IntroductionAlliance Garments Ltd manufacturing complex was surveyed for fire safety on5/5/2014 by Hughes Associates, Inc.

The purpose of the survey was to identify significant fire safety issues and toprovide recommendations for remediation based on applicable standardsspecified by the Accord. The scope of this initial fire safety inspection waslimited to the review and identification of major fire safety issues. Theinspection did not include identification of minor deficiencies, which will befurther addressed as part of follow-up inspections.

LimitationsThe information in this fire safety inspection report was obtained during a visitto the facility and during interviews with local factory management. It has notbeen possible to provide independent verification for all the information anddata collected, and, therefore, Hughes cannot accept general responsibility foromissions or errors arising from inaccuracies in this report from the informationobtained.

The findings and recommendations in this report are not intended to imply,guarantee, ensure or warrant compliance with any government regulations.Additionally, the results do not imply in any way that compliance with thefindings or recommendations as stated in this report will eliminate all hazards,risks or exposures or that hazards, risks or exposures not referred to in thisreport do not exist. Egress FeaturesTwo (2) exit stairs serve the building. Egress from the ground floor is provided by three (3)exits. Each floor had an adequate number of exits with sufficient exit capacity based onoccupant load. All exit stairs were not remotely located. All exits discharge to the exterior of thebuilding. All exit stairs are open to the floors due to unprotected openings.Sliding gates or roll-up doors were used at building exits, exit stairs and exit discharge. Egressdoors with locking features were present at exit stairs and exit discharge. All gates and doorswere open and unlocked during the inspection. Emergency evacuation plans were located atexit stairs. Fire drills are conducted monthly and records were maintained.

Fire Alarm SystemA manual fire alarm system was provided in the building. It consisted of manual notificationswitches. A single horn unit were provided at the second floor. It was asserted that theactivation devices are interconnected so the activation of a single device provides audiblenotification for the whole building. There is no fire alarm panel or system supervision. Therewere no records or assertions of regular testing or maintenance of the fire alarm system.

Fire Detection SystemSingle station smoke alarms are located in various places throughout the factory. These smokealarms are not tied into the fire alarm system. In the event they detect smoke, they will providea local alert sound. There were no records or assertions of regular testing or maintenance ofthe fire detection system.

Emergency Lighting & Exit SignsEmergency lighting was located inside and outside the exit enclosure walls. Lighting consistedof multiple compact florescent lights that have an alternate “Instant Power Supply” (IPS). Eachfloor had a number of illuminated exit signs (a mix of directional and non-directional) locatedalong the exit access and at exits on each floor. The factory reports that these are serviced bynormal electricity as well as “Instant Power Supply” (IPS). There were no records or assertionsof regular testing or maintenance of the emergency lighting system.Emergency lighting has on/off switches throughout the factory.

Findings & Recommendations

Table 1 summarizes the major fire safety issues identified during the inspection.Recommendations have been provided to address each issue.

An implementation schedule shall be developed by the factory to remediate each of thefindings. The specific timing of improvements, including any requested extensions due todesign / installation constraints, shall be submitted to the Accord for approval.
