The 2013 theme is, "Celebrating America’s Health Centers: Transforming Health Care in Our Local Communities." There is no doubt that in our community we have reason to celebrate based on the great steps partners are taking to truly transform local health care. Case in point: our new program with Wentworth Douglas Emergency Department. This program puts Lisa Duffey, a new Community Outreach Worker in Wentworth's Emergency Department to meet with patients prior to their discharge from the ER. Lisa helps connect patients with more cost effective medical and dental preventative care by talking with them about their particular situations and point them towards the services Goodwin can provide. While the patient is still in the ER, Lisa is empowered to guide patients through choosing a primary care provider and scheduling follow-up care at Goodwin Community Health. This initiative not only connects patients with the preventative medical and dental care they need, it changes patient behavior in a way that dramatically shifts costs for the overall health care system. Nationally, Community Health Centers are credited with having produced $24 billion in annual health system savings. Now that is something to celebrate... Happy National Community Health Center Week! MAPS A Million! Goodwin Community Health'sMedication Assistance Program (MAP)aims to help patients accessmedications at affordable rates. Thisyear we reached $1 Million in free anddiscounted meds for patients in need.Since the program began in July of2000, MAP has facilitaed $6,977,508.87in savings to our patients! A big thankyou to our own Linda Jewell who makesthe MAP program possible!

ROCK with the DOCS Thanks to more than 300 of YOU forcoming out for our first ever ROCK withthe DOCS event. The Rivermill atDover Landing had a packed dancefloor and your generosity raised $1,000for patient programs for local families inneed. Thank you! And don't put yourdancing shoes away... we are preparinganother rockin' good time for thisSpring. Stay tuned for details.

Health Centers not only locally, but nationwide. Dr.Thompson works tirelessly with the CommunityHealth Access Network to help create new forms,screens, and to troubleshoot software problems. Hehas designed templates to help providers becomemore efficient and incorporate best practices. Anynew form he creates, he posts on his computercompany website and anyone is allowed to usethese – for free. In 2004, we added oral health careto Goodwin Community Health, but our electronichealth record program did not have any screens fordental at the time. Dr. Thompson volunteered tocreate all the screens, including tooth charts andreports. We still use this system today. About fiveyears ago, we had to reduce staff hours due tobudget restraints, but Dr. Thompson was concernedthat if we reduced nursing coverage, patientswouldn’t receive the care they needed. For a fullyear, he covered the portion of salaries that weregoing to be reduced. When we initiated our capitalcampaign in 2010 for our new facility, Dr.Thompson’s company was the first to donate moneytoward our new building. Despite all that he hasdone for our patients, and other FQHCs, Dr.Thompson is soft spoken and humble. He doesn’tlike recognition or being the center of attention. Yet,I can’t help but bring his donations and support of

The RX Abuse Epidemic & You Over 70 peoplefrom around theregion and Stategathered on June7th to hear aboutthe current trendsand solutionsrelated toprescription drugmisuse. The eventwas put on byONE Voice forSoutheastern NH and members of the Rx Taskforce. Taskforce members teamed up to produce a shortvideo illustrating the importance of screening inmedical settings. Video Talent New Englanddonated substantial time to this project and workedwith Jason Lucey, APRN, Wendy Pressly, RN andTom Barnes of Pain Care, who wrote the script. The9 minute video demonstrates how medical providerscan implement strategies to prevent drug diversionamong patients while awaiting the implementation ofthe NH Prescription Monitoring Program. Couch to 5K Staff and patients completed a 6 week programthat physically prepared them to participate inGoodwin's own Father's Day 5K. The grouplearned about ways to become more active whichincluded participation in various cardio exercisesand yoga at The Works Health and Fitness Center.Congratulations to the Couch to 5K Group!

AIM HI Group Medical Visits willbegin September 4th. This groupwill have their health vitalsmonitored throughout the 6 weeksession while participating invarious healthy activities.Insulin Education with LisaGarneau RN/CDE September25th, 1:30-3:00 PM. This groupwill learn correct insulinadministration as well as otherdiabetes education.Vision Screening on September21st to our qualified patients atone of our neighborhoodproviders.Join the Wellness Focus Group!Next meeting is August 28th,1:30-3:00 PM. The groupdiscusses topics related to healthand wellness while offeringsupport to achieve a healthylifestlye.

Goodwin Earns Level 3 PatientCentered Medical Home The NationalCommittee for QualityAssurance announcedthat GoodwinCommunity Health hasreceived Recognitionfrom thePatient-CenteredMedical Home 2011program for usingevidence-based,patient-centeredprocesses that focus onhighly coordinated care and long-term participativerelationships.The patient-centered medical home is a model ofcare emphasizing care coordination andcommunication to transform primary care into “whatpatients want it to be.” Research shows that medicalhomes can lead to higher quality and lower costs,and improve patients’ and providers’ reportedexperiences of care.To receive recognition, which is valid for three years,Goodwin Community Health demonstrated the abilityto meet the program’s key elements embodyingcharacteristics of the medical home.
