Dr. Holcomb established that a quorum was present and the meeting that was scheduled to

begin at 12:00 p.m., was called to order at 12:05 p.m.

Open Session

2. Minutes from the June 5, 2009 board meeting:

Dr. Nalley made a motion to approve as amended to reflect that Dr. Godfrey was meeting

with Secretary of State Handel before arriving at 2:05 p.m. Dr. Hadley seconded the

motion and it carried unanimously. Dr. Holcomb commended the staff for the quality of

the minutes.

3. C.E. Audit Committee – Drs. Henry Cook, Sr. & Barry Stacey – No Report.

Dr. Holcomb discussed updating the CE form and having made available on the website. A

column will be added for on-site – vs. – not on site and if you hold an anesthesia permit – please

specify hours to be used for the required CE. Also, add to the CE block, ACLS and PALS. Dr.

Holcomb asked that Ms. Martin make the amendments and resend the CE reporting form to all

Board members.

4. Conscious Sedation/General Anesthesia Committee – Drs. Clyde Andrews & Barry Stacey

Dr. Andrews reported that the issue that is holding up this issue is the pediatric sedation portion

of the rule revisions. Dr. Andrews has been in touch with Dr. Lee Getter at MCG for his opinion

on training for pediatric sedation. Dr. Andrews also mentioned that the committee is ready for

the public forum as soon as the date, time and location are set. Dr. Holcomb asked that Dr.

Andrews come up with three dates for location in Macon and time. The committee will have a

forum in the next sixty days. The Board will provide the Executive Director with names of an

interested parties list for the forum.

5. Credentialing Committee – Dr. Becky Carlon - No Report

Dr. Stacey asked about an individual who has had Georgia practice and whether or not a person

could qualify for a credentials license who has practiced for 5 years in Georgia under a faculty

license. Dr. Nalley had additional questions about the same issue. Dr. Carlon stated that

individuals could always apply for rule variance/waiver requests and can be handled on a case-

by-case basis. Dr. Holcomb asked that any recommendations for revisions to be made to the

credentialing rules to the Executive Director to be provided to the committee.

6. Dental Hygiene Committee: - Ms. Pamela Bush, RDH – Ms. Bush reported that the

committee has not met for a couple of months.

Ms. Bush attended the Hygiene ERC meeting. Changes will be made to the way examiners see

the examination. Ms. Bush supports the changes and urges the Board to support the changes.

There will be a change to implement 3 examiners check in and out. The duties to team captain

will change.

Ms. Bush forwarded to the GDHA the link for the Volunteer Reporting form.

Ms. Bush has received e-mails from faculty members about the anesthesia issue. Ms. Bush is

asking for a follow-up from the Board on this issue.

The Hygiene Committee will meet at the conclusion of the September 11, 2009 Board meeting.

7. Examination & CRDTS Committee Reports: – Dr. Logan Nalley, Jr.

Dr. Nalley reported the MCG exam is August 14-16, 2009. Dr. Holcomb urged all members to

make their arrangements for the annual meeting.

8. Investigative Committee – Dr. Logan Nalley, Jr. – No Open Session Report.

Dr. Nalley asked for an update on Ms. Mickler’s condition. An update was provided by Ms.

Martin. Dr. Nalley asked that an open complaint status report be provided on a monthly basis.

9. Legislative Committee – Drs. Henry Cook & Steve Holcomb – Dr. Nalley reported that Drs.

Holcomb, Andrews and he are planning to visit with MS. Handel on August 26, 2009. Dr.

Holcomb encouraged any board members who had issues that they wanted shared with the

Secretary of State Handel to send them to him or Drs. Nalley and Andrews.

10. Licensure Overview Committee - Dr. Issac Hadley - No Report

Ms. Richardson reported that she drafted a reporting form to be used for the Licensure Overview

Committee and is finalizing the form to be utilized.

11. Rules Committee – Dr. Clyde Andrews – Dr. Andrews reported about the prior plans for a

committee meeting by conference call. Dr. Andrews and Executive Director, Anita Martin, will

work to establish a committee meeting to be held in Macon.

12. CRDTS Examination Report – Dr Clark Carroll – Executive Director, Anita Martin, will

send a reminder e-mail. Dr. Nalley requested that the Georgia Board members will get together

at the meeting to meet and discuss issues of interest. Dr. Holcomb instructed all board members

to e-mail Dr. Nalley when they plan to arrive and the programs they plan to attend. Dr. Nalley

will coordinate a meeting.

13. Request from Denise Howard, EMT, ECSI information on Professional CPR and

eACLS. – Dr. Stacey made a motion to approve. Dr. Nalley seconded the motion and it carried

unanimously.

14. Correspondence from Dr. Gene A. Kramer, Secretary, Joint Commission on National

Dental Examinations re: Nominations for the Public Member of the Joint Commission on

National Dental Examinations. – The Board viewed as informational. Ms. Richardson stated

she will review and let the Board know if she is interested.

15. Consider for ratification licenses that have been administratively issued: - Dr. Andrews

made a motion, Dr. Nalley seconded and the Board voted to ratify the following newly issued

Executive Director’s Open Session: Ms. Anita Martin

Ms. Martin stated that during Dr. Godfrey’s term as President 53 wall certificates (29

Hygienist and 24 Dentist) were issued. This matter had been discussed with Mr. Vaughn

and he stated that if we received complaints about this issue a revised certificate would be

issued at no charge to the licensee. Dr. Holcomb recommends that the individuals who

were issued a license during this time period be notified that they are incorrect and that a

correction will be made if so requested. Dr. Nalley stated it was not a major issue and the

Board would wait on a complaint, Dr. Hadley conferred with it not being a major issue.

Dr. Nalley made a motion to issue corrected licenses to these individuals. Dr. Carlon

seconded the motion and the Board vote was unanimous.

Ms. Martin stated that she would be meeting with the Executive Director of the Medical

Board, Ms. LaSharn Hughes when she came down to the Macon office; they are moving

to electronic applications and utilization of the Epiware site.

Ms. Martin stated that Investigative Assistant Judy Mickler plans to retire by October 1,

2009. Ms. Martin states we will have an intern in the position for 20 hours per week until

the position is filled.

Miscellaneous:

Dr. Godfrey stated that he has a report for the Board concerning his meeting with

Secretary of State Handel. He does not at this point have the report finalized but will have

a completed report for the next meeting.

Dr. Nalley asked about the 2009 Board meeting dates. The Investigative Committee

meeting will change to December 4, 2009 and have both Investigative Committee

meeting and the Board meeting as a Conference Call. The Investigative Committee will

schedule appointments for the morning of December 4, 2009. If a full Board meeting is

required for December 4, 2009, the meeting can begin at 12:00 p.m.

December 11, 2009 is the CRDTS re-exam if necessary.

Dr. Holcomb asked about the status of the ULP hearing notices. Mr. Dean and Ms. Fisher

are working on them. Mr. Dean will check on the status of the notices.

The Board asks that MCG provide names, license numbers and practice locations for all faculty

and resident practicing at off-site locations. Executive Director, Anita Martin, will draft a letter

and send to Dr. Holcomb.

Executive Session

Dr. Nalley made a motion, Dr. Andrews seconded, and the Board voted to enter into Executive

Session in accordance with O.C.G.A. §43-1-19(h)(2), 43-11-47(h) and §43-1-2(k) to deliberate

on applications and enforcement matters and to receive information on applications, investigative

reports and the Assistant Attorney General’s report. Voting in favor of the motion were those

present who included Drs. Carlon, Carroll, Godfrey, Hadley, Stacey, Ms. Bush and Ms.

Richardson. The Board concluded the Executive Session in order to vote on these matters and

continue with the public session.

Applications/Licensure:

1. L.H.J. – Dental Hygiene Applicant. – Approved

2. D.M.X. – Dental Hygiene Applicant – Approved

3. A.J.K. – Dental Hygiene Applicant. – Applicant must provide a written response about

the 01-08-08 DUI before the Board will consider the matter further.

4. B.A.L. – Dental Applicant. – Schedule for an LOC appointment.

J.K.S. – Dental Licensee – Provide a Private Consent Order allowing his practice after August

31, 2009 until Dr. S.’s U.S. residency status has been renewed. Licensee must provide an

updated residence status information within 6 months. Ms. Martin questioned about the federal

law requiring valid status to be issued a license. Mr. Dean advised that this should not be a

problem.

Dr. Nalley motioned, Dr. Carlon seconded and the Board voted to approve all recommendations

made in Executive Session.

The Board meeting adjourned at 1:45 p.m.
