Parent/Guardian Agreement: I authorize the administration of any first aid treatment necessary at The Great Exchange event in case of medical emergency and give permission to the physician selected to hospitalize and secure proper treatment for my child. Every effort will be made to contact the parent or guardian before such action. While every precaution will be taken to ensure the welfare and protection of the student, the staff and volunteers are hereby released from any liability in the event that the student is involved in an accident. I also understand that if my child refuses to follow basic rules of respect, I will be called to come get my child if necessary.
