This book will help clinicians acquire and develop the processes and skills of values-based practice. The aim of most patient–clinician consultations is to improve health outcomes. Often they succeed, and patients are satisfi ed and empowered. However, some consultations are unsatisfactory and result in failure to improve health outcomes: dissatisfaction on the part of patients, carers, or clinicians.

When consultations fail to achieve the desired results, the cause is not usually a failure of evidence-based practice. Today’s clinicians are trained in evidence-based medicine, educated, updated, and appraised. The most likely reason why things go wrong is a failure of values-based practice – not ascertaining the relevant values perspectives, and not acting on them in a coherent and purposeful manner.

If you rehearse and practice the elements of values-based practice detailed in this book, you will fi nd your consultations more personally rewarding, and your patients are likely to derive more benefi t.

K. W. M. (Bill) Fulford is Emeritus Professor of Philosophy and Mental Health in the University of Warwick Medical School, Fellow of St. Cross College, and Member of the Faculty of Philosophy, University of Oxford. Between 2005 and 2011, he was Special Advisor for Values-Based Practice for the Department of Health.

Ed Peile is Emeritus Professor of Medical Education in the University of Warwick Medical School. A Fellow of three Medical Royal Colleges, he was awarded the President’s Medal of the Academy of Medical Educators in 2009 for his lifetime contribution to medical education.

Heidi Carroll is a General Practitioner in Aberdeenshire, UK.

The provision of care within the context of the modern health service environment involves a wide range of professionals. The health care team might include general practitioners, nurses, midwives, hospital doctors, physiotherapists and other allied health professionals, as well as receptionists and practice managers. To optimise delivery of care at both individual and population levels, team members must work collaboratively with colleagues in their own profession and others.

Ordinarily, responsible conduct of research (RCR) “training” consists of lectures accompanied by generic exercises on “core” topics. Research Ethics takes a novel, philosophical approach to the RCR and the teaching of moral decision-making. Part A introduces egoism and explains that it is in the individual’s own interest to avoid misconduct, fabrication of data, plagiarism, and bias. Part B takes up contractualism and covers issues of authorship, peer review, and responsible use of statistics. Part C introduces moral rights as the basis of informed consent, the use of humans in research, mentoring, intellectual property, and confl icts of interests. Part D uses two-level utilitarianism to explore the possibilities and limits of the experimental use of animals, duties to the environment and future generations, and the social responsibilities of researchers.

This book presents a fresh approach to research ethics and will engage the moral imaginations of graduate students in all disciplines.

Ordinarily, responsible conduct of research (RCR) “training” consists of lectures accompanied by generic exercises on “core” topics. Research Ethics takes a novel, philosophical approach to the RCR and the teaching of moral decision-making. Part A introduces egoism and explains that it is in the individual’s own interest to avoid misconduct, fabrication of data, plagiarism, and bias. Part B takes up contractualism and covers issues of authorship, peer review, and responsible use of statistics. Part C introduces moral rights as the basis of informed consent, the use of humans in research, mentoring, intellectual property, and confl icts of interests. Part D uses two-level utilitarianism to explore the possibilities and limits of the experimental use of animals, duties to the environment and future generations, and the social responsibilities of researchers.

This book presents a fresh approach to research ethics and will engage the moral imaginations of graduate students in all disciplines.

Every accredited American hospital is required to have a mechanism for handling ethical concerns; most hospitals satisfy this requirement by constituting an institutional Healthcare Ethics Committee (HEC), a pattern which is repeated in most western countries. This book will help clinicians acquire and develop the processes and skills of values-based practice. The aim of most patient–clinician consultations is to improve health outcomes. Often they succeed, and patients are satisfi ed and empowered. However, some consultations are unsatisfactory and result in failure to improve health outcomes: dissatisfaction on the part of patients, carers, or clinicians.

When consultations fail to achieve the desired results, the cause is not usually a failure of evidence-based practice. Today’s clinicians are trained in evidence-based medicine, educated, updated, and appraised. The most likely reason why things go wrong is a failure of values-based practice – not ascertaining the relevant values perspectives, and not acting on them in a coherent and purposeful manner.

K. W. M. (Bill) Fulford is Emeritus Professor of Philosophy and Mental Health in the University of Warwick Medical School, Fellow of St. Cross College, and Member of the Faculty of Philosophy, University of Oxford. Between 2005 and 2011, he was Special Advisor for Values-Based Practice for the Department of Health.

Ed Peile is Emeritus Professor of Medical Education in the University of Warwick Medical School. A Fellow of three Medical Royal Colleges, he was awarded the President’s Medal of the Academy of Medical Educators in 2009 for his lifetime contribution to medical education.

Heidi Carroll is a General Practitioner in Aberdeenshire, UK.

The provision of care within the context of the modern health service environment involves a wide range of professionals. The health care team might include general practitioners, nurses, midwives, hospital doctors, physiotherapists and other allied health professionals, as well as receptionists and practice managers. To optimise delivery of care at both individual and population levels, team members must work collaboratively with colleagues in their own profession and others.
