Procedure for-Cimmpleting Profile Sheet
1. When administering a scale, mark "yes"for all items which you determine a childhas already achieved. In the ComMentscolumn, state 9?eyond this stage."-
2. When dciiii0 both 0 -4 and 4-7 scales onone child, remember to'exclude 4 yea(48 month) level items on the 4-7scaieu_il7e. begin wih 5 year levelitems. This avoids 4uplication.
3. On the profile sheet,' -use a differentcolor pen for pre and post tests. ,In.the upper right hand carner of the sheetnote-the color usedifor each.
*4. A number typed in a box is the numberof items_for that age bracket. Forexample, there are foxir itemSitin the48-60 month age bracket'for the' Percep-tual skills section.
