage.Sinkhole coverage is included in the HPC HO-6 base policy and cannot be excluded.

B. An assumed policyholder with sinkhole loss coverage from Citizens may request to exclude suchcoverage at the inception of the Company’s policy or at any subsequent renewal date. An assumedpolicyholder without sinkhole loss coverage from Citizens may request to purchase such coverage atany subsequent renewal date, subject to appropriate underwriting review and Section C. of this rule.No mid-term endorsements are acceptable. Use Endorsement HPC – 23 94 – Sinkhole Loss Coverage– Section I Property Coverage.

C. Renewal Endorsement Requirements

1. To remove sinkhole loss coverage, the Sinkhole Loss Coverage Selection / Rejection Form – HPCSLC-R must be complete and submitted to Heritage.

2. To add sinkhole loss coverage, the Sinkhole Loss Coverage Selection / Rejection Form – HPC SLC-R must be completed and submitted to Heritage at least 90 days in advance of the policy’srenewal date. An approved structural inspection must be completed no more than 45 days priorto the submission of the change request. The inspection shall document existing damage,evaluate the structural integrity of the dwelling and verify that there is no current or proximatesinkhole activity that has not been disclosed.

3. An “Approved” inspection service is one who has been designated by Heritage as competent toperform the evaluation and whose report format meets Heritage’s informational requirements.The policyholder will contract directly with the Approved inspection service and pay Heritage’spre-arranged fee. Heritage and the policyholder will receive a copy of the completed inspection.The inspection fee is not refundable whether or not the dwelling is deemed acceptable forsinkhole coverage by Heritage.

4. An inspection from an inspection service who has not been designated by Heritage as“Approved” may be submitted by the policyholder for consideration in lieu of an “Approved”inspection. This inspection must be completed by a professional engineer, a professionalgeologist, a geotechnical engineer, or other individual or entity recognized by Heritage aspossessing the necessary qualifications to properly complete the inspection. The inspection mustmeet all the requirements specified in Section C.2. The policyholder is responsible for all costsassociated with this inspection.

D. Upon receipt of the inspection, Heritage will review and either deny or approve sinkhole, losscoverage. If the inspection shows sinkhole damage, structural issues and / or prior or currentsinkhole activity, sinkhole loss coverage will be declined. In certain circumstances, our review of thesinkhole inspection may result in Heritage ordering a separate property inspection, at our expense,to determine insurability.

E. Sinkhole Loss Coverage Premium Determination

1. Coverage Premium

Determine the Sinkhole INDIVIDUAL PERIL PREMIUM by multiplying the sinkhole base rate bythe applicable rating factors. This process is described in the Total Policy Premium Calculationrule.

A. Coverage for a permitted incidental occupancy is limited under Section I and excluded under SectionII. The policy may be endorsed to provide expanded Section I coverage and Section II coverage on apermitted incidental occupancy in the dwelling or in another structure on the residence premises.

Use Endorsement HO 04 42 - Permitted Incidental Occupancies - Residential Premises, for Sections Iand II Coverage.

B. Permitted Incidental Occupancies

Examples of such occupancies are offices, schools or studios meaning offices for business orprofessional purposes, and private schools or studios for music, dance, photography and otherinstructional purposes.

C. If the permitted incidental occupancy is located in another structure, Coverage B does not apply tothat structure unless specific insurance coverage for that structure is added. See E. below for thecharge for specific insurance on the structure.

Note: Not applicable when Coverage B is excluded.

The policy automatically provides at no additional charge, $1,000 of loss assessment coverage, excludingearthquake, for assessments relating to the residence premises; subject to the coverage and perilsinsured against in each policy type.

The HPC HO-3 limit may be increased if no association losses have occurred to which this coverage mayapply.

The HPC HO-6 loss assessment minimum limit must be $2,000.

The second $1,000 of the $2,000 limit is provided by application of the “$2,000 Premium” chargeindicated in the table below.

A. Form HPC HO-6 provides exclusions for Coverage C - Personal Property and Section II Liability whenthe residence premises is regularly rented or held for rental to others. The policy may be endorsed,however, to eliminate these exclusions.

B. The Coverage C minimum limit of liability is $6,000.

C. The additional premium is developed by multiplying the BASE PREMIUM by 0.25 and rounding theproduct to the nearest whole dollar.

850. IDENTITY THEFT EXPENSE AND RESOLUTION SERVICES COVERAGE / HPC 09 IDT

A. The policy may be endorsed, for an additional premium, to provide coverage for loss that resultsfrom the act of Identity Theft or Identity Fraud. The endorsement provides coverage for:

1. Identity Theft Expenses: Pays up to $25,000 for expenses incurred by an insured as the directresult of identity theft.

2. Resolution: Access to resolution services from a consumer fraud specialist to assist in the processto restore identity.

B. By endorsement, coverage is extended to the policyholder, spouse, domestic partner, children livingat home (up to age 21 or full time student up to age 26), and grandparents living at the insuredpremises.

C. Additional Premium: $25

D. Use Endorsement HPCHO 09 IDT – Identity Theft Expense and Resolution Services Coverage.

860. PLATINUM PREFERRED (REPAIR PROGRAM – COMPANY OPTION) / HPCHO3 PPS & HPCHO6 PPS

A. The policy may be endorsed at the option of the policyholder. If selected, the endorsement will allowthe Company the option to select an appropriately licensed or qualified contractor to repairdamaged property as provided by the policy, if the Company so elects.

B. If the endorsement is added, the policyholder’s deductible obligation at loss settlement in the eventof a covered property loss other than a hurricane loss or sinkhole loss will be reduced by ten percent(10%) of the All Other Perils deductible. The credit does not reduce the applicable deductible underthe policy. The credit will apply only when the amount of a covered loss exceeds the applicabledeductible.

C. The Company may pay the amount of the deductible credit directly to the selected contractor onbehalf of the policyholder. The policyholder remains responsible for the balance of the deductible. Ifthe Company does not select the contractor for a covered loss to which the credit applies, theCompany will pay the amount of the credit (i) to the contractor if the covered property is repairedunder the terms of the policy, or otherwise (ii) to the insured (and mortgage company, if applicable).

D. Upon prior written notice to the Company, the endorsement maybe removed from the policy as ofthe next succeeding policy renewal date.

870. SCREEN ENCLOSURE

A. The policy excludes coverage for screen enclosures and pool cages.

B. Screen Enclosure coverage for All Other Perils is provided by use of endorsement HPCHO SCR AOPwith an appropriate increase in Coverage A or B.

C. Screen Enclosure coverage for All Other Perils and Hurricane may be purchased for an additionalpremium. Limits may be purchased in amounts of $5,000, $10,000, $15,000, $20,000 and $25,000.The Screen Enclosure limit selected is additional insurance. For hurricane loss, the policy hurricanedeductible applies. Use endorsement HPCHO SCR HUR.

D. Premium:

900. CAPPING RULE

For policies with an effective date of 8/28/2013 or later, the renewal total policy premium will be

subject to the following capping rule:

For HO3 policies, the renewal total policy premium will be capped on the upside at 110% of the

premium for the expiring policy, and on the downside at 108% of the premium for the expiring

policy.

For HO6 policies, the renewal total policy premium will be capped on the upside at 110% of the

premium for the expiring policy, and on the downside at 80% of the premium for the expiring

policy.

When applying the capping rule, the premium for the expiring policy will be recalculated to be

consistent with the policy characteristics associated with the new policy term.

901. MANDATORY ADDITIONAL CHARGES

A. Emergency Management Preparedness and Assistance Trust Fund

A fully earned annual surcharge of two (2) dollars will be imposed on every policy as required byFlorida Law.

B. Managing General Agency Fee

Pursuant to the provisions of Section 626.7451, Florida Statutes the Company’s Managing Agent asdefined in Section 626.091, Florida Statutes will charge a $25 policy fee on each new and renewalpolicy. The policy fee shall be a component of the Company’s Rate Filing and shall be fully earned.

C. Other Surcharges and Assessments

Other surcharges and assessments may be levied in accordance with state statute or an Office ofInsurance Regulation rule. These charges and the corresponding calculations will be disclosed on thePremium Calculation Worksheet if they are applicable.

UNDERWRITING GUIDELINES & GENERAL RULES

Rule 201. GENERAL INFORMATION

A. This manual contains information to assume homeowners insurance policies from Citizens PropertyInsurance Corporation on behalf of Heritage Property & Casualty Insurance Company (Heritage or theCompany). The Heritage assumption program is not intended for use in the voluntary market.

B. The Agent and Service Company shall be solely responsible for the favorable relationship between theinsured and Heritage.

Rule 202. ELIGIBILITY

All business has been assumed from an assumption from Citizens Property Insurance Corporation. No newbusiness is being accepted at this time. Unless the property is specifically determined to be ineligible forcoverage under the Citizens’ underwriting guidelines, the property shall be eligible for coverage in thisprogram.

Rule 203. EXTENT OF COVERAGE AND AUTOMATIC INCREASE IN LIMITS

A. Property

1. Homeowners – provides replacement cost coverage on structures and actual cash value onpersonal property.

2. Condominium Unit Owners – provides replacement cost coverage on additions and alterationsthat are not the responsibility of the condominium association and actual cash value on personalproperty.

Note: Replacement Cost Coverage on personal property may be purchased in Homeowners andCondominium Unit Owners risks.

B. Automatic Increase In Limits

1. General

The Coverage A Dwelling limit is based on “Insurance To Value.” Therefore, the Coverage ADwelling limit may be adjusted annually at renewal for inflation as determined by a nationalindex of construction costs.

The automatic increase in Coverage A does not apply to: Condominium Unit Owners policies.

2. Coverage Adjustment

The Coverage A Dwelling limit may be adjusted at each renewal for inflation. For example, if thenational index of construction costs increased by 3%, the Coverage A amount on a dwellinginsured for $100,000 will increase to $103,000. Other Section I blanket limits will be adjustedproportionately.

3. Renewal Declaration Page

If an adjustment is made to the Coverage A Dwelling limit, it will be indicated on the renewalDeclarations Page by the following statement: “Property coverage limit increased due to inflationmeasured by a national index of construction costs.

Rule 204. APPLICATIONS

No applications for new business will be accepted. Unless and until Heritage initiates a reapplicationprocess it will rely on the information received from the Citizens Property Insurance Corporation andsubsequent inspections and documentation to process business and make underwriting decisions.Underwriting surcharges and credits to a policy can only be removed by endorsement.

Rule 205. ENDORSEMENT AND REINSTATEMENT SUBMISSION PROCEDURES

A. Endorsement Submission Procedures

All endorsements that increase, broaden or extend coverage shall be signed by the applicant and theAgent of record and shall be accompanied by the required documentation as described in thefollowing paragraphs. All endorsement requests shall be legible and submitted to the Company nolater than five (5) business days from the day the endorsement was completed and signed by theinsured and Agent.

Requests to cancel policies must be submitted in writing and be signed by the named insured andagent.

The “5-day” rule also applies to the receipt of premiums. Thus, when premiums are accepted by theAgent, he or she must submit funds to the Company no later than five (5) business days from the daythe premium was accepted by the Agent.

B. Reinstatement Procedures

All reinstatements must be approved by the Company in advance. The Company will advise theAgent on the conditions to which approval for reinstatement will be granted. These conditions willinclude, but not be limited to, payment of premium, execution of a signed statement of no loss bythe insured, and review of current underwriting requirements.

Rule 206. ADDITIONAL UNDERWRITING REQUIREMENTS

A. Loss Frequency

For policyholders who have sustained 3 or more losses (excluding acts of God) in the 36 monthperiod immediately preceding the effective date or subsequent renewal date, the only policyavailable is Dwelling Fire Form HPC DP-1.

B. Older Homes

Homes over 50 years old must have updated electrical wiring and heating within the last 35 years tobe eligible for coverage (not applicable to condominium risks). Acceptable documentation from averifiable qualified inspector must be provided, upon request, prior to renewal.

C. Pools and Similar Structures

Properties with pools and similar structures must be completely fenced, walled or screened.

Note: Fence or wall must be a permanent installation with a minimum height of 4 feet and beconstructed of material that provide a reasonable barrier to entry as determined by Heritage(e.g. chain link, wood, steel, aluminum).

Exception: Properties not meeting these requirements are eligible for a Dwelling Policy without Liability.

D. Hurricane or Tropical Storm Binding Suspension

No request for new coverage or endorsement for increased coverage may be bound, written orissued, or monies received, regardless of effective date, when a Tropical Storm or Hurricane Watchor Warning has been issued by the National Weather Service for any part of the State of Florida, oras otherwise determined by the underwriting department.

E. Flood Insurance Requirements – Insureds with properties in Special Flood Hazard Areas, as defined bythe National Flood Insurance Program (NFIP) (e.g. A, AO, AH, A1-A30, AE, A99, V, V1-30, VE) mustmaintain a flood policy unless the applicant or insured sign the “Election Not To Buy Separate FloodInsurance” (HPC FW01) form, or an exception in this rule applies.

1. Waiver of Flood Option – Securing flood insurance is not a condition of coverage if the applicantor insured sign form HPC FW01. An applicant or insured who does not maintain a flood policy, or

does not sign the HPC FW01 form, may be denied Heritage coverage.

2. Coverage Requirements – If form HPC FW01 is not completed, or the property does not meet anexception, the insured must maintain a flood policy in effect, subject to the maximum limitsavailable from NFIP, as follows:

a. With building limits not less than 80% of the Heritage building limits, orb. Where NFIP issues an Actual Cash Value (ACV) policy, not less than 80% of the building ACV,

andc. With contents limits in any amount, if Heritage contents coverage exists.

3. Exceptions:

a. Condominium unit owner above ground floor. However, if building coverage is provided byan Apartment or Condominium Association master policy, building coverage is not required.

b. Condominium Association master policy, building coverage is not required.c. Policies with “windstorm or hail” coverage excluded.d. Certain risks (e.g., cooperative unit within cooperative building, gazebo, contents located in a

building not eligible for flood coverage under the NFIP “Ineligible Property” rule and the NFIP“Examples of Ineligible Risks” rule): A flood policy will not be required for these risks.

4. Notes:

a. A “Difference in Condition” (DIC) policy may not be substituted for the flood policyrequirement.

b. Flood Policy Waiting Period: If a flood application is pending due to a waiting period, the riskis eligible for Heritage coverage if otherwise eligible. The policy number must be provided toHeritage as soon as issued to avoid non-renewal.

Rule 207. UNINSURABLE PROPERTIES

The following risks may not be insured with Heritage.

A. Commercial Property

Commercial property including properties where any commercial activity is present beyond the strictinterpretation of the incidental offices permitted as an exception within this program.

B. Mobile Homes

C. Coverage Limits – Minimum/Maximum

Properties for which replacement cost (Coverage A) or Actual Cash Value (Coverage C) is eitherbelow or above the required limits.

D. Replacement Cost/Market Value Ratio

Properties with Replacement Cost exceeding 1 ½ times the market value, excluding land values.

E. Condemned Property

Properties which are condemned or scheduled to be condemned.

F. Property In Disrepair/Existing Damage

Properties in state of disrepair or properties with existing damage with no definitive proof of intentto repair within ninety (90) days.

G. Property Over 50 Years Old

Properties over 50 years old that have not been updated (N/A to HPC HO-6) are ineligible for renewalunless electrical wiring and heating are both in sound condition and have been updated within the last35 years. Shingle or other roofs not specified in this section must be less than 25 years in age and tile,State, clay, Concrete or Metal roofs must be less than 50 years in age, and have at least 3 years

remaining useful life. Acceptable documentation from a verifiable qualified inspector must be providedupon request.

H. Farms & Ranches

Dwellings located on a farm, ranch, orchard or grove; or where farming activities or ranchingoperations take place.

Exception: Dwellings used for residential purposes when farming or ranching is incidental, may beeligible for a Dwelling Policy excluding Liability (see Dwelling Manual).

I. Business Exposure

Properties where a business is conducted. Two salient elements to help identify a “business” include:(1) a profit motive and (2) continuity of the activity. (e.g., all manufacturing, retail sales whencustomer traffic is common, bed & breakfast operations, nursing homes, adult care living facilities,produce stands, “u-pick-it” produce operations, kennels, repair work including auto or appliancerepair, hair salons, and certain residential Home Day Care operations not Registered or Licensed –see “Residential Family Day Care Homes” rule for further information on Home Day Care risks)

Exception: Except incidental offices, schools or studios meaning offices for business or professionalpurposes and private schools or studios for music, dance, photography and other instructionalpurposes.

J. Heating And Electrical

1. Properties which have a portable heater or open flame as a primary source of heat, (e.g., electric,oil or kerosene portable space heater, gas heater, or any device utilizing an open flame).

Exception: Permanent and factory or professionally installed, central gas, fireplaces or woodburning stove heat systems.

2. Properties which contain any potentially hazardous electrical conditions, “knob & tube” oraluminum branch wiring circuits.

3. Properties equipped with electrical service less than 60 amps.

Exception: If approved by a Florida licensed electrician, licensed journeyman electrician, ormunicipal building inspector within the last five (5) years.

K. Roof Conditions and Age

1. Roofs that are damaged; or

2. Roofs that have visible signs of leaks; or

3. Roofs that have less than three years of remaining useful life. The “remaining useful life” is theremaining life expectancy of the roof covering to function as intended based upon an inspection ofthe wear and tear, decay, deterioration, decline, or defect, present from natural, climatic,construction, or other local conditions.

4. Shingle, built up tar and gravel or other roof coverings not identified in 5. below that are over 25years old; and

5. Tile, slate, clay concrete or metal roof coverings over 50 years old.

Exception to K.4 and K.5.: Risks that do not meet the roof replacement eligibility requirementsabove may be eligible for coverage by submitting acceptable documentation verifying the roof hasat least 3 years remaining useful life. Risks that establish roof eligibility under this exception are notrequired to provide documentation of full roof replacement until the policy renewal period in whichthe remaining useful life of the roof covering falls below the 3-year eligibility threshold. Acceptabledocumentation includes a copy of a completed roofing contract; a statement from a licensed

roofing contractor showing estimated age, condition and remaining useful life or other acceptableproof of remaining useful life.

L. Homemade/Rebuilt Property

Dwellings or structures that are homemade or rebuilt, or any dwelling constructed with extensiveremodeling.

Exception: If approved by local government building or zoning department or a certificate ofoccupancy has been issued.

M. Non-Habitational Property

Dwellings used, designed or constructed for non-habitational purposes.

N. Fraternity Or Sorority Houses

Fraternity, Sorority or any similar housing arrangement.

O. Vacant Or Unoccupied Property

Vacant or “unoccupied” dwellings. “Unoccupied” includes dwellings with personal propertycontained therein if the dwelling is no longer a place of usual return. Eligible seasonal or secondarydwellings are not considered vacant or unoccupied.

P. Seasonal Homes

A seasonal home (home with continuous unoccupancy of 3 or more consecutive months during any 1year period) unless the home is located in a “secured area” (limited access with locked gates orguards) or the home has a functioning central station fire and burglar alarm and documentationacceptable to the Company is provided.

Q. Special Flood Hazard Areas

Properties located in NFIP designated Special Flood Hazard Areas, (“A” and “V” zones), where aFlood Insurance Policy has not been purchased or has not been continued with coverage limits, notless than 80 percent of the building limits, subject to the maximum limits available from the NFIP.

Exception: (See Additional Underwriting Requirements Rule.)

R. Material Misrepresentation, Insurance Fraud or Arson

Applicants canceled or non-renewed for material misrepresentation in the past seven (7) years orinsurance fraud in the past fifteen (15) years or convicted of arson in the past twenty-five (25) years.

S. Property Constructed Over Water

Any insured location with a structure constructed partially or entirely over water. (e.g. boat houses,etc.)

Note: Ineligible structures do not include piers and docks.

T. Property Built On Landfills – Refuse

Properties built on landfills previously used for refuse.

U. Inaccessible Property

Properties not readily accessible year round to fire department equipment (e.g. isolated propertyincluding barrier islands not connected to the mainland by a road).

Exception: Barrier Islands with a responding fire station located on the island.

V. Excessive, Unusual or Extra-Hazardous Exposure

1. Properties with excessive, unusual or extra-hazardous liability exposure, (e.g. empty in-groundpools, skateboard or bicycle ramps, trampolines, vicious or exotic animals kept on premises).

Exception: May be eligible for a Dwelling Fire Policy excluding Liability coverage.

Properties with excessive, unusual or extra-hazardous property exposure (e.g. excessive crackingin the foundation, walls or roof, slab cracks, out of level foundations, flooring, walls and/or roof,unexplained depressions, active sinkholes or depressions on the premises, or unrepaired sinkholedamage).

Exception: May be eligible for an HPC HO-3 policy that does not include coverage for sinkhole.

W. Residential Family Day Care Homes

Note: Eligible conditions only apply to policies assumed from Citizens Property InsuranceCorporation that also had this endorsement at the time of the assumption. Policies that did not havethis endorsement at the time of assumption are not eligible.

1. Eligible – Including Personal Liability

a. Registered or LicensedFamily Day Care Homes registered in Florida or licensed in Counties requiring licensure areeligible for a Homeowners Policy including Personal Liability coverage.

b. Registration or License Not RequiredFamily Day Care Homes not required by law to be registered and/or licensed are eligible for aHomeowners Policy.

2. Eligible – Not Including Personal Liability

A separate unattached dwelling used for residential purposes which are located on the residencepremises where a day care business is located, may be insured under the Dwelling fire Programexcluding Personal Liability. All other non-residential buildings or structures located at the“described location” used in the day care business are ineligible for coverage.

3. Ineligible

a. Operating In Violation of LawHome Day Care operations not registered where required by Florida law or not licensed inCounties that require licensure, are ineligible for any coverage.

b. Commercial OperationAny child care operation not included within the provisions of this rule.

4. Definition – Family Day Care Home

“Family Day Care Home” means an occupied residence in which child care is regularly providedfor children for more than one unrelated family and which receives a payment, fee, or grant forany of the children receiving care, whether or not operated for profit.

A Family Day Care Home shall be allowed to provide care for only one of the four followinggroups of children, which includes those children under 13 years of age who are related to thecaregiver:

a. A maximum of 4 children from birth to 1 year old.b. A maximum of 3 children from birth to 1 year old, and other children, for a maximum total of

6 children.c. A maximum of 6 preschool children if all are older than 1 year old.d. A maximum of 10 children if no more than five are preschool age and of those five, no more

than 2 are under 1 year old.Note 1: “No Coverage For Home Day Care Business” mandatory exclusion endorsement, HO 04 96applies.

Note 2: If Personal Liability Coverage is provided under Paragraph “A”, a copy of a Certificate ofInsurance to Heritage from the insurer providing Commercial Liability on the Family Day Care Homeat limits equal to or greater than Heritage Personal Liability limits of liability is required.

X. Sinkhole

1. Any risk in which the insured location, including the residence premises, other structures, andgrounds to be insured has ever experienced a Sinkhole loss or Catastrophic Ground Cover Collapseloss, and the loss payments made by any insurer equals the policy limits for property damageprovided under Coverage A; or

2. Any risk in which the insured location, including the residence premises, other structures, andgrounds to be insured has ever experienced a partial Sinkhole loss or Catastrophic Ground CoverCollapse loss and the property has not been repaired and stabilized in accordance with theengineering recommendations upon which any payment or policy proceeds were based.

Y. Multiple Mortgages

Properties with four or more mortgages when the additional mortgagees are not governmentbacked loan programs such as FHA, VA, Fannie Mae, Freddie Mac, etc.

Z. Opening Protection

Properties insured under an HPC HO-3 with an insured value of $750,000 or more and located in thewind-borne debris region without opening protection as required by the Florida Building Code forthat area are ineligible. The OIR-B1-1802 must be completed and signed by a qualified inspector inorder to provide evidence of eligibility.

Rule 208. INSPECTIONS

A. An inspection requiring interior and exterior access may be conducted on any property insured byHeritage. Inspection information can verify wind-resistive features, risk characteristics, and/orgeneral eligibility for coverage.

B. Failure to respond to inspection requests or refusal to allow an inspection will result in one of thefollowing:

1. Removal of wind-mitigation credits if the inspection is intended to validate wind-mitigationfeatures. Credits are removed back to the inception date of the credit for existing policies.

2. Cancellation or non-renewal if the inspection is intended to validate eligibility for coverage.Cancellations and non-renewals are handled in compliance with statutory requirements andpolicy provisions.

Rule 209. STANDARD POLICY COVERAGE

Standard Policy Coverage – Declarations

The following condition shall be included in or endorsed to each policy issued:This policy is issued on behalf of Heritage Property & Casualty Insurance Company, and by acceptanceof this policy you agree:1. That the statements in the Declarations are your representations;2. That this policy is issued in reliance upon the truth of those representations; and3. That this policy represents the only agreements (relating to this policy), existing between you and

Heritage or any of our Agents.

Rule 210. CANCELLATIONS AND NONRENEWALS

A. If insurance is canceled, the additional or return premium will be computed on a pro-rata basis,subject to the minimum premium requirement. The Company will disregard February 29 in leapyears when determining pro-rata earned premiums.

B. With respect to any residential policy covering a risk with a dwelling limit of at least $100,000 and forwhich a 2% minimum deductible is available, the Company will not non-renew such policy for thepurpose of reducing hurricane loss for one (1) renewal period.

Rule 211. COMMISSIONS

The rate of commission payable to Agents for all coverage shall be contractual. Commissions as outlinedabove shall be an Agent’s only remuneration.

Note 1: No commissions shall be payable on any Florida Hurricane Catastrophe Fund PremiumRecoupment Surcharge, Florida Insurance Guaranty Association (FIGA) surcharge, the two dollar ($2)Emergency Management Preparedness & Assistance Trust Fund Surcharge (EMPA), Managing GeneralAgent (MGA) Fee and other charges as may be provided for by Florida Statute.

Note 2: In the event any policy premiums are CHARGED OFF, commission shall only be paid oncollected earned premiums.

Rule 212. POLICY PERIOD, MINIMUM PREMIUM AND WAIVER OF PREMIUM

A. All policies shall be issued for a one (1) year term at premiums applicable on the effective date of thepolicy term.

B. The policy may be extended for successive policy periods by extension certificate based upon thepremiums, forms and endorsements then in effect for Heritage.

C. The minimum policy writing premium applicable to all policy forms shall be fifty dollars ($50).

D. Additional or return premiums of five dollars ($5) or less shall be waived. The Company shall grantany return premium due if requested by the insured.

Rule 213. SECONDARY RESIDENCE PREMISES

Homeowners coverage on a secondary residence premises must be provided under a separate policy.

Rule 214. METHOD OF PAYMENT

Heritage accepts only the following methods of payment:

A. Full Payment Plan – 100% of the total policy premium is due by the renewal effective date of thepolicy.

B. Quarterly Payment Plan

1. 40% of the total policy premium (plus $3 installment fee and a $10 one time service fee) due bythe effective date of the policy or the date of issuance, whichever is earlier.

2. 20% of the total policy premium (plus $3 installment fee) due 90 days after the policy effectivedate.

3. 20% of the total policy premium (plus $3 installment fee) due 180 days after the policy effectivedate.

4. 20% of the total policy premium (plus $3 installment fee) due 270 days after the policy effectivedate.

C. Semi Annual Payment Plan

1. 60% of the total policy premium (plus $3 installment fee and a $10 one time service fee) due bythe effective date of the policy or the date of issuance, whichever is earlier.

Lienholders and Mortgagees (E.g. Escrow) are not eligible for the Quarterly or Semi Annual paymentplans.

When a payment instrument is returned because of insufficient funds to pay it, the Company mayimpose a charge of $15.

Please note Heritage does not accept Premium Finance Agreements.

Rule 215. ROUNDING OF PREMIUM

Each premium shown on the policy and endorsements will be rounded to the nearest whole dollar. Apremium of fifty cents ($0.50) or more will be rounded to the next higher dollar. In no event will BasePremium Calculation for any coverage be less than one whole dollar ($1).

In the event of cancellation by the Heritage, the return premium will be carried to the next higher wholedollar.

Rule 216. POLICY CHANGES

A. If insurance is increased or reduced, the additional or return premium will be computed on a pro-rata basis.

B. All changes are made using the rules and rates in effect at the latest renewal date.

C. Hurricane deductible options may only be amended effective at the normal policy renewal date.

Exception: If a lower hurricane deductible is requested and the policy has sustained a hurricane loss inthe current calendar year, the lower deductible will not become effective until January 1 of thefollowing calendar year.

D. A policy may not be written to circumvent this restriction.

Rule 217. TRANSFER OR ASSIGNMENT

Transfers or assignments are not available.

Rule 218. RESTRICTION OF INDIVIDUAL POLICIES

If a policy would not be issued because of unusual circumstances or exposures, the named insured mayrequest a restriction of the policy provided no reduction in the premium is allowed. Such requests willbe referred to the Company.

Rule 219. MANUAL PREMIUM REVISION

A manual premium revision will be made in accordance with the following procedures.

A. The effective date of such revision will be as announced.

B. The revision will apply to any policy or endorsement in the manner described in the announcementof the revision.

C. Unless otherwise provided at the time of the announcement of the premium revision, the revisionwill not affect in-force policy forms, endorsements or premiums, until the policy is renewed.
