I, the undersigned, declare that I have received the Nuseed Licence Induction Training in the operational procedures for research conducted under DIR123 and all relevant Information regarding biosafety measures as specified in the Gene Technology Act 2000 and the Gene Technology Regulations 2001 and are aware of all corresponding requirements Imposed for the conduct of this research.

I acknowledge that I have received, read and understood the conditions DIR123 and I agree to be bound by these conditions and those of the Gene Technology Act 2000 and the Gene Technology Regulations 2001 and comply with all requirements of the legislation regulating the conduct of research involving genetic manipulation.

This record of training also acknowledges that my 'Personal Information', as defined by the Act, may be provided to the Regulator through this record.
