Reviews and Overviews Mechanisms of Psychiatric Illness A Translational Neuroscience Approach to Understanding the Development of Social Anxiety Disorder and Its Pathophysiology Andrew S. Kalin, M.D. This review brings together recent research from molecular, neural circuit, animal model, and human studies to help un- derstand the neurodevelopmental mecha- nisms underlying social anxiety disorder. Social anxiety disorder is common and debilitating, and it often leads to further psychopathology. Numerous studies have demonstrated that extremely behaviorally inhibited and temperamentally anxious young children are at marked risk of de- veloping social anxiety disorder. Recent work in human and nonhuman primates has identied a distributed brain network that underlies early-life anxiety including the central nucleus of the amygdala, the anterior hippocampus, and the orbitofron- tal cortex. Studies in nonhuman primates have demonstrated that alterations in this circuit are trait-like in that they are stable over time and across contexts. Notably, the components of this circuit are differentially inuenced by heritable and environmental factors, and specic lesion studies have demonstrated a causal role for multiple components of the circuit. Molecular stud- ies in rodents and primates point to disrupted neurodevelopmental and neuro- plastic processes within critical components of the early-life dispositional anxiety neural circuit. The possibility of identifying an early-life at-risk phenotype, along with an understanding of its neurobiology, provides an unusual opportunity to conceptualize novel preventive intervention strategies aimed at reducing the suffering of anxious children and preventing them from de- veloping further psychopathology. Am J Psychiatry Fox et al.; AiA:112 S ocial anxiety disorder is highly prevalent and debi- litating (1), with an estimated prevalence of about 18% (2, 3). The disorder is characterized by marked fearfulness and anxiety in social or performance situations, frequently resulting in avoidance and signicant disability. In addi- tion to suffering with social anxiety disorder, aficted individuals often develop comorbid depressive and sub- stance use disorders (1). Data suggest that social anxiety disorder is 20%240% heritable, with environmental fac- tors accounting for the remaining variability (4). Although social anxiety disorder is commonly diagnosed during ad- olescence, a period during which teenagers attempt to ad- just to social change, it can start before adolescence, and its antecedents often manifest early in life (5, 6). Accu- mulating evidence suggests that a behaviorally inhibited or temperamentally anxious disposition during childhood can lead to the development of social anxiety disorder (5, 7). This at-risk phenotype is characterized by heightened, but nonpathological, levels of anxiety and may consti- tute a prodromal phenotype for social anxiety disorder. Although this phenotype is moderately stable over the course of development, it does not consistently predict the development of social anxiety disorder, nor is it in- variant. This suggests that early-life interventions targeting highly anxious children have the potential to prevent the development of full-blown social anxiety disorder and its common comorbidities. In this review, we take a cross-species approach to examine the behavior, neural circuits, and molecular sys- tems that underlie the risk of developing social anxiety disorder. We discuss the biological basis of temperamental anxiety using data from humans and rodents, but we focus on insights gleaned from our studies of nonhuman pri- mates. Rhesus monkeys are ideal for studying mechanisms underlying human development because, given their rel- atively recent evolutionary divergence from humans, the organization and function of the neural systems relevant to human anxiety, including the amygdala and prefrontal cortex, are conserved (8, 9). Rhesus monkeys and humans also share similar complex social environments that rely on parent-child bonding and peer relationships. These early relationships can both encourage and discourage the adaptive social and emotional learning that helps regulate anxiety and promote survival. Our aim is to understand how inborn and environmental inuences converge on the specic biological systems that underlie extreme tem- peramental anxiety and the risks it confers.
