You should be able to put your pen down and trace your insulative barrier around your whole building (at all scales) without having to pick up your pen. If you have to pick up your pen or something conductive is crossing through that barrier, you have a thermal bridge.
To minimize the impact of it:
1. Break the heat flow with thermal breaks
2. Minimize the penetration as much as possible (IE discontinuous elements)
3. Where something penetrates your insulative barrier, use a material that has a lower conductance (IE stainless steel rather than aluminum)
